Orthopaedic manual therapy : definition, characteristics and update on the situation in Belgium Summary : Although orthopaedic manual therapy (OMT) has existed for decades, and although a recent Belgian Royal Decree, published in 2014, recognized it as a particular professional qualification in physiotherapy for the treatment of neuromusculoskeletal dysfunctions, OMT remains little known by patients, but also by healthcare professionals. Yet, this professional qualification, based on clinical reasoning, using highly specific treatments, guided by the best available scientific and clinical evidence and the specific biopsychosocial characteristics of each patient, is the subject of a growing number of scientific studies pointing out its effectiveness. This article summarizes the knowledge related to OMT (definition, history, characteristics, techniques, indications, access and reimbursement) and describes its situation in Belgium. [less ▲]

Pain physiology education is an important component in the management of patients with chronic musculoskeletal pain. The Neurophysiology of Pain Questionnaire (NPQ) was developed in English to assess pain physiology knowledge in patients. This study aimed to translate the NPQ into French (NPQ-Fr) and to investigate the main psychometric properties of the NPQ-Fr. The translation was performed using the best practice translation guidelines. One hundred and one French-speaking patients with chronic non-specific spinal pain completed the NPQ-Fr to assess its acceptability and presence of floor/ceiling effects and test its dimensionality. The construct validity was tested by comparing the patients’ NPQ-Fr scores to those of 17 physiotherapists and investigating its correlation with subscales of the Short Form-36 questionnaire. The reliability (i.e., internal consistency and testretest reliability) was also investigated. To test the test-retest reliability, 70 patients were asked to complete the NPQ-Fr twice with one week in between. Regarding the NPQ-Fr psychometric properties: 1) acceptability was good; 2) internal consistency reached a Cronbach α-coefficient of 0.44; 3) no floor and ceiling effects were observed in patients; 4) a principal factor analysis generated three major factors; 5) construct validity was good; and 6) reliability was acceptable (intraclass correlation coefficient = 0.644; standard error of measurement = 1.5). The NPQ-Fr has satisfactory basic psychometric properties in patients with chronic spinal pain. [less ▲]

Introduction Ergonomics and dynamism in classroom seem to influence schoolchildren’s behavior and concentration [1-2]. This study aimed to evaluate the influence of dynamic school furniture associated with a postural education on schoolchildren’s behavior and concentration in classroom. Methods Seventy-seven primary school children were divided into an experimental group (EG) (n=46, age=6.4, height=119.3cm, weight=23.2kg, body mass=16.5) and a control group (CG) (n=31, age=6.5, height=119.6cm, weight=23.1kg, body mass=16.1) and took part in this 2-school-year-follow-up cluster randomized controlled study including 4 phases of evaluation (E1 to E4) consisting of assessing the children’s behavior (sit or not / on task or not) from videos and their attention capacities with 2 different tests. In the first year, the EG was assessed before any intervention (E1), after having provided a triangular and dynamic cushion and 3 months of postural education (E2) and after 6 months using the cushion. In the second year, outcomes were assessed after 2 years using the cushion (E4). The CG had identical evaluation design but without any intervention. Results Compared with the CG, the children of the EG significantly improved their behavior at each step of the study. So, children’s spent proportionately more time sitting when ergonomic and dynamic seating was combined with postural education (p≤0.04) and they had more “on task” behavior (p≤0.001). Furthermore, the intervention tends to optimize the children’s attention (p<0.05). Conclusions The children’s behavior and concentration in the classroom was improved using ergonomic and dynamic furniture combined with a postural intervention. Further studies are needed to confirm those results. This study suggests that early prevention seems efficient to optimized schoolchildren’s behavior. [less ▲]

Introduction: Literature is scarce concerning the effectiveness of high dose radial extracorporeal shock wave therapy (ESWT). Therefore, the aim of this study was to investigate its effectiveness on pain, function and pressure pain threshold in patients with chronicplantar fasciitis (PF). Materials and Methods: Patients with a diagnosis of chronic (pain lasting for more than 3 months) uni- or bilateral PF with a history of failed conservative treatment were included. After a 6-week control period during which no treatment occurred, a 2-week treatment period followed by a three weeks of rest (cicatrization phase) was provided. The treatment period consisted of three radial ESWT sessions (2000 impulses of 10 Hz frequency per session with an energy flux density of 0,275mJ/mm2) separated by a one-weekinterval. Patient assessments (pain intensity, foot function and pressure pain threshold (PPT) at the site of maximum local tenderness disability) were conducted at baseline, after the 6-week control phase (pre-treatment) and at the end of the cicatrization phase (post-treatment). Results: Thirty patients (19 women (63.3%)) with chronic PF and a mean age of 51.9±11 years were included in the present study. No drop-out occurred throughout the study period. No changes were observed at the pre-treatment assessment session except for pain intensity which decreased slightly but significantly (P<0.05). At the post-treatment session, highly significant (P<0.001) and clinically meaningful changes occurred for pain intensity (-34%), foot function score (-60%) and PPT (+68%). Conclusions: The present study suggests that high dose radial ESWT is a feasible and effective way to quickly and significantly decrease pain and disability in most patients with chronic PF.
