Any designer (architect, consulting engineer...), who must choose the technical solution of a heating installation in a house, is always confronted with the same alternatives: combined production of both hot water for heating needs and domestic hot water, assured by a combined boiler or a boiler producing only heating hot water and a separate water heater (instantaneous or with storage) for domestic hot water needs. The objective of this present study is to give the designer simple graphs which become an easy way to assume, with a good accuracy, the “global annual efficiency” of each solution, and allow a sensitivity study of efficiency to main parameters of each system. Thirty hospitalized, severely obese patients (40 +/- 2 yr, 82 +/- 4 percent weight excess) were submitted to a 13-d protein-supplemented fast (PSF) with 70 g milk proteins/d (1.26 MJ or 300 kcal). The mean weight loss during PSF was 5.4 +/- 0.3 kg corresponding to 422 +/- 39 g/d. Comparison of the urinary nitrogen excretion with daily protein intake revealed that the nitrogen balance was equilibrated during PSF. Blood glucose decreased moderately but significantly during the whole PSF period whereas plasma insulin was only reduced during the first 9 d and tended to rise thereafter. Plasma FFA increased rapidly and remained elevated until the end of the study (+ 60 per cent); serum total cholesterol and plasma triglycerides showed a 26 and a 35 per cent decrease respectively. Basal plasma glucagon was slightly increased. Due to the low sodium intake (42 mmol/d) urinary sodium excretion dropped rapidly. Simultaneously both systolic (-13 mmHg) and diastolic (-7 mmHg) arterial blood pressure decreased significantly. The biological tolerance was good: metabolic acidosis was prevented with sodium bicarbonate, excessive rise in serum uric acid was corrected with allopurinol and a marked decrease in serum potassium was avoided with an appropriate dose of spironolactone. Twenty-six patients could be weighed 6 to 15 months after PSF: 12 showed a further weight reduction (6.6 +/- 1.6 kg) and seven a discrete weight gain (1.0 +/- 0.4 kg). Thus, PSF was well accepted and was profitable in 19 out of our 30 patients. It should be restricted to cases of severe and refractory obesity and performed under careful medical supervision. Serial measurements of serum uric acid were performed on patients suffering from acute myocardial infarction. Nearly 80 percent of the cases demonstrated a fall in uric acid concentrations during the first two days of hospitalization and a subsequent return to initial levels within six to eight days. There was a relationship between the decrease in uric acid levels and the serum lactate dehydrogenase activity. No evidence could be found that male patients were hyperuricemic as compared to control subjects. However, female patients between 40 and 60 years of age demonstrated significantly higher uric acid levels than healthy women of corresponding ages, even after adjustment for diuretic use. [less ▲]

We measured the c.s.f. and serum creatine kinase activities in the first hours following the trauma, for 24 severe head injury patients. We found a good correlation between total CK and CKBB isoenzymes levels in the c.s.f. only. The c.s.f. values are a quantitative index of brain dysfunction assessed by the Liege coma scale, associating the Glasgow coma scale and the study of the brain stem reflexes. We investigated the relation between haptoglobin (Hp) phenotypes and serum levels of various biochemical markers after myocardial infarction in 496 patients. In 122 subjects selected on the basis of short delays until hospitalization, patients with Hp 2-2 had higher cumulated creatine kinase activity than patients with Hp 1-1, or Hp 2-1 (P less than 0.05), as well as higher myoglobin concentrations (P less than 0.02) 12 to 28 hours after admission. Comparison of serum enzyme activities in the remaining 374 patients confirmed that Hp 2-2 patients had significantly higher total creatine kinase, creatine kinase isoenzyme MB fraction, aspartate aminotransferase, and lactate dehydrogenase peak levels. Complications of left ventricular failure were more frequent in these patients (P = 0.05). Our results suggest that Hp 2-2 patients have more severe myocardial infarctions than Hp 1-1 and Hp 2-1 patients, However, no difference in the distribution of haptoglobin phenotype was found between patients who had a myocardial infarction and healthy subjects, indicating that Hp 2-2 does not predispose to the occurrence of infarction. Established essential HTA is considered to result from the influence of both heredity and environment. Among the environmental factors, dietetary sodium has been proposed to play an important role inducing an increase in peripheral vascular resistance. The primary abnormality in essential HTA could consist in a genetically transmitted membrane defect impairing the cellular mechanisms which normally compensate a sodium load. The analysis of this defect implies more certainly elucidation of the effects of sodium ions at the blood vessel musculature level. But smooth muscle cells are unfortunately rather inaccessible. Yet it would be of great interest to dispose of a simple laboratory test to detect primary HTA. Therefore, on account of their availability and ease of manipulation, blood cells have been studied and already twenty years ago, LOSSE (12) recorded an increase of the intracellular sodium concentration in erythrocytes of hypertensive patients. Since then, different groups described different abnormalities' in the movement of ions across the blood cells membranes. Indeed modifications of the passive permeability (11, 12, 13), the sodium potassium pump (1, 5, 14), the sodium lithium countertransport (2, 7, 18) and the sodium potassium cotransport (3, 8, 9,10) have been successively demonstrated. GARAY et al. (9,10,11) observed a decrease in this last transport system for essential hypertensives and a high proportion of their young offspring. Conversely for normotensive patients, without a family history of HTA or in secondary hypertensives, a normal cotransport was found. The finding of an abnormal cotransport in a young normotensive may thus elicit preventive measures such as appropriate sodium diet, prevention of excessive weight gain, clinical follow up during oral contraception or pregnancy. The first observations showed a very clearcut difference between the two populations. However, many authors using the same test procedure, found a large overlap suggesting the measurement of this cotransport will not prove useful to discriminate primary HTA. However, the methodology was not always the same and difference could also arise from the definition of normal control groups which needed to be carefully selected. Thus in this study, we would evaluate the modification of the Na+ K+ cotransport in primary and secondary HTA, using strictly the experimental methodology originally described by GARAY et al. (9, 10). Afterwards, it would be interesting to study the effect of obesity, diabetes and the menstrual cycles on this transport system. [less ▲]
