Despite considerable advances in antifungal therapies over the past years, invasive mycoses remain a stubborn and dramatic problem. Voriconazole is a new triazole antifungal agent which confers a relative survival benefit in fluconazole-resistant invasive candidiasis, and in invasive aspergillosis, fusariosis and Scedosporium infections as well. The diversity of onychomycoses is large when the disease is scrutinized using traditional, fluorescence or confocal microscopy. Histomycology is a non-invasive assessment performed on nail clippings. The location of and the density in fungal cells is variable. In some instances, these aspects remain clinically unsuspected. In vivo confocal microscopy can bring the same information. Computerized image analysis of histological sections is the most performant means for quantifying the fungal load. Immunohistochemistry brings information about the identity of the fungus or the association of different fungi present in the nail plate. Mixed infections may be unifocal or located at different levels in the nail apparatus. The viability of fungi as assessed by vital stains can be assessed under the microscope and quantified by flow cytometry. The different facets of nail histomycology are complementary and shed some light on aspects sometimes unsuspected of onychomycoses. [less ▲]

Gestation may also be a period during which the aspect of some melanocytic naevi can change eventhough the tumor remains benign. Distinction between a benign and a malignant lesion may then be a dilemma. Clinical morphometry, dermoscopy, skin surface biopsy, conventional histology and immunohistochemistry are valuable means that can be used to secure maximum diagnostic confidence. [less ▲]

BACKGROUND: Melanotic schwannoma is a pigmented nerve tumor that may be located in the skin and express local aggressivity. This tumor may occur singly. It may also be part of the Carney complex which consists of various, but specific, tumors. OBJECTIVE: We report two cases of subcutaneous melanotic schwannoma localized on the trunk in two men aged 37 and 45 years. METHODS: Conventional histology and immunohistochemistry were performed. RESULTS: One melanotic schwannoma was associated with a cutaneous atypical myxoma and multiple melanocytic lesions, all being part of the Carney complex. The other case had no associated signs. In both cases, the melanotic schwannoma was completely excised and did not recur. CONCLUSION: Melanotic schwannoma is rare and curable by surgery. It must not be confused with malignant melanoma and other pigmented neoplasms. The Carney complex should be carefully ruled out. [less ▲]

The therapeutic outcome of onychomycoses is uncertain. Comparative short-term efficacy studies on antifungals abound and report contradictory findings. Few unbiased follow-up studies have scrutinized the long-term outcome. Basically, none of the current antifungals can guarantee cure in all instances. In addition, relapses are not rare. The causes of therapeutic failure in onychomycoses are multiple. The most important are the lack of diagnostic accuracy, inadequate antifungal choice or delivery modality, and presence of dormant conidia, sequestrated mycelium pockets or resistant fungal species. The concept of fungicidal drug derived from selected in vitro studies appears irrelevant in clinical practice. [less ▲]

Thus, anthropophilic tinea capitis is highly predominant in areas where new immigrants from Black Africa and North Africa have settled. By contrast, zoophilic tinea capitis is found largely in areas where the cat population is infected. Kerion often affects professions in which contact with infected livestock is involved. Prevention of tinea capitis thus has distinct regional features. Treatment, on the other hand, has a universal basis. [less ▲]

Histiocytes belong to distinct groups of cells. Similarly, histiocytoses represent some pathologic expressions of the various histiocyte lineages recognized by their distinct phenotypes. The recognition of four major histiocytosis types relies on the distinction between monocytes-macrophages, dermal dendrocytes, Langerhans cells and the so-called indeterminate cells. Each of these groups exhibits a distinctive clinical and histological presentation. The risk of internal involvement, the prognosis and therapy are different as well. The diagnostic mainly relies on the clinico-pathologic assessment, and more particularly on the combination of histological, immunohistochemical and ultrastructural examinations. [less ▲]

BACKGROUND: Radiotracer and blue-dye lymphatic mapping is a recommended combined method to guide sentinel lymphadenectomy and full regional lymph node dissection in selected patients with cutaneous melanoma. OBJECTIVE: To evaluate the diagnostic accuracy and the prognostic value of gamma-probe-directed lymphatic mapping in cutaneous melanomas. METHODS: Sixty-five stage I and II melanoma patients underwent gamma-probe-directed lymphatic mapping. Sentinel lymph nodes were studied by both conventional and immunohistochemical stainings. The median follow-up was 11 months. RESULTS: Sensitivities of preoperative and intraoperative sentinel lymph node detection were 100 and 98%, respectively. Only 1 failure of detection and 1 missed same-basin metastasis were experienced in the axillary and cervical areas, respectively. Eleven patients (16.9%) had sentinel node metastases leading to adjuvant therapy. CONCLUSION: Gamma-probe-directed lymphatic mapping is useful for staging melanoma. However, in the expectation of a more specific identification of the sentinel lymph node, the standard protocol remains recommended for exploring the axillary and cervical areas. The histological examination supported in some cases by immunohistochemistry remains mandatory in all cases. [less ▲]

Some cutaneous functions of importance are governed by periodical variations according to the nycthemeral timing. Similarly to other organs, the skin is under the influence of a coherent organisation of circadian and nythemeral rhythms modulating various biological cycles which usually display a wide amplitude. The cutaneous biorhythms in humans suggest that during daylight the skin boosts diverse protective functions with regard to environmental threats. In the evening and at night, the skin increases its renewal and diverse metabolic processes. Biorhythms in the skin which are in line with the organism's needs, merit to be recognized. Chronophysiology, chronopathology and clinical chronopharmacology may help reach optimal therapeutic decisions. [less ▲]
