Background: Human cutaneous malignant melanoma (CMM) is an aggressive cancer showing a dramatic worldwide increase in incidence over the past few decades. The most prominent relative epidemiological increase has been disclosed in young women. The aim of the study was to assess the effects of chronic sun exposures in order to rate the extend of melanocytic stimulations in the vicinity of CMM. Methods: The study was designed to evaluate the melanin distribution and density using ultraviolet light illumination. The present study was performed on surgical excision specimens of thin CMM lesion removed from the upper limbs of 55 Caucasian adults (37 women and 18 men). Two control groups comprised 23 men and 21 women of similar ages who had medium-size congenital melanocytic nevi, also present on the upper limbs. The peritumoral skin was scrutinized using a Visioscan® VC98 device, revealing the faint mosaic melanoderma (FMM) pattern that grossly indicates early signs of chronic photodamage in epidermal melanin units. Results: The median extent of relative FMM was significantly higher in the CMM male group. By contrast, the CMM female group showed a reverse bimodal distribution in FMM size. Only 12/37 (32.5%) of the CMM female group had an increased FMM size, whereas 25/37 (67.5%) of females with CMM had a global FMM extent in the normal range, relative to the controls. Conclusion: Thin CMM supervening in young women appear unrelated to repeat photoexposure. Other mechanisms are possibly involved. [less ▲]

alpha-Hydroxy acid (AHA) formulations are commonly used for skin chemical peelings. The primary target is the stratum corneum (SC). The aim of this study was to assess the effects of various glycolic acid concentrations and commercial phenolic acid formulations on the SC. Quantitative colorimetry of a corneoxenometry bioassay was used. The test procedure involved glycolic acid concentrations ranging from 3% to 70% in alcoholic solution. Exposure times were set for 1 min and 3 min. The bioassay showed consistent reactivity with a dose-effect relationship when using the selected low exposure times. In a similar procedure the aggressiveness of commercially available phenolic acid formulations was identified not using hazardous in vivo testing. Corneoxenometry appears useful for in vitro testing of AHA peeling agents during short exposure times. [less ▲]

BACKGROUND: Climacteric skin aging affects certain biophysical characteristics of facial skin. The purpose of the present study was to assess the symmetric involvement of the cheeks in this stage of the aging process. METHODS: Skin viscoelasticity was compared on both cheeks in premenopausal and post-menopausal women with indoor occupational activities somewhat limiting the influence of chronic sun exposure. Eighty-four healthy women comprising 36 premenopausal women and 48 early post-menopausal women off hormone replacement therapy were enrolled in two groups. The tensile characteristics of both cheeks were tested and compared in each group. A computerized suction device equipped with a 2 mm diameter hollow probe was used to derive viscoelasticity parameters during a five-cycle procedure of 2 seconds each. Skin unfolding, intrinsic distensibility, biological elasticity, and creep extension were measured. RESULTS: Both biological elasticity and creep extension were asymmetric on the cheeks of the post-menopausal women. In contrast, these differences were more discrete in the premenopausal women. CONCLUSION: Facial skin viscoelasticity appeared to be asymmetric following menopause. The possibility of asymmetry should be taken into account in future studies of the effects of hormone replacement therapy and any antiaging procedure on the face in menopausal women. [less ▲]

