A pregnant 3.5 year-old high-productive Holstein cow in the third month of lactation showed colic signs, marked anorexia, and stopped milk production. Clinical examination revealed tachypnea, pyrexia and the mucous membranes were all congested. Signs of abdominal discomfort were observed, the abdomen was tense and painful; digestive activity was poor. Blood investigations revealed inflammation, cholestasis, and leucocytosis with marked neutrophilia. Trans-abdominal ultrasonography revealed decreased digestive transit and enlarged gall-bladder. Hepatic or biliary involvement was suspected. A right-flank exploratory laparotomy revealed a gallbladder filled with firm and mobile 0.5 to 1cm diameter fluctuant masses. Digital palpation allowed cholelith extraction through the cystic duct into the duodenum. After the surgery, the cow progressively regained a normal appetite and pain signs decreased. Blood samples analysed 10 days after the surgery still showed inflammation but to a lesser extent. Hepatic enzymes were markedly increased, indicating hepatic injury. Three months after the surgery, the cow was healthy and was inseminated. This case report suggests that cholelithiasis may be diagnosed and successfully treated in early stage of the disease in cattle, though this condition is rare in this species. This is the second report of manual cholelith extraction in a cow. [less ▲]

An 11-year-old, mixed-breed dog suffering progressive asymmetric hind limb ataxia was diagnosed as having spinal cord compression at the L1 spinal level. An intramedullary, encapsulated tumor was surgically removed. Tumor microscopic features, positive neuron-specific enolase (NSE) immunostaining, and Grimelius staining led to the diagnosis of paraganglioma. The dog was free from any neuroloaic signs 12 days after surgery. [less ▲]

REASONS FOR PERFORMING STUDY: Sarcoids are the commonest form of equine skin tumour. Several therapeutic measures have been described but none is considered to be universally effective. Electrochemotherapy (ECT) is a new anticancer therapy that utilises electrical field pulses to induce increased cell membrane permeability to antitumour hydrophilic drugs, such as cisplatin. The increased intracellular concentration of the drugs has a significant therapeutic benefit. The procedure has not been previously reported in a large number of horses. OBJECTIVE: To validate ECT as a novel alternative treatment for equine sarcoids. METHODS: A retrospective study evaluating the efficacy of cisplatin ECT in the treatment of equine sarcoids was performed. Electrochemotherapy treatments were applied under general anaesthesia at 2 week intervals with or without prior excision or debulking. Electric pulses were directly applied to the lesions following intra-tumoural injections of an aqueous solution of cisplatin. RESULTS: One-hundred-and-ninety-four sarcoids on 34 horses, 2 ponies, 11 donkeys and one mule were treated with ECT. The 4 year nonrecurrence rate was 97.9% for animals (47/48) and 99.5% (193/194) for tumours. When ECT was used as a single treatment, a significant influence of tumour size (ρ= 0.55) on the number of treatments required for cure was shown. When prior surgery was performed, there was a significant influence (P<0.001) of the excision quality (complete or incomplete) and the healing mode (closed or open wound) on the number of treatments. The most common adverse effect was a slight oedematous reaction for lesions located on thin skin regions. CONCLUSION AND CLINICAL RELEVANCE: Results demonstrate that ECT, with or without concurrent tumour debulking, is an effective alternative for treatment of equine sarcoids [less ▲]

The acyclic nucleoside phosphonate (S)-1-(3-hydroxy-2-phosphonylmethoxypropyl)cytosine (HPMPC) was used topically for the treatment of persistent mucocutaneous infections in two cases. One patient with AIDS suffered from a perineal lesion due to infection with herpes simplex virus type 2 (HSV-2) and did not respond to acyclovir and was intolerant of foscarnet. A bone marrow transplant recipient developed orofacial lesions due to infection with herpes simplex virus type 1 (HSV-1) that failed to respond to therapy with both acyclovir and foscarnet. After topical application of HPMPC, the HSV-2 lesions completely resolved. However, the lesions recurred 3 weeks later, and, upon subsequent treatment with HPMPC, regressed. On recurrence, the virus was found to be sensitive to acyclovir, which the patient was given. Again HSV-2, which was resistant to acyclovir, emerged; similar observations were made after another cycle of HPMPC therapy. The HSV-1 isolates were resistant to acyclovir and foscarnet. Following local HPMPC treatment, the lesions regressed, but after 1 week, a second course of topical HPMPC therapy had to be instituted for recurrent infection. The lesions again regressed, and as the recurrent virus was sensitive to acyclovir, the patient was successfully treated with the drug. The results of this study point to the potential usefulness of topical HPMPC in the treatment of immunocompromised patients with HSV-related mucocutaneous infections that are refractory to therapy with acyclovir and/or foscarnet. A case of successful tricuspid valve replacement with a mechanical prosthesis and pulmonary valvulotomy for carcinoid heart disease is reported. The patient was a 61 years old women. The primary tumor was in the terminal ileum. Liver metastasis and carcinoid syndrome were present since 8 years. After cardiac surgery, the patient survived 38 months and late death was related to disseminated metastasis. Even in case of metastasis, carcinoid tumor is slow growing. Without cardiac operation for correction of valvular lesions, terminal symptoms and death may often be related to cardiac operation for correction of valvular lesions, terminal symptoms and death may often be related to cardiac failure rather than to tumoral growth. Thus, even in presence of metastasis, cardiac surgery may be mandatory to improve both quality of life and survival. [less ▲]
