[less ▲]

Background: The aim of this study was to analyze the use of magnetic resonance imaging (MRI) as a modern medical imaging technique in radiotherapy with special emphasis on the integration of MRI and a novel technique in brachytherapy to optimize treatment outcome in cervical cancer. Methods: In addition to the CT based shrinking volume conformal teletherapy in 31 patients with locally advanced cervical cancer, MRI examination with a special adjustable applicator at the treatment site was performed for the brachytherapy planning. To avoid excessive doses to the healthy structures during complex cervical radiotherapy isodose curves were calculated upon the information of the MR image and dose distribution was evaluated. Results: The consecutive application of CT and MRI limited the possibility for overdosage of the critical organs and undertreatment of the advanced tumor spread in all cases. The overall response rate for the complex treatment was 74.2% with complete regression in 25.8% of the cases. Based on the exact information of the three dimensional digital data radiation doses could be optimized without increasing the possibility of acute complications rate. Conclusion: The introduction of 3D treatment planning for teletherapy pelvic and boost irradiation of cervical carcinoma as well as for the brachytherapy part of the complex treatment is to be recommended. Background. Chronic alcohol abuse is associated with a wide range of cognitive deficits. However, little is known about memory for real-life events (autobiographical memory) in non-amnesic alcoholic patients. The purpose of this study was to investigate (a) non-amnesic alcoholics' ability to recall specific autobiographical memories and (b) their subjective experience when they access specific memories.Method. Twenty non-amnesic (without Korsakoff syndrome) recently detoxified alcoholics and 20 healthy controls completed the Autobiographical Memory Test (AMT), which assesses the frequency of specific (versus general) memories recalled in response to cue words, and the Memory Characteristics Questionnaire (MCQ), which assesses subjective experience (e.g. the amount of sensory and contextual details experienced) when remembering specific events.Results. Alcoholic patients recalled specific memories less frequently and general memories more frequently than healthy controls. Nevertheless, when a specific past event was accessed, alcoholic patients subjectively experienced as many sensory and contextual details as controls.Conclusions. These findings suggest that non-amnesic alcoholics have difficulties strategically accessing event-specific autobiographical knowledge, which might result from changes in frontal lobe function that are associated with alcoholism. The aim of concrete surface preparation prior to repair is to obtain a surface with properties close to that of the bulk concrete. In this paper, the results of an investigation intended to evaluate the mechanical degradation induced by different surface preparation techniques are presented. Optical microscope observations clearly show that more cracking is induced with jack-hammers than with the other investigated techniques and that the cracking density increases with the hammer weight. While more information is needed to evaluate quantitatively the effects of concrete removal operations, it appears from the reported results that techniques such as sandblasting, scarifying and water jetting are preferable, as they clearly leave much less defects in the substrate. Nowadays, the management of ectopic pregnancy should be codified as often as possible. Two types of treatment are possible. One surgical and the other medical. Our study is concerned with a series of 97 patients hospitalized for suspicion of ectopic pregnancy for whom the risk factors and treatment were analyzed. Our conclusion is that the medical treatment should be more frequently proposed and surgery should prefer the conservative treatment whenever it is still possible. [less ▲]

The polled locus has been mapped by genetic linkage analysis to the proximal region of bovine chromosome 1. As an intermediate step in our efforts to identify the polled locus and the underlying causative mutation for the polled phenotype, we have constructed a BAC-based physical map of the interval containing the polled locus. Clones containing genes and markers in the critical interval were isolated from the TAMBT (constructed from Angus and Longhorn genomic DNA) and CHORI-240 (constructed from horned Hereford genomic DNA) BAC libraries and ordered based on fingerprinting and the presence or absence of 80 STS markers. A single contig spanning 2.5 Mb was assembled. Comparison of the physical order of STSs to the corresponding region of human chromosome 21 revealed the same order of genes within the polled critical interval. This contig of overlapping BAC clones from horned and polled breeds is a useful resource for SNP discovery and characterization of positional candidate genes. [less ▲]

A study is presented on the crystallization of ornithine acetyltransferase from yeast, which catalyzes the fifth step in microbial arginine synthesis. The use of the counter-diffusion technique removes the disorder present in one dimension in crystals grown by either the batch or hanging-drop techniques. This makes the difference between useless crystals and crystals that allow successful determination of the structure of the protein. The crystals belong to space group P4, with unit-cell parameters a = b = 66.98, c = 427.09 angstrom, and a data set was collected to 2.76 angstrom. [less ▲]

Topology discovery systems are starting to be in- troduced in the form of easily and widely deployed software. Unfortunately, the research community has not examined the problem of how to perform such measurements efficiently and in a network-friendly manner. This paper describes several contributions towards that end. These were first presented in the proceedings of ACM SIGMETRICS 2005. We show that standard topology discovery methods (e.g., skitter) are quite inefficient, repeatedly probing the same interfaces. This is a concern, because when scaled up, such methods will generate so much traffic that they will begin to resemble DDoS attacks. We propose two metrics focusing on redundancy in probing and show that both are important. We also propose and evaluate Doubletree, an algorithm that strongly reduces redundancy while maintaining nearly the same level of node and link coverage. The key ideas are to exploit the tree-like structure of routes to and from a single point in order to guide when to stop probing, and to probe each path by starting near its midpoint. Following the SIGMETRICS work, we implemented Doubletree, and deployed it in a real network environment. This paper describes that implementation, as well as preliminary favorable results. [less ▲]

