BACKGROUND: More than 90% of vitamin D synthesis is dependent on UV exposure. Photosensitive disorders such as lupus erythematosus, protoporphyria and xeroderma require strict sun avoidance, and vitamin D deficiency has been demonstrated in these patients. Melanoma patients are also instructed to avoid sun exposure and may hence be expected to be vitamin D deficient. MATERIALS AND METHODS: Winter and summer vitamin D levels were compared in a group of melanoma patients (n =61) and age- and phototype-matched controls (n = 53) without photosensitive disorders. RESULTS: Oral supplementary vitamin D intake was reported in 32.7% of the melanoma patients and in 15.1% in the control group. Despite oral supplementation, only 25% of the melanoma patients and the controls presented with vitamin D levels of 30 ng/mL or higher. In non-supplemented subjects in the melanoma and control groups, respectively, mean winter vitamin D levels were below the recommended threshold at 12.6 ng/mL vs. 13.2 ng/mL, respectively, but not statistically different. These values increased significantly in both groups during the summer to 24.6 and 23.8 ng/mL respectively. CONCLUSION: Unexpected, significant increases in vitamin D levels were seen in melanoma patients during summer, suggesting non-adherence with photoprotective measures and reflecting a heliophilic behaviour. Vitamin D supplementation is recommended in melanoma patients during both winter and summer. [less ▲]

Xerosis represents a physiological response of the stratum corneum (SC) to environmental threats. The influence of the environmental dew point (DP) is not fully understood. This parameter is the air temperature at which the relative humidity is maximum. This study aimed to assess the relationship between the environmental DP and the water-holding capacity of the skin and lower lip vermilion. For comparison, SC property was evaluated after occlusive application of cooled and uncooled hydrogel pads. Electrometric measurements using a dermal phase meter (DPM) device were performed on the back of the hands, the cheeks and the lower lip of 40 healthy menopausal women. Assessments were performed in the outdoor conditions during winter and spring. The same measurements were recorded after hydrogel pads, at room temperature or cooled to 4 degrees C, were placed for 15 min on the test sites. The environmental DP was recorded at each evaluation time. The SC water-holding capacity was discretely influenced by the DP. In the open-air environmental conditions, a positive linear relationship was found on the cheeks between the DP and DPM values. The relationship was weaker on the lips. Conversely, a consistent increase in DPM values was recorded immediately after removal of the cooled and uncooled hydrogel pads. The observations made in the open-air testing conditions are consistent with the predicted events following the Arrhenius law. By contrast, the combination of cooling and occlusion by the hydrogel pads is responsible for the reverse effect on the SC. [less ▲]

Many of the skin fungal infections in cancer patients may look similar to those infections in non-cancer patients. However, in some cases, they become more extensive and even life threatening. Prolonged and severe neutropenia is the main risk factor for the dramatic issue of fungal infections. The dermatomycoses in cancer patients can be classified in four broad groups: primary superficial dermatophytoses, primary superficial yeast infections, opportunistic mold infections with distinct potential for dissemination and secondary cutaneous manifestations of fungaemia. Occasionally, more than one fungus are found inside a given skin lesion. A special condition is represented by the mycotic colonization of mucosal squamous cell carcinomas. Angio-invasion by fungi accounts for the frequency of disseminated infections prevailing in immunocompromised cancer patients. In case of skin involvement, the dermatologist may assist by recognizing subtle semiological signs and performing biopsies for swift histological examination, molecular biology and/or culture. OBJECTIVE: There is ample clinical evidence that skin is responsive to physiopathological levels of circulating thyroid hormones. The aim of the study was to assess some physical changes of the skin in the presence of moderate thyroid dysfunction. SETTING: University Hospital. PATIENTS AND METHODS: A total of 119 adults suffering from hypothyroidism or hyperthyroidism and 60 healthy controls were enrolled in this study. Hormonal dosages (TSH, fT3, fT4) were assessed in the serum. A series of biometrological assessments were also performed on the volar and dorsal aspects of the forearms. These included electrometric assessments (Nova Dermal Phase Meter, Corneometer), evaporimetry (Tewameter)), colorimetry (Mexameter), ultrasound shear wave propagation (Reviscometer) and squamometry X. Correlations were searched between each of the serum hormonal dosages and each of the biometrological parameters. RESULTS: The hormonal changes in the untreated patients with thyroid dysfunction were modest in intensity. A few outlier values with regard to the normal range were found for each biometrological parameter. No correlations were found between fT3 or fT4 and each of the physical parameters. By contrast, significant negative linear correlations were found between thyroid-stimulating hormone (TSH) and skin hydration measured by the Corneometer and the Nova DPM. CONCLUSION: This multipronged exploratory study shows that direct or indirect effects of TSH may influence the stratum corneum hydration. This correlation seemed very sensitive, as no other specific biophysical parameter was significantly correlated with the thyroid hormonal concentrations in the serum. However, our findings do not exclude the possibility of some other skin changes supervening in case of more severe thyroid dysfunction. The mechanism by which TSH alters the stratum corneum hydration is yet unknown. [less ▲]

