L’argument de ce XLème Colloque des Hôpitaux de jour psychiatriques nous a plongés dans un abîme de questionnements théoriques et pratiques. Défiés par cette immersion dans les tréfonds du modèle, nous avons choisi de nous accrocher aux fondements de notre univers. La mythologie grecque sera donc notre source d’inspiration en mettant un dieu antique à notre disposition. Chronos sera notre guide dans un périple réflexif soumettant nos théories et notre pratique à l’épreuve du temps. Le récit de la conception d’un atelier thérapeutique, l’atelier « Art & Temps », sera notre fil d’Ariane dans notre tentative de symboliser notre pratique thérapeutique quotidienne et ses enjeux. Par la suite, l’exposé d’un cas clinique concrétisera un raisonnement théorique visant à situer notre modèle de prise en charge dans le réseau psychiatrique global. [less ▲]

During a new period of disruption with the hospital-tradition of psychiatric care, the specificity and originality of the Day Hospital remains to be defined. The metaphoric dimension of the therapeutic “passe-partout”, could be appliqued to hospitals which are to this day, part of institutional therapy story: The care teams have effectively replaced the walls to accommodate the most complex mental diseases. Several therapeutic “keys” are well produced and co-constructed with the fundamental goal of allowing the patient to (re) gain freedom-bridges lost under the weight of his psychopathology. Even though in 40 years the Day Hospital continues to evolve and reinvent, it interestingly gets through crises. This ability to overcome crises is seen in the deinstitutionalization crisis where it was handled with stability due to its originality. Stability does not mean stagnation: the daily questioning of the therapeutic work group leads us… Thanks to this article, we will specifically highlight the therapeutic effect observed across the different support models to the Day Hospital. The symptom remains a kind of "royal road" into get to the day hospital. But what does it convey ? What innovative therapeutic responses can the day hospital provide ? After a brief reflexion on the symptom, from a semiologic and diagnostic point of view, and an overview of the psychodynamic, systemic and cognitive-behavioral models, we will examine the ingredients that nourish the integrative therapeutic work at the day hospital. Through clinical vignettes, we will describe the work of our institution by highlighting elements related to the concepts of institutional psychotherapy, setting, individual and group transferences and counter-transferences, holding, maternal and paternal functions, intrasubjective and intersubjective reality, object-shoring, and psychological envelope. We will present a particular therapeutic activity called "My Key" by looking at the work of symbolization carried out at the hospital, as well as the notions of the malleable medium and the transitional object. We will see how the day hospital, as an area of care within the scope of a temporal discontinuity, enables psychic continuity and opens the road to the subject’s recovery. [less ▲]

Le travail avec les familles est indispensable en pédopsychiatrie, qu'il s'agisse de thérapie familiale ou d'accompagnement familial. Dans cette communication, nous réfléchissons à la place de l'identification projective dans toute rencontre thérapeutique avec une famille en souffrance, sur base d'un travail clinique avec des parents dont un des enfants est handicapé par le syndrome de Prader Willi. Actuellement, l'identification projective est considérée comme un mécanisme normal et important du fonctionnement des parents et des thérapeutes. La clinique du handicap nous montre la grande difficulté des parents à entrer dans ce processus lorsque leur enfant réel est si loin de l'enfant attendu. [less ▲]

Through an interactive workshop called "the feelings through the senses", we are going to describe three successive stages allowing to tame the psychic experiences via the sensory channels, the hearing then the sight, isolated artificially at first then gathered secondly. A work of verbalization in small groups is realized at the end of every session. To test the emotional changes among our patients benefiting from this institutional psychotherapy, we have been assessing, for two years, for every patient, at the beginning and the end of hospitalization, the following dimensions: awareness of feelings, representation of emotions, capacities of social regulation of the feelings. These first results are encouraging and testify that the therapeutic work in institution enhances transfer and counter- transfer movements by leading new corrective emotional experiences in a "secure" environment. It’s known that sexual health of people with psychiatric and psychological disorders is often highly impoverished. Mental health professionals may feel helpless against these sexual difficulties, because of being poorly trained to treat them and lacking the means to do it. In this context, it seems important to inform patients about sexual difficulties and treatment options. Although it’s impossible to assign a sex therapist to each mental health service, it could be very useful to use bibliotherapeutic documentation to guide people with sexual difficulties in finding solutions. This mode of treatment which consists in putting information and therapeutic guidelines on writing is called « bibliotherapy ». Some bibliotherapies have already been developed and tested in the field of sexual disorders. Others, experimental, are nowadays under clinical evaluation, notably by three of the authors of the present paper. Bibliotherapy could be a helpful adjunct in improving the sexual health of dayhospital patients. [less ▲]

We will examine more closely the population of alcoholic patients attending the day hospital on the demographic, psychiatric (DSM IV), psychological (TCI-R and YSQ3) and social levels, then illustrate the effectiveness of it in their their taking charge.
