Sexually transmitted diseases (STD) show frequent epidemiological changes. We report a case of secondary syphilis diagnosed in a 32-year old woman during post-partum. The disease, although showing a typical presentation, raises the problem of dating with precision the contamination time. This is of importance for treating the newborn at risk of congenital syphilis. We highlight the constant attention that should be brought to disclose this venereal disease that shows so variable clinical and histological presentations. Syphilis seems on the rise in the Mosan valley and in some cities where prostitution is imported from Eastern Europe and Africa. [less ▲]

Yeasts of the genus Malassezia are part of the normal skin biocenosis and are involved in a series of distinct skin disorders and specific dermatomycoses in man and animals. Several species are currently distinguished. Their relative in vitro susceptibility to antifungals appears different according to the species and to the nature and route of administration of the drug. Corneofungimetry is an ex vivo bioassay allowing to test the fungal response on human stratum corneum following oral intake of a given antifungal by volunteers. Two series of cyanoacrylate skin surface strippings (CSSS) were harvested from the volar forearm of 30 volunteers before and after a 2-week treatment with itraconazole 200 mg daily. They were coated by olive oil and inoculated with suspensions of seven different Malassezia spp. After a 1-week culture on CSSS, the amount of viable yeasts was assessed using neutral red staining assisted by computerized image analysis. Growth of the seven species was not similar on the CSSS from untreated stratum corneum. The ranking order from the most proliferative to the least was M. restricta, M. sympodialis, M. globosa, M. furfur, M. obtusa, M. slooffiae and M. pachydermatis. Their growth was abated to almost the same level after itraconazole treatment. It is concluded that in vivo treatment with itraconazole is highly active against all Malassezia spp. colonizing the human stratum corneum. [less ▲]

BACKGROUND: Psoriasis is an immunogenetic disorder. Factor XIIIa+ dermal dendrocytes (DD) are part of the pathobiological changes in the plaque type of the disease. OBJECTIVE: The present study aimed at comparing the effect of 3 vitamin D(3) derivatives on the epidermis, microvasculature and DD in psoriasis. METHOD: Twenty men suffering from chronic plaques of psoriasis on the trunk were enrolled in this study. They applied twice a day for 3 weeks calcipotriol, tacalcitol and calcitriol, each to one plaque. Another similar lesion received petrolatum as a placebo treatment. Skin biopsies were taken at entry and at completion of the 3-week treatment phase. Immunohistochemistry was performed using the lectin of Ulex europaeus and an antibody to factor XIIIa. Computerized image analysis served to measure the stratum Malpighii area, the microvasculature area and the DD numerical density in the papillary dermis. RESULTS: At entry in the study, the 4 test sites were indistinguishable with regard to the stratum Malpighii area, the papillary microvasculature area and the papillary DD density. The 3 histometric parameters appeared correlated with each other. At completion of the 3-week treatment phase, the 3 vitamin D derivatives had decreased the size of the stratum Malpighii. In addition, calcitriol had also reduced the DD density in the papillary dermis. No other significant changes were yielded. CONCLUSION: As assessed by histometry, the psoriatic epidermis responded to a short treatment using the 3 vitamin D derivatives. The better result compared to the control site was achieved by calcitriol. DD appeared to be most controlled by the same drug. The microvasculature did not appear to be decreased at the 3-week time point in treatment. [less ▲]

The diversity of onychomycoses is large when the disease is scrutinized using traditional, fluorescence or confocal microscopy. Histomycology is a non-invasive assessment performed on nail clippings. The location of and the density in fungal cells is variable. In some instances, these aspects remain clinically unsuspected. In vivo confocal microscopy can bring the same information. Computerized image analysis of histological sections is the most performant means for quantifying the fungal load. Immunohistochemistry brings information about the identity of the fungus or the association of different fungi present in the nail plate. Mixed infections may be unifocal or located at different levels in the nail apparatus. The viability of fungi as assessed by vital stains can be assessed under the microscope and quantified by flow cytometry. The different facets of nail histomycology are complementary and shed some light on aspects sometimes unsuspected of onychomycoses. [less ▲]

Gestation may also be a period during which the aspect of some melanocytic naevi can change eventhough the tumor remains benign. Distinction between a benign and a malignant lesion may then be a dilemma. BACKGROUND: Melanotic schwannoma is a pigmented nerve tumor that may be located in the skin and express local aggressivity. This tumor may occur singly. It may also be part of the Carney complex which consists of various, but specific, tumors. OBJECTIVE: We report two cases of subcutaneous melanotic schwannoma localized on the trunk in two men aged 37 and 45 years. METHODS: Conventional histology and immunohistochemistry were performed. RESULTS: One melanotic schwannoma was associated with a cutaneous atypical myxoma and multiple melanocytic lesions, all being part of the Carney complex. The other case had no associated signs. In both cases, the melanotic schwannoma was completely excised and did not recur. CONCLUSION: Melanotic schwannoma is rare and curable by surgery. It must not be confused with malignant melanoma and other pigmented neoplasms. The Carney complex should be carefully ruled out. [less ▲]

The therapeutic outcome of onychomycoses is uncertain. Comparative short-term efficacy studies on antifungals abound and report contradictory findings. Few unbiased follow-up studies have scrutinized the long-term outcome. Basically, none of the current antifungals can guarantee cure in all instances. In addition, relapses are not rare. The causes of therapeutic failure in onychomycoses are multiple. The most important are the lack of diagnostic accuracy, inadequate antifungal choice or delivery modality, and presence of dormant conidia, sequestrated mycelium pockets or resistant fungal species. The concept of fungicidal drug derived from selected in vitro studies appears irrelevant in clinical practice. [less ▲]

Thus, anthropophilic tinea capitis is highly predominant in areas where new immigrants from Black Africa and North Africa have settled. By contrast, zoophilic tinea capitis is found largely in areas where the cat population is infected. Kerion often affects professions in which contact with infected livestock is involved. Prevention of tinea capitis thus has distinct regional features. Treatment, on the other hand, has a universal basis. [less ▲]

Histiocytes belong to distinct groups of cells. Similarly, histiocytoses represent some pathologic expressions of the various histiocyte lineages recognized by their distinct phenotypes. The recognition of four major histiocytosis types relies on the distinction between monocytes-macrophages, dermal dendrocytes, Langerhans cells and the so-called indeterminate cells. Each of these groups exhibits a distinctive clinical and histological presentation. The risk of internal involvement, the prognosis and therapy are different as well. The diagnostic mainly relies on the clinico-pathologic assessment, and more particularly on the combination of histological, immunohistochemical and ultrastructural examinations. [less ▲]
