Canine idiopathic pulmonary fibrosis (CIPF) is a progressive interstitial lung disease mainly affecting West Highland white terriers (WHWTs). Thoracic high-resolution computed tomographic (T-HRCT) findings for CIPF acquired under general anesthesia have been described previously. However, the use of general anesthesia may be contraindicated for some affected dogs. Sedation may allow improved speed and safety, but it is unknown whether sedation would yield similar results in identification and grading of CIPF lesions. The aim of this prospective, observational, method-comparison, case-control study was to compare findings from T-HRCT images acquired under sedation versus general anesthesia for WHWTs affected with CIPF (n=11) and age-matched controls (n=9), using the glossary of terms of the Fleischner Society and a scoring system. Ground-glass opacity (GGO) was identified in all affected WHWTs for both sedation and general anesthesia acquisitions, although the GGO extent varied significantly between the two acquisitions (P<0.001). Ground-glass opacity was the sole lesion observed in control dogs (n=6), but was less extensive compared with affected WHWTs. Identification and grading of a mosaic attenuation pattern differed significantly between acquisitions (P<0.001). Identification of lesions such as consolidations, nodules, parenchymal and subpleural bands, bronchial wall thickening, and bronchiectasis did not differ between acquisitions. The present study demonstrated that T-HRCT obtained under sedation may provide different information than T-HRCT obtained under general anesthesia for identification and grading of some CIPF lesions, but not all of them. These differences should be taken into consideration when general anesthesia is contraindicated and sedation is necessary for evaluating WHWTs with CIPF. [less ▲]

Canine idiopathic pulmonary fibrosis (CIPF) is a progressive parenchymal lung disease of unknown origin, mainly described in old-aged West Highland white terriers (WHWTs). It is characterized by exercise intolerance, cough and dyspnoea/tachypnea with a progressive deterioration until death from respiratory insufficiency. CIPF shares clinical features with human idiopathic pulmonary fibrosis (IPF), while tomodensitometric and histopathological findings do not appear to be exactly the same. Over the past 10 years, several studies have been performed to improve our knowledge about CIPF. However, this disease is still misunderstood and clinicians are dealing with several challenges including the absence of clinical or biological markers for estimating the presence, severity or progression of the disease and related comorbidities such as pulmonary hypertension, the absence of etiologic agent, and the absence of targeted therapy. Consequently, the aims of the present project were (1) to investigate whether high-resolution computed tomography (HRCT) of the lungs obtained under sedation can be used for the diagnosis and for the follow-up of the disease, (2) to study a new echocardiographic parameter for the diagnosis of precapillary pulmonary hypertension induced by CIPF, (3) to study the potential roles of 2 chemokines of interest, CCL2 and CXCL8, as biomarkers of fibrosis and as actors in the pathogenesis of the disease, (4) to determine breed variation of basal blood concentrations of the same chemokines, vascular endothelial growth factor (VEGF), and serotonin, and (5) to search for the presence of herpesvirus as a possible etiologic agent. Background Early recognition of left-sided congestive heart failure (CHF) in dogs with degenerative mitral valve disease (DMVD) is important because it influences medical therapy, timing of follow-up, and outcome.HypothesisPulmonary vein diameter-to-pulmonary artery diameter ratio (PV/PA) measured by echocardiography can predict CHF.AnimalsNinety-eight client-owned dogs, 37 controls, and 61 dogs with DMVD.Methods Prospective clinical cohort study. History, physical examination and Doppler-echocardiography were performed. Dogs were classified as International Small Animal Cardiac Health Council class I, II or III. Congestive heart failure was identified in a subset of 56 dogs based on radiographic findings. The PV/PA was measured in bidimensional (2D) and M-mode by 2 investigators blinded to the radiologists’ conclusions.ResultsInterobserver coefficients of variation for PV/PA acquisition and measurement were <10%. The PV/PA in control dogs was approximately 1 and increased with class of heart failure. The presence of CHF could be best predicted by measuring PV/PA in 2D echocardiography (cut-off, 1.7; area under the curve, 0.98; CI, 0.97–0.98; P < .001) with a sensitivity of 96% and a specificity of 91%.Conclusion and clinical importanceThe PV/PA is a simple and reproducible echocardiographic variable that increases with class of heart failure and may help discriminate dogs in CHF from asymptomatic dogs with DMVD. Additional studies are required to determine whether PV/PA might provide additional information in the integrated interpretation of Doppler-echocardiographic indices of left ventricular filling pressures and could be used for rapid assessment of CHF in dogs in a critical care setting.