BACKGROUND: The observation of photo-exposed skin under ultraviolet light reveals a mosaic pattern of varying intensity in epidermal melanization. Several patterns of mosaic subclinical melanoderma (MSM) have been described using a specially designed CCD camera and the ultraviolet light-enhanced visualization (ULEV) method. Vitamin D(3) and its analogues influence the biology of keratinocytes and melanocytes. OBJECTIVE: To assess the effect of calcipotriol on MSM. METHODS: This randomized split-face study was conducted in 27 men to compare the effect of once daily applications of 5% calcipotriol cream or a moisturizing cream on the heterogeneity of facial MSM. Computerized image analysis of video images was used at 1-month intervals before and during a 2-month treatment, as well as during a 3-month follow-up. RESULTS: At both sites, the average melanin content of the epidermis showed no significant change over time. However, the mottled appearance was smoothened at the calcipotriol site, whereas it was increased at the site receiving the moisturizer. CONCLUSION: The decreased heterogeneity in MSM after calcipotriol applications suggests a control of the epidermal melanocyte unit by the vitamin D(3) derivative. [less ▲]

BACKGROUND: Colours of striae distensae are often different from that of the surrounding skin. A close look using dermoscopy discloses distinct patterns of melanized networks at these sites. The aim of the study was to design a method of high-\resolution analytical analysis of the skin colours using the combination of photographic dermoscopy and small field reflectance colorimetry. METHODS: Clinical photographs were taken from striae distensae and their surrounding skin using a Dermaphot (Heine Optotechnik, Hersching, Germany). A final magnification of 125x was obtained on paper photographs. The reflectance colorimeter Visi-Chroma VC-100 (Biophotonics, Lessines, Belgium) was used to measure colours of the pigmentary networks in the L*a*b* system. Differential colour parameters (deltaE*ab, deltaL*, deltaa*, deltab*) were calculated for each case between the lesional and the surrounding normal skin, and between the melanized reticulated pattern and the enclosed lighter areas. RESULTS: Objective colorimetric assessments distinguished four distinct types, namely striae albae, striae rubrae, striae caeruleae and striae nigrae. The latter peculiar hyperpigmented type of striae distensae was specifically identified by epiluminescence examination in dark-skinned subjects. The fine-melanized honeycomb network present on the adjacent intact skin was reshaped inside striae in a streaky pattern perpendicular to the striae axis. Strong linear correlations were found between all combinations of deltaL* and deltab* evaluating colours of the reticulated and the honeycomb alveolar patterns both inside and outside the striae distensae. By contrast, no correlations were found between deltaa* and the other colorimetric parameters. CONCLUSION: The direct and/or indirect influences of melanocyte mechanobiology appear to have a prominent effect on the various colours of striae distensae. [less ▲]

