La chirurgie élective recouvre les interventions qui peuvent normalement être programmées calmement sans danger immédiat pour la santé du patient. Naïvement, on pourrait s’attendre à ce que cette caractéristique contribue à des décisions d’intervention mûrement réfléchies et donc à peu de disparités entre les pratiques. Et pourtant on découvre dans la littérature internationale que le lieu de résidence du patient influence nettement la probabilité de subir certaines interventions. Ces différences sont-elles aussi observées en Belgique ? Quelle intervention, de la cataracte, du syndrome du canal carpien ou de la sténose carotidienne, présente la plus grande variabilité ? Où fait-on le plus d’arthroscopies en Belgique ? La prothèse totale de hanche et la prothèse totale de genou sont-elles liées géographiquement ? Et qu’en est-il de la césarienne, dont on a encore récemment beaucoup entendu parler dans l’actualité, pour la proportion inquiétante de césariennes dans certains hôpitaux ? Illustrer simplement les différences de pratique ne suffit pas. Classiquement, il s’agit ensuite d’essayer d’éliminer l’effet de plusieurs variables qui pourraient objectivement expliquer les différences observées, comme des comorbidités, des variables socioéconomiques, des facteurs d’offre médicale, … Mais les disparités de pratique subsistent en Belgique même après avoir éliminé l’effet de ces variables. Ceci avait déjà été mis en évidence dans des études précédentes. On débouche alors inévitablement sur un questionnement à propos de l’opportunité des soins. Cette recherche démontre que pour certaines interventions, il est possible de mettre en évidence une association entre « trop de soins donnés » et des « soins médicalement inopportuns ». Mais ce n’est pas toujours le cas, le questionnement reste alors entier et l’instauration de mesures qui viseraient à diminuer des différences non justifiées reste à l’ordre du jour. Une collaboration de fond avec les acteurs de terrain pour chaque domaine spécifique est un « must », comme le prouve cette étude. En outre, cette étude a montré une fois de plus que la Belgique est riche en bases de données qui ne demandent qu’à être exploitées, pas seulement par le KCE mais aussi par tous les autres acteurs qui cherchent à utiliser le plus efficacement possible les moyens disponibles pour offrir à tous les patients les meilleurs soins. The establishment of tools for trends analysis in groundwater is essential for the prediction and evaluation of measures taken within context of the Water Framework Directive and the draft Groundwater Directive. This report describes the historic development of contaminant inputs into the groundwater system for the TREND 2 subcatchments. The historic input data are essential to interprete and understand time trends in groundwater, which is the focus of coming deliverables. The emphasis is on diffuse contaminants and includes figures of 1950-present nitrogen inputs fot the Wallonian and Dutch Meuse catchments and the German Schleswig-Holstein catchments. For the Dutch Meuse basin also historical inputs of P, K, S, Ca, Mg, Na, Zn and Cd were estimated. Inputs of the pesticide atrazine were estimated for the small French Brévilles catchment, using interviews with farmers. On a regional scale, all of these cases show increasing inputs up to about 1985 and a decrease afterwards due to regulations from that time on. Trends in inputs of other diffuse pollutants (cations, heavy metals, pesticides) seem to be correlated to the well established nitrogen inputs. The effects of this increase and subsequent decrease on concentrations in the groundwater system will be analysed in further work within TREND2. [less ▲]

This deliverable provide a description of hydrogeological equipment available, hydrogeological tests and monitoring carried out up to now or planned for the next months to better understand hydrodynamic conditions pollutant transfer mechanisms in the gravel alluvial aquifer of the river Meuse and to assess groundwater– surface water interactions in the Flémalle former cokery test site. The monitoring campaign and the experiments are performed using existing and recently drilled piezometers in the site. Up to now, continuous groundwater level monitoring, Meuse river water level monitoring, pumping tests and tracer tests have been carried out. They have already confirmed that the gravel aquifer is drained by the Meuse river. [less ▲]

Based on a first inventory of available case studies, test sites have been selected in the Walloon part of the Meuse Basin in Belgium. These sites meet the requirements and objectives of the researches performed by different partners in AquaTerra. Available information and datasets have been collected for these sites and complementary investigations and experiments have been organized. Existing data and recently collected data have been compiled into a hydrogeological database developed by HGULg for the Walloon region and adapted to the specificities and needs of the AquaTerra project. The deliverable provides a general description of the hydrogeological database developed by HGULg: the conceptual model, specific adaptations performed for the AquaTerra project and interfaces for encoding, querying and exporting hydrogeological information stored in the database. Are the universalisation and the globalisation of trade exchanges synonymous with a standardization in which agro-food products uprooted of their soil will