We aimed to investigate the dynamics of adrenocorticotropin (ACTH) and cortisol secretion in pituitary-dependent Cushing's syndrome with bilateral macronodular adrenal hyperplasia presenting as a single adrenal macronodule, and to determine the imaging characteristics of this syndrome. Three female patients were studied. Plasma ACTH and serum cortisol secretion were studied by determining their rhythmicity and pulsatility and their responses to the administration of ovine corticotropin-releasing factor, thyrotropin-releasing hormone, metyrapone, tetracosactrin, insulin and dexamethasone. Techniques used to localize the anatomical lesion were bilateral simultaneous inferior petrosal sinus sampling, magnetic resonance examination of the pituitary, computed tomography (CT) scanning and [75Se]cholesterol scintigraphy of the adrenal glands. Plasma ACTH and serum cortisol levels were measured using a commercial radioimmunoassay and an immunoradiometric assay. The ACTH and cortisol pulse number and amplitude were calculated using established computer software. In all three patients ACTH and cortisol secretory dynamics fulfilled the requirements for diagnosis of pituitary-dependent Cushing's syndrome. A close relationship between ACTH and cortisol pulses also favored a pituitary dependency. Study of the amplitude of cortisol pulses classified two patients in the group of hypopulsatile Cushing's disease. Adrenal CT scanning demonstrated the presence of a large single nodule. [75Se]Cholesterol scintigraphy showed bilateral radionuclide uptake, although mostly localized over the adrenal nodule. All patients underwent successful trans-sphenoidal hypophysectomy. We propose here ET-LOTOS, a timed extension of LOTOS. It is an enhancement of Timed LOTOS that we presented at FORTE 92. We show how some simple modifications allow us to improve the expressiveness of our former formalism. To assess ET-LOTOS, we apply it to the specification of a small case study. Finally, we show how the semantics of ET-LOTOS could be modified to easily define more powerful operators, if one ever had to. [less ▲]

Thus far, only very few cases with neuronal maldevelopment in the peripheral nervous system have been reported (Table 1). The present sporadic case manifested itself with peripheral sensorimotor polyneuropathy in early infancy. Clinical findings included pareses and hypaesthesia of distal extremities and severely reduced nerve conduction velocities. During adolescence, cerebellar ataxia developed. Sural nerve biopsy taken at the age of 14.5 years showed severe fascicular hypoplasia, aplasia of large myelinated nerve fibers, and subtotal deficiency of small myelinated nerve fibers without numerical reduction of unmyelinated axons. There was no structural evidence of a progression of myelinated fiber breakdown although some collagen pockets and empty Schwann cell processes among preserved unmyelinated axons indicated some loss of unmyelinated fibers. These findings are interpreted as representing maldevelopment of the myelinated fibers in the peripheral nervous system. Appropriate classification of this unique disease among the known developmental disorders of peripheral nerves is discussed. [less ▲]

The alterations in cerebral hemodynamics during and following 39 episodes with bradycardia of different severity have been studied by analysis of Doppler flow velocity waveforms amongst 16 stable preterm infants (range of conceptional age at the time of study 33-39 weeks; weight 1730-2820 g). Each episode with bradycardia has been classified as mild on seven occasions (heart rate between 100-120 beats/min), moderate on 15 occasions (80-100), or severe on 17 occasions (heart rate below 80) Depending on the severity of the bradycardia, the time-averaged mean flow velocity (V) could decrease by 80% below the preexistent baseline value and the end-diastolic velocity (D) dropped towards the zero line, whereas peak systolic velocities did not change The magnitude of the percentage decrease of V correlated positively with the severity of bradycardia, indicating a progressive decline of cerebral blood flow (CBF). Following bradycardia, V could increase up to 75% above the preexistent baseline value. Simultaneously, an increase of mean arterial blood pressure and D could be documented. Peak systolic velocities remained unaltered. The magnitude of the percentage increase of V following bradycardia depended both on the severity of bradycardia as well as on the drop in transcutaneous oxygen during the preceding bradycardia. Interpretation of these findings suggested that the transient cerebral hyperperfusion following bradycardia compensates for the hypoxic-ischemic episode, sustained during the preceding epoch of apnea and bradycardia. The isolation of preantral follicles from the ovaries of bovine fetuses, calves and adult cows was performed using a simple, rapid mechanical and enzyme method. The ovaries were cut into small pieces with a tissue chopper. Then, the suspension was filtered successively through 500 and 100 μm nylon mesh filters. This simple mechanical procedure resulted in large numbers of isolated preantral follicles: 2,142 ± 254; 512 ± 92 and 298 ± 54 from the ovaries of bovine fetuses, calves and cows, respectively. In addition, the ovarian fragments between 100 and 500 μm were suspended in 10 ml of M199 Hepes medium plus 5% FCS and divided into 2 equal parts: one portion was used for collagenase treatment (200 U/ml) for 20 minutes, while the other served as a control. Collagenase treatment resulted in 841 ± 161; 216 ± 51 and 52 ± 17 preantral follicles from fetuses, calves and cows, respectively, compared with 312 ± 86; 52 ± 15 and 10 ± 2 in the control group. The use of collagenase with ovarian fragments selected by filtration as a method for increasing the rate of recovery of preantral follicles is described here. [less ▲]

In this study we report on the adsorption behavior of end-attaching triblock and diblock copolymers. The triblock copolymers consist of relatively short PEO end blocks and a much longer PS middle block or, in one case, of zwitterionic end groups with a PS middle block. The diblock materials consist of a PEO block of fixed length and varying polystyrene lengths. A silicon wafer with an oxide layer is used as the surface, and adsorption takes place from toluene; only the end groups adsorb, leaving the polystyrene block dangling in solution. We find that diblock copolymers which range from symmetric to moderately symmetric obey the scaling law for the surface coverage which was proposed in the theory of Marques and Joanny (i.e., sigma is-proportional-to 1/N(A), where N(A) is the number of segments in the adsorbing block). However, we find that for triblock copolymers of comparable asymmetry the surface coverage scales according to the relationship sigma is-proportional-to 1/beta2, where beta is the ratio of the size of the nonadsorbing block to that of the adsorbing block. This implies that moderately symmetric triblock copolymers behave like highly asymmetric diblock copolymers; the adsorption behavior is dominated by the nonadsorbing block size. Our experiments demonstrate the observation of extremely long-lived nonequilibrium states. On the basis of these observations we propose a simplified version of a recent theory of adsorption which is capable of qualitatively describing much of the experimentally observed phenomena. The mechanism of fragmentation during the SIMS process is examined, and the contribution from naturally occurring isotopes is evaluated. The data are shown to fit a model of a completely random copolymer system if they are corrected for the effects of impurities introduced in the syntheses and for the contribution from C-13. [less ▲]

We present four children from two families with the typical 11q- phenotype resulting from an unbalanced segregation of a parental translocation. In the first family, the father had a 46,XY,t(5;11)(q24;q23.3) constitution. The father of the three other children had a 46,XY,t(11;17)(q23;p13) translocation. Despite associated partial deletion, three of the children had a typical 11q- phenotype. The fourth one, whose pregnancy was terminated in the second trimester, had a hypoplastic left heart but no other considered gross anomalies. A review of 36 previous cases, including 5 due to translocations (4 familial rearrangements, and 1 of unknown origin) is given with emphasis on the relationships between break-points and phenotype. Undescribed manifestations in our patients include agenesis of corpus callosum adactyly and malrotation of the gut. [less ▲]
