Osteoporosis in very elderly subjects is now considered, in most developed and several developing countries as a major social, clinical and financial burden. While many compounds have been investigated in the prevention or treatment of spinal fractures, few of them have unequivocally demonstrated their ability to reduce the risk of non vertebral and more specifically hip fractures in the very elderly. This situation may seem highly paradoxical since hip fractures are unanimously considered to be the most dramatic and disabling consequence of osteoporosis. The present article reviews the current evidence available to justify anti-osteoporotic medications to be recommended to very elderly subjects, in the perspective of reducing their risk of appendicular fractures. Only few dissecting aneurysms type A survive without surgery. Opposingly, the dissecting aneurysms type B are successfully managed by the medical treatment "anti-impulsion". A personal series is reported, that evidence important cardiovascular mortality in the follow-up in chronic dissecting aneurysms type B; one part of this late mortality is linked to rupture of the thoracic descending aorta. Thus, the prevention of this late mortality depends on a strict follow-up of the characters and dimensions of descending aortic thoracic aorta. [less ▲]

Limb threatening ischemia is a challenge for the vascular surgeon. Recent progress in revascularization procedures allow to minimize the primary amputation rate in the management of chronic critical limb ischemia. The authors discuss the prevalence and causes of chronic critical limb ischemia, with a special interest for diabetic arteriopathy. The technique of crural and pedal vessel revascularization is described, as well as the innovative tourniquet technique for distal bypass surgery. A review of published series of infrapopliteal bypass surgery is made. The experience of the authors during last decade with crural and pedal bypass surgery is analyzed. [less ▲]

Diabetic nephropathy is one of the leading causes of end-stage renal failure in western countries. This disease develops over several years. Early stages, if they are detected in time, can lead to preventive treatment at a moment when the disease is still reversible. This paper reviews the main primary and secondary preventive measures that have been proven efficacious. Those are essentially the optimal treatment of hyperglycaemia and hypertension, and probably the use of agents that specifically block the renin-angiotensin axis. We briefly discuss the results of the main studies that have led to those conclusions. [less ▲]

The actual results confirm the dominating influence of psychosocial factors on prematurity and low birth weight. The study performed in Liege indicates that these factors must be taken into account for better care of the future mother. The systematic use of a prenatal questionnaire on psychosocial factors draws attention towards the personal situation of the future mother. Detection of some organic or psychosocial risk factors must prompt prophylactic measures even in the absence of any sign of pathologies. Prevention of prematurity has, over the last 30 years, been the primary objective of prenatal follow-up. Its importance must be further emphasized. [less ▲]

Différentes approches pharmacologiques systémiques ont été testées pour réduire le risque de resténose et de complications après angioplastie coronaire, avec ou sans stent, objectif prioritaire dans la population diabétique où ce risque est significativement accru. Le but de cet article est de décrire les effets des principales classes médicamenteuses évaluées quant au risque de resténose, à la nécessité de recourir à de nouvelles procédures de revascularisation et à l’incidence des événements cardiaques majeurs (décès, infarctus non mortels et revascularisations). Nous analyserons le rôle des antiagrégants plaquettaires, des acides gras oméga 3, des statines, des anti-inflammatoires, des immunomodulateurs, des anti-oxydants et des inhibiteurs du système rénine-angiotensine. Dans toute la mesure du possible, nous centrerons notre atten tion sur les résultats obtenus dans la population diabétique. [less ▲]

En ce début de siècle, nous assistons à un changement du type de violence associé aux manifestations sportives, notamment aux matches de football. En effet, la violence dans le sport, qui se cantonnait aux stades de football et avait un caractère exceptionnel et spontané, a gagné les zones urbaines et le coeur des villes. La société contemporaine est désormais confrontée à une violence dans le sport souvent préméditée et généralement provoquée par un noyau dur de supporters qui recherchent la confrontation systématique et organisée, lors des matches et des tournois locaux, nationaux et internationaux. Etant donné que le hooliganisme devient international et crée des problèmes similaires en Europe et partout dans le monde, il est nécessaire de lui apporter, à l’échelle internationale, une réponse commune et concertée, axée sur la prévention. La prévention de la violence dans le sport propose des actions et des structures fondées sur des expériences en cours qui ont montré une certaine efficacité. Ces initiatives donnent une gamme de solutions permettant de mettre en place des activités de prévention intégrées de haut niveau, qui peuvent être adaptées aux besoins locaux. [less ▲]

Long-lasting, because ageing started in the second half of the XXth century and will become even more pronounced over the XXIst century. With important implications, because it has and will have substantial consequences on human life and social composition. This article reviews quantitative data on ageing to appraise its extent and implication, the goal being to extend and stimulate the debate on ageing.
