This paper is devoted to the qualification of Lyme disease in France. Firt vectorial disease in the northern hemisphere, Lyme diseas is transmitted by a genus of tick called Ixodes (in Europe by Ixodes ricinus ; in Asia by Ixodes persulcatus ; in United States and in Canada by Ixodes scapularis). Concerned only with human health and limited to Lyme, a first framing has grown around physicians and patients which are involved in a controversy. Progressively, a second framing has emerged, called vectorial and structured around ecologists, normally not involved in human health. Based on the vector, this framing includes other diseases transmitted by Ixodes ricinus which, during the last 30 years, have been discovered : Tick-Borne Encephalitis, Babesia divergens, Bartonella infection, Tularemia, etc. Linked to these new pathologies, the media coverage of Lyme has finally opened the « vectorial transmission category » – usually associated to mosquitoes and south countries – to ticks and diseases from the north. A third framing dedicated to animal reservoir and called zoonotic is emerging at last. We can see in that framing overlapping a kind of “ecologization” of sanitary issues. [less ▲]

Introduction: There is a growing body of evidence to suggest that chronic silent inflammation is a key feature in abdominal obesity, metabolic syndrome, type 2 diabetes (T2DM) and cardiovascular disease (CVD). These observations suggest that pharmacological strategies, which reduce inflammation, may be therapeutically useful in treating obesity, type 2 diabetes and associated CVD. Area covered: The article covers novel strategies, using either small molecules or monoclonal antibodies. These strategies include: approaches targeting IKK-b-NF-kB (salicylates, salsalate), TNF-alpha (etanercept, infliximab, adalimumab), IL-1beta (anakinra, canakinumab) and IL-6 (tocilizumab), AMP-activated protein kinase activators, sirtuin-1 activators, mammalian target of rapamycin inhibitors and C-C motif chemokine receptor 2 antagonists. Expert opinion: The available data supports the concept that targeting inflammation improves insulin sensitivity and beta-cell function; it also ameliorates glucose control in insulin-resistant patients with inflammatory rheumatoid diseases as well in patients with metabolic syndrome or T2DM. Although promising, the observed metabolic effects remain rather modest in most clinical trials. The potential use of combined anti-inflammatory agents targeting both insulin resistance and insulin secretion appears appealing but remains unexplored. Large-scale prospective clinical trials are underway to investigate the safety and efficacy of different anti-inflammatory drugs. Further evidence is needed to support the concept that targeting inflammation pathways may represent a valuable option to tackle the cardiometabolic complications of obesity. [less ▲]

This paper proposes a novel approach to detect differential item functioning (DIF) among dichotomously scored items. Unlike standard DIF methods that perform an item-by-item analysis, we propose the “LR lasso DIF method”: logistic regression (LR) model is formulated for all item responses. The model contains item specific intercepts, an effect of the sum score and item-group interaction (i.e. DIF) effects, with a lasso penalty on all DIF parameters. Optimal penalty parameter selection is investigated through several known information criteria (AIC, BIC and cross-validation) as well as through a newly developed alternative. A simulation study was conducted to compare the global performance of the suggested “LR lasso DIF” method to the logistic regression and Mantel-Haenszel methods (in terms of false alarm and hit rates). It is concluded that for small samples the LR lasso DIF approach globally outperforms the logistic regression method, and also the Mantel-Haenszel method, especially in the presence of item impact, while it yields similar results with larger samples.
