[en] Gestational trophoblastic diseases include placental pathologies comprising fertilization abnormalities (hydatidiform moles) and malignant lesions (choriocarcinoma, placental site trophoblastic tumor and epithelioid trophoblastic tumor). Due to their low incidence and heterogeneity, their diagnosis, management and treatment are not always optimal.
Following the example of other European countries, a national registration system with two reference centers has been set up to guide physicians and patients and to propose individualized management. The centers offer their expertise through a systematic centralised pathology review by a panel of experts. HCG values are plotted in regression curves. In case of gestational trophoblastic neoplasia, an imaging work-up is proposed, from which the FIGO score and stage are derived and will guide the choice of treatment. Belgian centers offer a

[fr] Les maladies trophoblastiques gestationnelles constituent une variété de pathologies placentaires comprenant des anomalies de la fécondation (môles hydatiformes) et des lésions cancéreuses (choriocarcinome, tumeur du site d’implantation placentaire et tumeur trophoblastique épithélioïde). Leur diagnostic, prise en charge et traitement sont malaisés du fait de leur faible incidence et de leur hétérogénéité. A l’instar d’autres pays européens, un registre national belge et deux centres de référence, l’un francophone, l’autre néerlandophone, ont été créés afin d’apporter un soutien aux médecins et aux patientes et d’harmoniser les prises en charge personnalisées. Tout cas de maladie trophoblastique fait l’objet d’une relecture anatomopathologique systématique par un panel d’experts. Le centre de référence dresse une courbe d’évolution des taux d’hCG. En cas de néoplasie trophoblastique gestationnelle, un bilan d’extension est réalisé et permet d’établir un stade et un score FIGO dont dépend le choix du traitement. L’approche multidisciplinaire, en partenariat
étroit avec le médecin référant, constitue le fondement de l’expertise des centres de référence. Un site internet (www.mole-chorio-bgog.eu) héberge les informations utiles aux médecins
et aux patientes, ainsi qu’un forum d’échange.