Senescence of people represents a global expression of obsolescence of their organs, tissues, cells and constitutive molecules. Skin, similarly to any other organ, is ageing in particular ways. Over the past century, the time effects on skin have been expressed differently. Skin of any individual presently engaged in the Third Age looks different from that of his/her line ancestral. What is the expected future? The Third Age population is expanding and skin problems call for a variety of management procedures. Prevention of the diverse types of skin ageing has made tremendous progresses particularly in the field of preventive and corrective dermocosmetology. The future should further speed up such trends. BACKGROUND: The gravitational syndrome resulting from venous pressure elevation occasionally develops on the legs during pregnancy. The limb tends to enlarge and become stiffer. The body contours are altered. AIMS: To assess incipient gravitational edema due to chronic venous insufficiency using measurements of the skin tensile strength. METHOD: A total of 21 women aged 28-37 years were enrolled in the study. Evaluations were made twice in each subject following an alternate use and avoidance of daytime elastic contention. Skin viscoelasticity was measured on the mid portion of the calves using a computerized suction device. RESULTS: The discretely increased consistency of skin showing abnormal rheological characteristics at the site of incipient gravitational edema was significantly improved by contention therapy. Under progressive suction measurements, both skin distensibility, biologic elasticity and hysteresis were increased after wearing tight stockings. The biologic elasticity appears to be the most sensitive parameter pointing to the diagnosis of gravitational syndrome. CONCLUSION: Noninvasive measurements of the skin viscoelasticity, particularly the biologic elasticity, represent an objective assessment of both early gravitational edema and its control by contention therapy. BACKGROUND: Climacteric ageing refers to the period of women's life centred by the perimenopause. This step of life is responsible for initiating skin atrophic withering and slackness responsible for changes in the viscoelasticity of the dermal connective tissue. Some women benefit from hormone replacement therapy (HRT) for controlling some unpleasant internal climacteric changes (osteoporosis, vasomotor instability, ...). OBJECTIVE: To investigate some corrective effects of HRT on the functional damages in the dermal tensile strength. PATIENTS AND METHODS: This study was performed on 200 healthy Caucasian women. In vivo skin viscoelasticity was assessed using the suction method. RESULTS: Skin distensibility and hysteresis appeared increased at menopause with or without HRT. By contrast, the biological elasticity significantly decreased out of HRT. CONCLUSION: Some drugs and cosmetics might attempt at improving such viscoelastic attributes of the skin at menopause. Dandruff and seborrheic dermatitis form a continuum of desquamative conditions associated with the excessive colonization of the stratum corneum by yeasts of the genus Malassezia spp. Such colonization is heterogeneous in density, ranging from discrete to abundant according to the fungal ability to adhere to individual corneocytes. The rate of action of antidandruff shampoos shed some doubts about a direct antifungal effect, but rather suggests the involvement of innate immune defenses and the chemical alteration of molecular adhesion modalities between Malassezia and corneocytes. The fund of knowledge regarding the versatility of presentation of MM metastases is still quite incomplete. The recent literature pertaining to the current understanding of the mechanisms underlying two special features of MM metastasis is reviewed. On the one hand, a long disease-free interval (MM dormancy) may occur before the surge of overt metastases. On the other hand, the so-called MM smouldering phenomenon refers to the condition where regional metastases wax and wane for long periods of time on restricted skin regions. It is important to emphasize that local micrometastases often predict sentinel lymph node involvement but may not reflect progression of the primary MM to full-blown visceral metastatic competence. It is likely that a combination of factors impacts the versatile MM metastasic progression. Among the main factors, one has to mention the phenotypic heterogeneity and variability in the phenotype of MM cells, the presence of MM stem cells and MM cells engaged in an amplification proliferation pool, as well as the host immune response, and possibly the induction of a particular stromal structure and vascularity. At present, immunohistochemistry is taken for granted in the establishment of malignant melanoma (MM) diagnosis. In recent years, molecular diagnosis in dermatopathology has benefited from a vast array of advances in the fields of genomics and proteomics. Sensitive techniques are available for detecting specific DNA and RNA sequences by molecular hybridization. This paper intends to update methods of molecular cytogenetics available as diagnostic adjuncts in the field of MM. Cytogenetics has highlighted the pathogenesis of atypical melanocytic neoplasms with emphasis on the activation of the mitogen-activated protein kinase (MAPK) signalling pathway during the initiation step of the neoplasms. 20 to 40% of MM families have mutations in the tumour suppressor gene p16 or CDKN2A. In addition, somatic mutations in p16, p53, BRAF, and cKIT are present in MM. Genome-wide scan analyses on MM indicate positive associations for genes involved in melanocytic naevi, but MM is likely caused by a variety of common low-penetrance genes. Molecular dermatopathology is expanding, and its use in the assessment of melanocytic neoplasms appears to be promising in the fields of research and diagnosis. Molecular dermatopathology will probably make its way to an increased number of diagnostic laboratories. The expected benefit should improve the patient management. This evolution points to a need for evolution in the training requirements and role of dermatopathologists. [less ▲]