BACKGROUND: The pathobiological dynamics of psoriatic lesions are complex and difficult to perceive by clinical inspection alone. Non-invasive bioengineering methods may prove to be useful in this field. OBJECTIVE: To identify some subtle capacitance variations in the stratum corneum of chronic psoriasis lesions. METHOD: The newly developed method of skin capacitance imaging was used to provide non-optical images of the hydration of the superficial layers of the stratum corneum. RESULTS: Compared to the uninvolved skin, psoriatic lesions usually showed an overall lowered capacitance, admixed with foci of moderately higher capacitance. Still other sharply circumscribed blotches with higher capacitance were present. The latter aspect corresponded to inflammatory areas. Sweating appeared markedly impaired inside the lesions. The fingerprint of some patients was altered, thus potentially interfering with the current biometric security procedures using the same method. CONCLUSION: Skin capacitance imaging is a non-invasive, non-optical method that distinguishes three contrasting levels of stratum corneum hydration in psoriatic lesions. The lowest capacitance level probably corresponded to xerotic orthokeratosis. The medium capacitance level presumably identified foci of parakeratosis and clumps of neutrophils. The highest capacitance level suggested exsudation at the site of prominent vessel dilation and dermal inflammation. Impaired sweating in the psoriatic lesions may potentially interfere with body thermoregulation. [less ▲]

BACKGROUND: The treatment of chronic leg ulcers remains a stubborn problem in many patients. Topical 2% ketanserin ointment, a 5HT2-serotoninergic blocking agent, has been reported to improve healing of decubitus, venous, diabetic and ischaemic ulcers. METHOD: The present double-blind intra-individual comparative study was performed in 12 women with diabetes presenting with at least two similar leg ulcers. In each subject, the two lesions were randomly assigned to be treated for 8 weeks by 2% ketanserin ointment or its unmedicated vehicle. OBJECTIVE: assessments of the dynamics of wound healing were performed using computerized morphometry. Evaluations were performed at 2-week intervals for 8 weeks. RESULTS: A significant decrease in relative wound area was observed on the ketanserin-treated ulcers compared with the placebo group. CONCLUSION: Topical ketanserin is a valuable therapy for difficult-to-treat leg ulcers. [less ▲]

Nail changes occur in about 25% of systemic lupus erythematosus (SLE) cases. Onycholysis has been reported as the most frequent abnormality in SLE. Nailbed hyperkeratosis may be observed in both SLE and discoid lupus erythematosus (DLE). Involvement of the nail apparatus in DLE is extremely uncommon and never restricted to it. We report on a patient in whom the clinical features on the proximal nailfold were similar to those observed on the skin of a patient with typical DLE. This has, to the best of our knowledge, not yet been reported. The patient also exhibited a very distinctive prominent subungual hyperkeratosis. Interestingly, the patient developed biological alterations suggesting a systematization of the disease. Only a combination of systemic corticoids, retinoids and antimalarials was able to achieve nail improvement and this partial resistance to therapy may be explained by the very unusual subungual hyperkeratosis. BACKGROUND: The innate melanin pigmentation of skin is modulated during lifetime by a series of factors, including ageing and chronic ultraviolet light exposure. Actinic lentigines may be of particular concern from a cosmetic point of view. Conventional hypopigmenting agents are usually deceptive. Using cyclodextrins to form inclusion compounds with these agents might represent a more active drug delivery system. OBJECTIVE: To assess sensitive and objective methods predicting the effects of a 2% hydroquinone-cyclodextrin formulation on solar lentigines. STUDY DESIGN: Thirty Asian adults applied a 2% hydroquinone-cyclodextrin formulation once daily on solar lentigines of a forearm for 2 months. The other untreated forearm served as a control. Monthly assessments were performed using skin colorimetry and fluorescence video recording combined with image analysis. Corneomelametry following photodensitometry of cyanoacrylate skin surface strippings was performed after melanin staining of the samples. RESULTS: The control untreated solar lentigines showed no significant variations in the biometrological parameters. By contrast, the test formulation induced lightening of the solar lentigines. Corneomelametry disclosed improvement after 1 months' treatment, whereas the other assessments only reached significance after 2 months. CONCLUSIONS: A 2% hydroquinone-cyclodextrin formulation proved to be effective in lightening actinic lentigines. Corneomelametry predicted the effects discernible at a later stage by in vivo colour assessments of the melanin content. [less ▲]