no longer be differentiated from manufacturers or supermarket brands? Original food products belong to the inheritance of the territories and the consumers are attracted more and more by their authenticity. The GeoTraceAgri project resolutely supports agriculture and the sustainable promotion of the territory as opposed to universalisation which standardizes and moves away those who produce for consumers. Geotraceability aims at associating information of geographical nature with the traditional data of traceability. Farming origin and operations have become factual and verifi able data is available everywhere in the world, thus making it possible to bring additional guarantees to the signs of quality. The GeoTraceAgri (GTA) project largely contributed to the realisation of geotraceability. With the implementation from January 1,2005 of the new Common Agricultural Policy and its regulation imposing on the Member States a single system of declaration, all the agricultural parcels now form part of a European database of geographical references. This new regulation reinforces the basis of the concept of geotraceability, whereas throughout the project it was necessary to defi ne geo-indicators for integrated or crop production with very few geographical data on the farming precedents. The development of the GTA prototype rests on a decentralized architecture and Web services. It was indeed necessary to conceive a system which is readily accessible on Internet for farmers, co-operatives and collectors, and potentially with the administrations which have control responsibilities. In term of acceptability, the potential users realise the potential economic benefi ts of the concept and of the indicators of geotraceability in their plan of exploitation, on the other hand sociological acceptability is less evident which induces the need for communication to make for its adoption. This fi nal report fi nal illustrates the fi rst stage : the GeoTraceAgri partners are continuing their research on the defi nition of an integrated system of geotraceability for the Common agricultural policy and the plan of analysis of the results of GeoTraceAgri should lead to the marketing within two years of an application making it possible to integrate the geo-indicators into management software for the actors of the agro food chain. Cette recherche intitulée RASCA (Recherche-Action pour la Sécurité de la Chaîne Alimentaire) est financée par le SPP Politique Scientifique et réalisée par le laboratoire SPIRAL-ULg sous la direction du professeur Sébastien Brunet. En outre, elle s’inscrit dans deux études-pilote du CCMS-OTAN (Committee on the Challenges of Modern Society of the NATO ; www.nato.int/ccms), l’une consacrée aux réponses à apporter aux risques (CCMS-NATO Pilot Study on Effective Risk Response Strategies – MERREA ; www.nato.int/ccms/pilot-studies/ERRS/errs-index.htm), et l’autre, relative à la Sécurité de la Chaîne Alimentaire (CCMS- NATO Pilot Study on Food Chain Security ; www.nato.int/ccms/pilot-studies/fcs/fcs-index.htm). D’une manière générale RASCA est découpée en deux volets. Le premier volet comporte un large descriptif de la problématique « sécurité alimentaire en Belgique » : identification des acteurs institutionnels et des stakeholders ; descriptions des outils, des procédures et des moyens techniques existants, mis en place et développés par ces différents acteurs, etc. Le second volet est consacré à l’analyse de cas afin de constater comment les différents acteurs interagissent, comment les outils sont mobilisés, les procédures appliquées. Cette photographie des pratiques et de la réalité du terrain est riche d’enseignements et constitue le noyau dur et l’apport principal de cette recherche. Ainsi, nous avons procédé à une analyse comparative de deux crises alimentaires récentes (dioxine 1999 et peste aviaire 2003) afin d’évaluer la refonte des dispositifs institutionnels chargés d’assurer la sécurité de notre alimentation et notamment l’apport (l’effectivité et l’efficacité) de l’Agence fédérale pour la Sécurité de la Chaîne Alimentaire comme pierre angulaire du nouveau dispositif. Enfin, nous avons réalisé une étude de cas prospective relative au terrorisme alimentaire afin d’évaluer la faisabilité et la probabilité de tels scénarios. Les apports de cette recherche, à la fois descriptive, évaluative et prospective, sont multiples. D’une part, RASCA contribue à améliorer la compréhension du caractère vulnérable de nos sociétés modernes dites du risque et contribue à augmenter les connaissances sociologiques et institutionnelles en la matière. D’autre part, cette recherche est jalonnée de micro-apports tantôt relatifs à l’inventaire des bonnes pratiques en matière de gestion de crise, tantôt relatifs à l’amélioration des processus de communication internes ou externes, à l’amélioration des dispositifs de contrôle de la chaîne alimentaire, ou encore à l’identification des enjeux futurs et des pistes de travail possibles. The study is conducted in the frame of a programme of monitoring of all landfill sites in Wallonia. The "odour" part is assigned to ULg. The present report concerns the site of Tenneville which includes also a composting area. Besides the usual field inspection technique, the research group used also GC-MS analyses aiming at estimating the composition of the atmosphere over the tipping area. A study of the influence of the particular relief of the site is also provided. [less ▲]