Most soniferous fishes producing sounds with their swimbladder utilize relatively simple mechanisms: contraction and relaxation of a unique pair of sonic muscles cause rapid movements of the swimbladder resulting in sound production. Here we describe the sonic mechanism for Ophidion barbatum, which includes three pairs of sonic muscles, highly transformed vertebral centra and ribs, a neural arch that pivots and a swimbladder whose anterior end is modified into a bony structure, the rocker bone. The ventral and intermediate muscles cause the rocker bone to swivel inward, compressing the swimbladder, and this action is antagonized by the dorsal muscle. Unlike other sonic systems in which the muscle contraction rate determines sound fundamental frequency, we hypothesize that slow contraction of these antagonistic muscles produces a series of cycles of swimbladder vibration. Background. The spectral entropy of the electroencephalogram has been proposed to monitor the depth of anaesthesia. State Entropy (SE) reflects the level of hypnosis. Response Entropy (RE), computed from electroencephalogram and facial muscle activity, reflects the response to nociceptive stimulation. We evaluated the effect of rocuronium on Bispectral Index (TM) (BIS) and entropy responses to laryngoscopy. Methods. A total of 25 patients were anaesthetized with propofol using a target-controlled infusion. At steady state, they randomly received 0.6 mg kg(-1) rocuronium (R) or saline (S). After 3 min, a 20 s laryngoscopy was applied. BIS, RE and SE were recorded continuously and averaged over 1 min during baseline, at steady state, 2 min after R or S administration (R/S+2) and 0, 1, 2 and 3 min after laryngoscopy (L0, L1, L2, L3). Results. At R/S+2, the RE-SE gradient was higher in Group S than in Group R. Laryngoscopy provoked an increase in BIS, RE and SE. Comparing R/S+2 and L0 values in Groups R and S, BIS increased from 43 (6) to 49 (8) and 42 (9) to 51 (15), SE increased from 43 (7) to 50 (8) and 41 (10) to 55 (12), and RE increased from 46 (8) to 54 (9) and 47 (12) to 66 (15), respectively. BIS and SE did not differ between groups. At L0, RE and RE-SE were higher in Group S [66 (15) and 11 (4), respectively] than in Group R [54 (9) and 4 (2), respectively]. Conclusions. Rocuronium alters the RE-SE gradient and the RE and RE-SE responses to laryngoscopy. Muscle relaxation may confound interpretation of entropy monitoring. [less ▲]

This work presents the results of surgery in thoraco-abdominal aortic aneurysms (TAA) and thoracic descending aortic aneurysms (TDA) in one single center between January 1rst, 1996 and December 31, 2005. It concerns open surgery in 42 and endovascular procedures in ten patients. Forty two patients (11 TDA and 31 TAA (4 type I, 12 type II, 6 type III and 9 type IV)) define the open surgery series. Twenty six patients were operated on elective basis and 16 patients in emergency condition. Surgical correction was made under partial cardio-pulmonary bypass (PCPB) in 70% of cases via femoral vessels; most significant intercostal arteries were reimplanted and cerebro-spinal fluid (CSF) drainage used in half of the cases. Operative mortality was zero in the elective group (0/26) and attained 19% in the emergent group (3/16). Mortality was linked to cerebrovascular accidents (CVA) in two cases and post-pump left lung hemorrhagic intarction in one case. The paraplegia accounts 2/26 in the elective group and one in the emergent group (1/16). That is 7.1% in both groups. At the end of five years, survival is 66% in elective group and 74% in the emergency group. Ten patients (5 TDA and 5 TAA (2 type I, 3 type III)) were treated endovascularly. Operative mortality and postoperative paraplegia were nil. [less ▲]

Successful embryo development requires an extensive endometrial angiogenesis in proximity of implantation site. The glycoprotein hCG is produced even before implantation by trophoblast in normal pregnancy. In this manuscript, we demonstrate an angiogenic effect of hCG in several in vivo (chick chorioallantoic membrane, matrigel plug assay, aortic ring assay) and in vitro experimental models. In contrast, human placental lactogen (hPL) did not display angiogenic properties. LH/hCG receptor was detected in endothelial cells by reverse-transcriptase polymerase chain reaction (RT-PCR) and by Western blotting. In mice aortic ring assay, angiostimulation by hCG was abrogated by deletion of LH/hCG receptor (LuRKO mice). Use of recombinant hCG and anti-hCG antibody (Ab) further confirmed the specificity of this angiogenic activity. By using dibutyryl cAMP, adenylate cyclase, or protein kinase A inhibitors, we demonstrate that hCG-mediated angiogenesis involves adenylyl-cyclase-protein kinase A activation. Addition of hCG to endometrial epithelial epithelial cells, but not to cultured endothelial cells, stimulated vascular endothelial growth factor (VEGF). VEGF and hCG also displayed additive activities. Altogether, these data demonstrate that peritrophoblastic angiostimulation may result from a paracrine dialogue between trophoblast, epithelial, and endothelial cells through hCG and VEGF. [less ▲]