BACKGROUND: Dye mixtures containing henna, P-phenylenediamine (PPD), and indigo are applied to the skin in various parts of the world for religious purposes, social recognition or fun. AIM: To identify eventual allergens responsible for contact sensitization to skin decoration in three young children. Lesions remained strictly limited to the site painted during the summer holiday. None of the children had a previous history of contact dermatitis, but two of them suffered from severe seasonal allergic rhinoconjunctivitis. FINDINGS: Patch testing revealed severe contact hypersensitivity to PPD in all three patients, and one child presented a positive prick test to 1% henna in water. The eruptions resolved following spontaneous elimination of the dyes and topical applications of corticosteroids, but left residual depigmentation. CONCLUSION: PPD is the main allergen identified in allergic reactions to decorative skin paintings. [less ▲]

Background: Although the incidence of melanoma is increasing and many informative campaigns have been organized. The general population is still little informed about this tumour. Aims: To organize a media campaign, with more relevant information and the opportunity for free skin inspections. Methods: A ‘Task Force’ organized a media campaign in April 1999 and convinced 65% of the Belgian dermatologists to give up 4 h of their time to do free skin examinations for skin cancer on Monday 26 April 1999; it was called ‘Melanoma Monday’. Results: A total 2767 patients were screened. We found 25 melanomas and suspected 59 basal cell carcinomas. In the following 4 weeks another 141 melanomas were found. These 166 melanomas found in one month represent 15 –20% of the total number of melanomas per year in Belgium. Summary: A media campaign with relevant information combined with screening opportunities can lead to the early detection of melanomas in a large number of patients and can continue to alert people at risk in the following weeks. Background: The prevalence and clinical implications of asymptomatic genital herpes remain elusive. Objective/aim: We report two cases of clinically asymptomatic intrautcrine herpes simplex virus II (HSV-II) infection in order to discuss their pathological significance and demonstrate the potential utility of specific DNA probes and antibodies. Methods: Standard histology, immunohistochemistry (IHC) and in situ hybridization (ISH) were applied to formalin-tixed. paraffin-em bedded lissue sections of curettage specimens from women presenting deciditalised fragments retention after normal delivery. Results: Hisiological analysis revealed a focal clearing of endometrial epithelial cell nuclei, suggesting a viral etiology. ISH and IHC with differetit DNA probes or antibodies confirmed the diagnosis of HSV-II infection at the time of specimen collection. Clinical signs of muco-cutaneous involvement in the mothers and of neonatal HSV infection were not observed. Conclusion: These findings suggest that intrauterinc HSV-II infection is not always associated with disease. The IHC and ISH methods can be helpful to a.ssess the diagnosis of HSV-II intrauterine infections in the presence of optically clear cndotTietrial nuclei. Whether or not such HSV-II occult infection may be associated with viral shedding and risk of sexual transtnission was not evaluated in the present study. © 1997 Elsevier Science B.V. [less ▲]

Aim : A clinical, randomized, parallel, multicenter study was undertaken to compare the efficacy and Ihe tolerability of two known steroid creams, betamethasone 17-valerate and clobetasol propionate, alone or left under a hydrocolloid dressing (Contreei'* Derma Cover), in the treatment of chronic plaques of psoriasis. Methods : A total of 70 patients with symmetrical localized psoriasis were divided randomly into two groups for the study. Each patient applied ihe corticosteroid preparation under Contreet° Derma Cover once a week on one plaque and the same topical corticosteroid alone h.i.d on the other plaque; the treatment lasted 3 weeks for betamethasone 17-valerate, and 2 weeks for clobetasol propionate. Results : The heahng rate reached 79% with the Contreet" combined treatment versus 15% (P < 0,0001) with the standard treatment for betamethasone 17-valeratc (group I) and Sb% versus 14*70 (P < 0. 0001) for clobetasol propionate (group It). The mean difference in healing rate was 65% {95% confidence interval (C.I), 46-84%) and 69% (95% CL, 50-87%) in groups I and II, respectively. Conclusion : The use of the new generation of hydrocolloid dressings in combination with steroid preparations can be considered as another therapeutic option for the treatment of chronic localized psoriatic lesions. Furthermore, with this method the amount of the topical corticosteroid used can be reduced. [less ▲]
