The authors recently observed 2 elderly female patients with ischemic pain of the upper extremity as the first manifestation of giant cell arteritis. They presented with rest pain of both upper extremities and even gangrene of the thumb in 1 case. Subclavian and radial pulses were absent while peripheral pulses in the lower limbs were preserved. The angiography was so typical that the diagnosis of inflammatory arteritis was made, despite negative temporal artery biopsy. The patient with thumb gangrene was successfully operated on, the occlusive axillary lesions being bypassed by a long venous carotid humeral bypass graft. A biopsy of the axillary artery showed a granulomatous lymphoplasmocellular infiltration. A high-dose corticotherapy (24 mg daily) was begun in both cases, with dramatic improvements of general state, lowering of the erythrocyte sedimentation rate, and even reapparition of a reduced radial pulse in 1 patient. The authors discuss the incidence, symptoms, diagnosis, and treatment of systemic giant cell arteritis, with special attention to extracranial involvement. These case reports may broaden the knowledge of the diverse manifestations of giant cell arteritis and of its systemic character with widespread vascular involvement. [less ▲]

"Giant Condylomas" of the cervix are very uncommon, and have not been fully characterized in the English literature. We report 4 cases of cervical giant condyloma seen in our practice. Patients were predominantly young and presented with a cervical lesion producing bleeding or a mass effect. Biopsy/excision revealed a uniformly bland, exophytic squamous epithelial proliferation with viral cytopathic changes and absence of stromal invasion. Human papilloma virus types 6 and 11 were detected in all cases. Follow-up was uneventful without recurrence or spread. Giant condylomas of the cervix as defined in this report signify a benign albeit extensive variant of low-risk human papilloma virus infection. This term is proposed as a specific descriptor for such lesions and should be considered in the setting of any large well-differentiated exophytic cervical squamous lesion in young or immunosuppressed women. The term "giant condyloma of Buschke and Loewenstein" should be discontinued given the lack of specificity. [less ▲]

We have experimentally studied granular arches through electrical measurements. The packing is composed of 2d metallic pentagons and is submitted to small taps. Large electrical fluctuations are observed and they are distributed along power laws. The indicates the presence of long-time memory effects event the packing density remains constant around a value rho = 0.72 +/- 0.02. Large electrical fluctuations should be associated with the breaking/creation of granular arches. [less ▲]

The putative role of gibberellins in the transition to flowering was investigated in Sinapis alba, a caulescent long-day (LD) plant. It was observed that: (1) physiological doses of exogenous gibberellins (GA(1), GA(3), GA(9)) do not cause the floral shift of the meristem when applied to plants grown in short days but have some positive effect on the flowering response to a suboptimal LD; no inhibition was observed in any case; (2) GA-biosynthesis inhibitors (prohexadione-Ca and paclobutrazol) considerably inhibit stem growth but have some negative effect on flowering only when a suboptimal LD is given; and (3) the floral transition induced by one 22-h LD does not correlate with any detectable change in GA content of the apical bud, of the leaves, and of the phloem exudate reaching the apex. Taken together, these results suggest that GAs do not act as a major signal for photoperiodic flower induction in Sinapis. [less ▲]

Peu de temps après, il est devenu clair que le gigantisme et l'acromégalie ont la même étiologie: une hypersécrétion d'hormone de croissance, la plupart du temps par un adénome hypophysaire, produit un géant si elle a lieu avant l'âge adulte et un acromégale si elle survient après la puberté. Au cours du XXe siècle, cette affection sera caractérisée et divers traitements efficaces mis au point. A la fin du siècle, les mécanismes génétiques responsables de la maladie seront élucidés. Some years later, it became clear that acromegaly and gigantism share the same etiology: GH hypersecretion due to a pituitary adenoma, induces gigantism if already present during puberty, or an acromegalic if it appears only during the adulthood. During the XXth century, the disease has been well described and is now well controlled with several treatments. Recently, genetic alterations responsible for the disease have been elucidated.
