[more ▼] The presence of hyperferritinemia has to rule out acquired causes such as chronic inflammatory disorders, hemolytic anemia, liver diseases as hepatitis B or C, alcohol abuse and non alcoholic fatty liver disease, specially in patients with normal transferrine saturation. Genetic testing for hemochromatosis is systematically indicated in all patients with elevated transferrine saturation. When an iron overload is demonstrated in the absence of these classic causes, second-line genetic testing should be considered to exclude non HFE hemochromatosis. The aim of this paper is to propose a practical algorithm in the diagnosis of hyperferritinemia and to precise the diagnostic and therapeutic management of genetic hemochromatosis. The model used to assess the overall risk was adapted for Belgium. Otherwise, very few things were changed from the Executive Summary of the European Guidelines. [more ▼] Because of the common failure of the various medical procedures used for the treatment of morbid obesity, surgeons have searched alternatives among which gastroplasty appears to offer now the best efficacy/safety ratio. Such approach has become more and more popular in our country, but success is almost highly variable from patient to patient. A careful medical assessment is necessary before performing gastroplasty. It mainly aims at choosing the appropriate indications (failure of previous well-applied medical therapy, exclusion of any hormonal cause of obesity, presence of complications due to weight excess, ...) and at excluding cardiovascular (unstable coronaropathy, carotid stenosis, ...), digestive (recurrent oesophagitis, ...) or psychiatric contra-indications. Such medical assessment before gastroplasty should allow to increase the success rate and to reduce the incidence of complications after the surgical procedure. [more ▼] The genetic mechanism controlling sexual differentiation had remained unknown for a long time. Karyotype analysis of sex-inverted patients or individuals with ambiguous sexual differentiation has enabled the localization and identification of genes involved. It is currently known that the SRY gene is responsible for the initiation of a cascade reaction leading to male differentiation of the primitive gonad. SRY is a +/- 820 base pairs gene located on the small arm of the Y chromosome, more precisely within the 1A1 alpha sub-segment. Although a few other genes are known to be involved in the downstream regulation of SRY, their precise mode of action is yet unknown. [more ▼] For years now, hypertensive patients have been treated following the general rules known as the stepped care program. This approach is presently criticized. The recent interest for new drug treatment, the introduction of new first fine drugs, the use of lower dose of antihypertensive agents and the search for prediction of the response have to be introduced in our actual management of the hypertensive patient.