BACKGROUND: Hypomelanosis of Ito is a rare genetic disorder characterized by whorled areas of hypomelanosis. The purpose of the present study was to revisit some aspects of Ito's hypomelanosis. METHODS: Clinical observations included ultraviolet-light-enhanced visualization (ULEV) method. Histochemistry, immunohistochemistry, and electron microscopy were performed on biopsy samples from the hypopigmented areas and the surrounding skin. RESULTS: Both the ULEV and microscopic examinations revealed the heterogeneity of the pigmentation. Hypomelanosis was characterized by a reduction in melanosomes, both in melanocytes and keratinocytes. These organelles were immature and atypical, showing a weak tyrosinase immunoreactivity. Melanosome macroautophagy was prominent in keratinocytes. Some clusters of the same cells exhibited strong immunoreactivity for the Mac 387 antibody (Ca(2+) -dependent calprotectin). Ulex europaeus agglutinin-1 (UEA-1) decorated the superficial layers of the epidermis. Such features are typically found in functionally altered keratinocytes. A number of dermal cells exhibited intense phagocytic activity linked to lysozyme immunoreactivity. CONCLUSIONS: Both the melanosome depletion and macroautophagy of immature melanosomes in keratinocytes appeared to represent prominent aspects of hypomelanosis of Ito. In sum, Ito's hypomelanosis combines structural and functional changes affecting both the melanocytes and keratinocytes in the skin. [less ▲]

INTRODUCTION: Basal and squamous cell carcinomas are the most common non-melanoma skin cancers (NMSC) in humans. Their prevalence is higher in immunocompromized patients. Results of some animal experiments have indicated that TNF acts both as a tumour promotor and an inductor of apoptosis. AREAS COVERED: Peer-reviewed articles about human skin cancers possibly related to TNF antagonists. The occurrence and growth kinetics of NMSC are possibly increased in some patients under TNF antagonist therapy. Other issues of such biological treatment suggested include the activation of other distinct skin malignancies, including malignant melanoma. Benign melanocytic tumours appear to be boosted as well. At present, most of the reported findings only represent anecdotal case reports. The influence of cumulative co-factors must not be neglected, particularly the effect of other therapies administered to the patients. The occurrence of antibodies to some TNF antagonists may decrease both the treatment efficacy and the risk of skin cancer progression. EXPERT OPINION: More research needs to be performed in order to firmly establish and understand the risk of anti-TNF biologicals in the area of human skin cancers. At present, NMSC progression appears to be boosted on areas of skin field cancerization. Benign melanocytic naevi may develop as well. [less ▲]

Under specific light illumination, particularly ultraviolet (UV) and near-UV light stimulation, the skin produces both specular light reflectance and, possibly, specific fluorescent emission. These properties offer diagnostic clues and disclose some peculiar functions of the skin. A series of superficial infections (erythrasma, some tinea capitis types, tinea/pityriasis versicolor, dermatophytoses, etc.) and pilosebaceous follicles enriched in Propionibacterium spp show fluorescence. This latter characteristic is downgraded or lost while on some anti-acne treatments. A quenching effect of fluorescence is observed following the application of sunscreens. The (pre)neoplastic areas prepared for methylaminolevulinate photodynamic therapy (MAL-PDT) show reddish fluorescence following drug metabolisation producing porphyrins by the abnormal activated cells. Of note, when using a recording sensitive CCD camera instead of casual visual observation, skin fluorescence may be superimposed on the specular reflectance of the incident light. With the current technology, these situations are not distinguished with confidence. Any harsh and scaly lesion appears brighter following yellowish specular light reflectance. Stratum corneum samplings collected on clear self-adhesive discs or cyanoacrylate skin surface strippings are conveniently examined ex vivo, taking advantage of the same optical properties. [less ▲]
