Gaucher disease is a relatively rare metabolic disease caused by the inherited deficiency of the lysosomal enzyme glucocerebrosidase. Gaucher disease affects multiple organs, among which is the skeleton. Bone involvement occurs frequently in Gaucher disease, and is one of its most debilitating features, reducing the quality of life of patients. Bone status is an important consideration for treatment to ameliorate symptoms and reduce the risk of irreversible complications. We have conducted a systematic review of all the various aspects of Gaucher disease, focusing on different skeletal manifestations, pathophysiology of bone alterations, clinical symptoms, and current diagnostic and therapeutic approaches. BACKGROUND: Given the limited availability of healthcare resources and the recent introduction of new anti-osteoporosis drugs, the interest in the cost effectiveness of drugs in postmenopausal osteoporosis remains and even increases. OBJECTIVE: This study aims to identify all recent economic evaluations on drugs for postmenopausal osteoporosis, to critically appraise the reporting quality, and to summarize the results. METHODS: A literature search using Medline, the National Health Service Economic Evaluation database and the Cost-Effectiveness Analysis Registry was undertaken to identify original articles published between January 1, 2008 and December 31, 2013. Studies that assessed cost effectiveness of drugs in postmenopausal osteoporosis were included. The Consolidated Health Economic Evaluation Reporting Standards (CHEERS) statement was used to assess the quality of reporting of these articles. RESULTS: Of 1,794 articles identified, 39 studies fulfilled the inclusion criteria. They were conducted in 14 different countries and nine active interventions were assessed. When compared with no treatment, active osteoporotic drugs were generally cost effective in postmenopausal women aged over 60-65 years with low bone mass, especially those with prior vertebral fractures. Key drivers of cost effectiveness included individual fracture risk, medication adherence, selected comparators and country-specific analyses. Quality of reporting varied between studies with an average score of 17.9 out of 24 (range 7-21.5). CONCLUSION: This review found a substantial number of published cost-effectiveness analyses of drugs in osteoporosis in the last 6 years. Results and critical appraisal of these articles can help decision makers when prioritizing health interventions and can inform the development of future economic evaluations. [less ▲]

The stay cables on cable-stayed bridges have been the subject of considerable research in order to understand the origins of the dry galloping phenomenon. Surface irregularity is one of the last parameters that have not been thoroughly examined. This paper focuses on static wind tunnel tests on original High Density Polyethylene cable covers in a range of Reynolds numbers from the sub-critical regime to the critical, corresponding to values ranging from Re = 9.6x104 to Re = 3.3x105. The experiment consists in testing cable covers of various diameters in order to investigate the effect of surface irregularity (roughness and circularity defect) on the mechanism of dry galloping excitation. Previous studies reported that dry galloping is caused by the appearance of a negative pressure bubble on one side of the circular cylinder at the critical Reynolds number range, leading to a rapid drop in the drag coefficient and the appearance of a non-negligible lift force. The results of the present investigation demonstrate that there is a clear correlation between the single bubble pressure pattern and the circularity defect along the tube. They further show that surface roughness has little effect on the location of the bubble. The paper also treats the spatial and temporal correlation of the instantaneous pressure pattern along the tube with respect to the circularity defect. [less ▲]

Dans les pages suivantes, je suggère qu’Ehrenfels et Meinong ont apporté certaines améliorations substantielles à la position de Brentano. Ces améliorations résident essentiellement dans les deux points suivants : (1) les volitions se caractérisent par une dimension « active » ou conative qui fait défaut aux sentiments, ce qui interdit de les ranger dans une seule et même « classe » de phénomènes psychiques ; (2) le concept de valeur trouve son origine empirique dans les volitions ou les phénomènes conatifs, non dans les sentiments ou phénomènes affectifs. Le photogramme est, au cinéma, la plus petite unité de prise de vue. Image fixe, unité élémentaire du film, le photogramme forme la cellule figée d’un mouvement qui se reconstitue sous l’effet d’un défilement réglé (24 photogrammes/seconde) par le dispositif technique du cinéma. Image paradoxale en ce qu’elle reste invisible au spectateur malgré sa force iconique, le photogramme trouve cependant, dans le flux qui le conduit insensiblement à la disparition, certaines zones de visibilité qui en révèlent le caractère à la fois fragile et intensif. Nous allons donc envisager certaines pratiques ou certains usages du photogramme cinématographique, tout en essayant de dégager, au départ de ces pratiques, les éléments d’une théorie du photogramme, que Roland Barthes et Sylvie Pierre appelaient déjà de leurs vœux. Des protos-photogrammes et de leur usage scientifique chez Muybridge et Marey, aux usages artistiques contemporains, en passant par des pratiques publicitaires, promotionnelles ou analytiques, nous allons voir que le photogramme reste un objet singulier. It analyses them from a perspective that characterizes FTAs into “WTO-extra” and “WTO-plus” agreements. By this analysis, an “Americanization” of the European interregional strategy is partially verified. Nevertheless, this “Americanization” appears to be partial, due to the inclusion of “regional integration” as a transversal theme in European FTA. [less ▲]

The basic intuition which underlies this interpretation is that the social sciences are best understood as generalizing cultural sciences. It is usual to think of the spectacular development of the Brentano School in Austria as being tied to the rise of a specific research program, which is sometimes dubbed “Austrian phenomenology”. I discuss here some evidences for the existence of such a program. In the discussion, I highlight two claims which I take to be constitutive of the Brentanian phenomenology. The first one is the epistemological claim according to which our concepts all have an empirical origin, which has to be located in the experience we have of our own mental states. The second one is the methodological claim according to which describing a mental phenomenon P is nothing but analysing P, where “analysing P” simply means “distinctly perceiving all the constituents of P”. Although a deeper insight in those two claims would require considerable further investigations, I suggest they should be part of the picture when it comes to adjudicating on the specificity of the Brentanian program.
