Abstract :
[en] Background: Cardiac troponins (cTn) are considered as the best biomarkers for detection of myocardial cell injury and NT-proBNP as the best for the cardiac insufficiency. In this study, cTnT was measured by new commercially available high-sensitive methods in subjects undergoing a marathon and an ultra-trail. Our aim was to compare cTnT and NT-proBNP levels before and after the stress tests, in sportive subjects. Materials and Methods: Twenty eight subjects (26 men, 42.5 ± 11 yo) ran the Maasmarathon (42.195 kilometers) between Visé (Belgium) and Maastricht (The Netherlands) and 33 subjects (33 men, 45.7±9.3yo) ran the Ultratour of Liège (Belgium ; 67 km). We took blood sample before (T0), just after (T1) and 3 hours after the race (T3). In all the patients, cTnT concentrations were measured by high sensitive methods (hsTnT, Roche Diagnostics) on heparin plasma. The NT-proBNP was also determined with the kit Roche on heparin plasma. The protocol was approved by the Ethics Committee of the University of Liège (Belgium). All subjects gave their informed consent. All statistical analyses were performed using Medcalc version 8.1 for Windows. p-value <0.01 was regarded as statistically significant. Results and discussion: A significant difference between hsTnT concentrations at T0 and T1 (p<0.0001), and between T0 and T3 (p<0.001) for NT-proBNP have been observed, but not between T1 and T3. This observation appeared only after a strenuous exercise. However, up to now this type of exercise is not reproducible easily in a laboratory. Moreover, nobody knows if these observations would have cardiac consequences at long terms. Conclusions: Measurement of cardiac troponins by high sensitive methods allows detecting significant release of biomarkers from the heart during exercise. The value of NT-proBNP are also significant but less than TnThs. We think that the TnThs could be an interesting tool in the future to help sport medicine to detect risk of developing a cardiac problem in the future or a sudden death.
[fr] Objectif Le but de l’étude est d’étudier l’impact d’un exercice physique intense, représenté par des épreuves différentes d’endurance, sur les marqueurs cardiaques. Méthode Quatre populations sont comparées, un groupe contrôle avec 16 participants dits sédentaires ( âge = 37 ans ± 4,39 ans), un groupe de 24 semi-marathoniens ( âge = 41 ans ± 8,76 ans), un groupe de 28 marathoniens ( âge = 44,1 ans ±8,37) et un groupe de 33 participants à un ultra-tour ( âge = 45,8 ans ± 8,7 ans). Chaque population effectue trois prises de sang, une juste avant, une juste après, et une dernière trois heures après la fin de l’épreuve. On a dosé différents marqueurs cardaiques :troponine T (TnThs) et peptide natriurétique (NT-proBNP). Les participants à l’ultra-tour seront en plus soumis à une échocardiographie et à un ECG en pré et post-effort. Pour l’analyse statistique, nous avons utilisé le logiciel Medcalc version 8.1 et les résultats présentant une p-value <0.01 ont été considéré comme statistiquement significatifs. Résultats Nous observons une augmentation des taux de troponine T et du peptide natriurétique mais avec une cinétique différente comparée à celle obtenue lors d’un infarctus du myocarde. L’imagerie médicale chez les participants de l’ultra-tour présente des adaptations cardiaques à l’entraînement en endurance, à savoir une hypertrophie ventriculaire gauche HVG et un bloc de branche droit incomplet BBDI. Il est observé à l’échocardiographie une diminution des volumes télésystolique et télédiastolique du ventricule gauche à la fin de l’épreuve ainsi qu’une diminution du strain longitudinal. Conclusion Les courses d’endurance provoquent l’augmentation des marqueurs cardiaques et celle-ci pourrait-être expliquée par un changement transitoire de la perméabilité des myocytes avec re-larguage du contenu cytosolique. Au niveau de l’imagerie médicale, nous avons observé une adaptation du myocarde au volume d’entraînement et une altération transitoire de la fonction ventriculaire provoquée par la déshydratation.
