&lt;br/&gt;Abstract: Orthostatic hypotension (OH) is defined by a drop in arterial blood pressure (BP) of at least 20 mmHg for systolic BP and 10 mmHg for diastolic BP after standing. Symptoms are generally quite typical, but may also be rather vague. Diagnosis may be easily made by the physician in his/ her office, and confirmed, if necessary, by more sophisticated measurements. Pathophysiology is generally rather complex, but mostly involves a defect in the autonomic nervous system, in its sympathetic component. Failure of peripheral vasoconstriction seems to play a more important role than the defect in reflex tachycardia. Causes of OH are multiples. OH may occur in healthy subjects, when exposed to exceptional circumstances, but is more generally associated with various diseases, either neurological disorders or pathologies characterized by hypovolemia. Medications can also aggravate the risk of OH, among which some antihypertensive or psychotropic agents. Elderly people, especially frailty subjects, are exposed to a high risk of OH, whose origin is often multifactorial, and this complication may have serious medical consequences.; : L’hypotension orthostatique (HO) se définit par &#xD;
une chute de pression artérielle d’au moins 20 mmHg pour la &#xD;
systolique et 10 mm Hg pour la diastolique lors du redressement. Les symptômes d’appel sont souvent évocateurs, mais &#xD;
peuvent être également frustes. Le diagnostic peut se faire simplement au cabinet de consultation et peut être confirmé par &#xD;
des mesures plus spécialisées, si nécessaire. La physiopathologie est généralement complexe, mais implique très souvent &#xD;
une atteinte du système nerveux autonome, essentiellement &#xD;
dans sa composante sympathique. Un déficit de vasoconstriction périphérique semble jouer un rôle plus important qu’un &#xD;
déficit de la tachycardie réflexe. Les causes sont multiples. &#xD;
L’HO peut survenir chez un sujet sain, dans des circonstances &#xD;
exceptionnelles, mais est plus souvent associée à des pathologies diverses, neurologiques ou caractérisées par une hypovolémie. Différents médicaments peuvent aggraver le risque &#xD;
d’HO, dont certains antihypertenseurs et psychotropes. La &#xD;
personne âgée, particulièrement en cas de fragilité, est exposée &#xD;

&lt;br/&gt;Abstract: Cette étude rétrospective s’inscrit dans le cadre des recherches de la thèse de doctorat sur l’épargne sanguine à travers l’itinéraire clinique du patient opéré cardiaque. Son objectif est de dresser un état des lieux de la prise en charge des patients opérés cardiaques au CHU de Liège, des pratiques d’épargne sanguine qui y sont employées et de leur impact sur le recours à la transfusion, les durées de séjour et la mortalité. Cet audit devrait permettre de 1) mettre en évidence les éléments pouvant être améliorés dans l’organisation actuelle de la prise en charge de ces patients afin de concourir aux objectifs d’épargne sanguine 2) servir de base à l’élaboration d’un itinéraire clinique du patient opéré cardiaque.</description>

&lt;br/&gt;Abstract: To determine the role of cyclooxygenase (COX)-2 in anthracycline-induced cardiac toxicity, we administered doxorubicin (Dox) to mice with genetic disruption of COX-2 (COX-2−/−). After treatment with Dox, COX-2−/− mice had increased cardiac dysfunction and cardiac cell apoptosis compared with Dox-treated wild-type mice. The expression of the death-associated protein kinase-related apoptosis-inducing protein kinase-2 was also increased in Dox-treated COX-2−/− animals. The altered gene expression, cardiac injury, and dysfunction after Dox treatment in COX-2−/− mice was attenuated by a stable prostacyclin analog, iloprost. Wild-type mice treated with Dox developed cardiac fibrosis that was absent in COX-2−/− mice and unaffected by iloprost. These results suggest that genetic disruption of COX-2 increases the cardiac dysfunction after treatment with Dox by an increase in cardiac cell apoptosis. This Dox-induced cardiotoxicity in COX-2−/− mice was attenuated by a prostacyclin analog, suggesting a protective role for prostaglandins in this setting.</description>

&lt;br/&gt;Abstract: Neurotrophins (NTs) modulate the growth of human malignancies, including lung cancers. Our prospective study evaluated the accuracy of pleural NTs [nerve growth factor, brain-derived neurotrophic factor (BDNF), neurotrophin 3 (nT3) and 4 (nT4)] levels for differentiating benign from malignant pleural exudates. Levels of NTs were measured by ELISA in 170 patients with non-neutrophilic (&lt;50%) exudative benign or malignant pleurisies diagnosed by pleuroscopy. Fifty-nine benign (9 infections and 50 inflammatory diseases) and 111 malignant (50 extrathoracic tumors, 51 lung cancers and 10 mesotheliomas) pleural exudates were diagnosed by thoracoscopy. Levels of BDNF were significantly higher in malignant than in benign effusions [17 pg/ml (0-367) vs. 8 pg/ml (0-51), p&lt;0.05]. ROC analysis showed an area under the curve of 0.609 (p=0.012; best threshold 44 pg/ml). Pleural BDNF levels were significantly higher in pleural metastasis of pulmonary tumors and in mesothelioma than in pleural benign effusions. Finally, a higher proportion of pleural nT3 was detected in squamous cell lung carcinoma in comparison to that in non-squamous cell lung carcinoma (72.7 vs. 10%, p&lt;0.0001). NTs and particularly BDNF may play a role in the pathogenesis of malignant pleural effusions.</description>

&lt;br/&gt;Abstract: We now have access to a large library of publications validating transparietal thoracic echography in various clinical situations. Parietal lesions, including osteolysis, can be detected and biopsied during the thoracic ultrasound (TUS) examination. To evaluate the parietal extension of lung cancers, TUS has proved superior to tomodensitometry. Pleural effusions can be easily diagnosed and aspirated. Pneumothoraces can be detected using well defined lung artifacts with a high frequency probe. Pleural and peripheral lung nodules can be detected and biopsied with real time visualization; the procedure is safe and accurate. Lung consolidations with a pleural contact can be diagnosed; this is particularly useful for pregnant women. In conclusion, TUS is a precious diagnostic tool for chosen applications, and can help to guide interventional procedures. The portable devices are also very useful for bedridden patients or for out of hospital use.</description>

&lt;br/&gt;Abstract: Aortic dissection is one of the most serious aortic diseases by its potential for rupture, but also for other complications, such as cerebral or splanchnic ischemia, which may be fatal. If open surgery is the rule for lesions of the ascending aorta (type A), type B (not concerning the ascending aorta) is first a matter of medical treatment except when complications are present. In this case the placement of a stentgraft is a valuable alternative to open surgery. We report a patient presenting with a type B aortic dissection, characterized by rapid expansion and complicated by peri-aortic leakage, who was successfully treated by thoracic aortic stentgraft placement. This was done in a hybrid operating room associating the characteristics of a classical operating room for cardio-vascular surgery with those of an interventional radiology suite.</description>

Over the last years, many research works have been performed in this way to improve prognosis. Targeted therapies seem promising. They were notably developed against epithelial growth factor receptors and tumoral angiogenesis. We report here the clinical history of a patient with an advanced non-small-cell lung cancer presenting an exceptional response to small-molecule tyrosine kinase inhibitors.</description>
