We demonstrate that under some magnetospheric conditions protons and oxygen ions are accelerated once per Saturn magnetosphere rotation, at a preferred local time between midnight and dawn. Although enhancements in energetic neutral atom (ENA) emission may in general occur at any local time and at any time in a Saturn rotation, those enhancements that exhibit a recurrence at a period very close to Saturn's rotation period usually recur in the same magnetospheric location. We suggest that these events result from current sheet acceleration in the 15-20 Rs range, probably associated with reconnection and plasmoid formation in Saturn's magnetotail. Simultaneous auroral observations by the Hubble Space Telescope (HST) and the Cassini Ultraviolet Imaging Spectrometer (UVIS) suggest a close correlation between these dynamical magnetospheric events and dawn-side transient auroral brightenings. Likewise, many of the recurrent ENA enhancements coincide closely with bursts of Saturn kilometric radiation, again pointing to possible linkage with high latitude auroral processes. We argue that the rotating azimuthal asymmetry of the ring current pressure revealed in the ENA images creates an associated rotating field aligned current system linking to the ionosphere and driving the correlated auroral processes. OBJECTIVE: A series of antibiotics may be responsible for toxic epidermal necrolysis. We report two successive episodes of toxic epidermal necrolysis in the same patient. Drug imputability criteria designate a cross-reactivity between two antibiotics of different chemical classes but sharing the beta-lactam ring in common. DESIGN: Descriptive case report and review of the literature. SETTING: Medical intensive care unit in a university medical center. PATIENT AND MAIN RESULTS: A 75-yr-old woman developed a first episode of toxic epidermal necrolysis (involving 40% of the body surface) after intake of cefotaxime, a third-generation cephalosporin. Perfusions of high-dose immunoglobulins rapidly improved the lesions, followed by partial reepithelialization in 5 days. Sepsis required the administration of meropenem, which is a carbapenem antibiotic. The epidermal destruction immediately recurred, with extension to previously uninvolved skin areas and fatal consequences. CONCLUSIONS: The beta-lactam ring present in cephalosporins and carbapenems represents the putative chemical structure responsible for the presently reported cross-reactivity to two antibiotics of different classes. Drugs having any chemical similarity to the initial culprit compound should be strictly avoided when possible in the management of toxic epidermal necrolysis. [less ▲]

In this paper, neural networks approaches are compared for predicting the high pressure (HP) steam flow rate from a Kraft recovery boiler. We apply two types of neural networks: a static multilayer perceptron and a dynamic Elman’s recurrent neural network. Starting from a one-day database of raw process data related to the boiler, the goal is to model and predict the next 12-hours of HP steam flow production from the boiler to the steam turbine. The results illustrate the potential of the dynamic approach in this task. [less ▲]

Diabetes mellitus and exocrine insufficiency are the commonest pancreatic manifestations of mitochondrial diseases. In contrast, pancreatitis has rarely been described in mitochondrial syndromes. We report on a patient with Kearns-Sayre syndrome and recurrent episodes of acute pancreatitis for which no explanation could be found other than the associated mitochondrial dysfunction. Interestingly, pharmacological disruption of mitochondrial metabolism in various models as well as in patients can cause pancreatitis, further supporting this association. A diagnosis of pancreatitis should be considered in any patients with mitochondrial disease and recurrent abdominal pain. [less ▲]

We have previously shown that the PLAG1 gene on chromosome 8q12 is consistently rearranged in pleomorphic adenomas of the salivary glands with t(3;8)(p21;q12) translocations. The t(3;8) results in promoter swapping between the PLAG1 gene, which encodes a novel zinc finger protein, and the constitutively expressed gene for beta-catenin (CTNNB1), a protein with roles in cell-cell adhesion and the WG/WNT signalling pathway. In order to assess the importance of other translocation partner genes of PLAG1, and their possible relationship to CTNNB1, we have characterized a second recurrent translocation, i.e. the t(5;8)(p13;q12). This translocation leads to ectopic expression of a chimeric transcript consisting of sequences from the ubiquitously expressed gene for the leukemia inhibitory factor receptor (LIFR) and PLAG1. As for the t(3;8), the fusions occurred in the 5'-noncoding regions of both genes, exchanging regulatory control elements while preserving the coding sequences. The results of the current as well as previous studies indicate that ectopic expression of PLAG1 under the control of promoters of distinct translocation partner genes is a general pathogenetic mechanism for pleomorphic adenomas with 8q12 aberrations. This scenario conforms to Sekanina's conceptual model for the release of sizable fragments of an inert dust mantle from the nucleus surface: an outburst is triggered as some of the mass rapidly disintegrates into fine dust.
