Paclitaxel is an antimucrotubule agent used for the treatment of metastatic breast cancer. The development of scleroderma-like skin changes is an exceptional adverse effect. We report two patients with metastatic breast cancer treated by paclitaxel who presented an enexplained unilateral lymphedema of the upper limb. Medical work-up did not demonstrate any mechanical or tumor obstruction of the lymphatic vessels. Dermatologic examination revealed cutaneous and bucutaneous indurations and hardening of certain areas of the involved arm. The clinical suspicion of scleroderma was confirmed by histology and immunohistology. Treatment was initiated with methotrexate and methylprednisolone, resulting in a clear reduction of the lymphedema and a regression of the scleroderma. These two cases suggest a pathogenic role of paclitaxel-induced scleroderma in the subsequent development of severe localized lymphedema. [less ▲]

Abstract BACKGROUND: Human sweat glands are heterogeneous in their structures and functions. Accordingly, eccrine, apocrine, and apoeccrine glands are distinguished. AIMS: Some immunohistochemical markers are expected to distinguish the sweat gland types in their secretory and excretory parts. METHODS: This study used two sets of antibodies. The first panel was composed of antibodies directed to well-defined sweat gland structures. The molecular targets included the low-molecular-weight cytokeratins CAM 5.2, the S100-B protein, the epithelial membrane antigen (EMA), the carcinoembryonic antigen (CEA), and the lectin Ulex europaeus agglutinin-1 (UEA-1). A second exploratory panel of antibodies targeted syndecan-1 (CD138), NKI-C3 (CD63), and CD68. They were used to disclose some undescribed antigen expressions in human sweat glands. RESULTS: The first set of antibodies confirmed previous findings. The immunoreactivities of the three sweat gland types were similar in the excretory ducts. By contrast, they were distinguished in the deeper coiled secretory portions of the glands. CONCLUSION: Clues supporting their distinction and probably their functional activity were obtained by immunohistochemistry using the S100-B protein, CEA and CD63 antibodies. The immunoreactivity to the S100-B protein, CEA and CD63 possibly help identifying apoeccrine sweat glands or a peculiar functional activity of eccrine sweat glands. [less ▲]

Myxofibrosarcoma (MFS) is a variant of the group of malignant fibrous histiocytomas. It is one of the most aggressive types of soft tissue neoplasms. The clinical presentation is not pathognomonic and the histological aspects are highly heterogenous, frequently delaying the diagnosis or leading to misdiagnosis. Complementary histochemical and immunohistochemical stainings are mandatory to achieve the diagnosis of MFS. A 78-year-old male patient is presented illustrating this diagnostic pitfall. Extensive surgery followed by radiotherapy is the first choice treatment. [less ▲]

Psoriasis is a chronic relapsing immunoinflammatory dermatosis that is commonly associated with systemic comorbidities. The pathogenic importance of interleukin (IL)-12 and IL-23 is beyond doubt, as well as the involvement of T helper cells (Th)1 and Th17 cells. There is upregulation of the p40 subunit shared by IL-12 and IL-23 and of the IL-23 p19 subunit, but not an increased expression of the IL-12 p35 subunit. This indicates that IL-23 appears more involved than IL-12 in the pathogenesis of psoriatic plaques. Ustekinumab is a fully human monoclonal antibody of the immunoglobulin (Ig) G1 class targeting the p40 subunit common to both IL-12 and IL-23, thus inhibiting both IL-12 and IL-23 receptor-mediated signalling. Ustekinumab is part of the recent biologic therapies active in psoriasis, autoimmune arthritides, and inflammatory bowel diseases. [less ▲]

AIM: Both interstitial granulomatous dermatitis (IGD) and palisaded neutrophilic granulomatous dermatitis (PNGD) are rare disorders typically associated with systemic autoimmune conditions. They probably represent different aspects of a disease spectrum encompassing the concept of autoimmunity-related granulomatous dermatitis (ARGD). CASE REPORT: A 61-year-old woman presented with ARGD and autoimmune hepatitis. The clinical presentation suggested PNGD, while histopathology was consistent with IGD. DISCUSSION: The association of ARGD with autoimmune hepatitis is apparently a rare event. The present case shows that the clinicopathological correlation in ARGD does not always clearly fit with the classical presentations of IGD or PNGD. [less ▲]

Overproduction of sweat, sweaty skin, and body malodors are felt unpleasant by many people in diverse social groups. This review summarizes the functional aspects of the eccrine, apocrine, and apoeccrine sweat glands as encountered in health and disease. A series of measuring methods are conveniently used to explore any trouble in sweat production. These include the gravimetric method, water evaporation quantification, skin staining procedures, dye injections, casting replicas, average electrometric assessments, and skin capacitance mapping / imaging (SCM). Qualitative assessment is provided by collection of sweat and volatile compounds followed by chromatographic analysis. At present, SCM appears to be the most valuable method for assessing the number of active glands and their individual sweat production by the eccrine and apoeccrine glands. [less ▲]

Psoriasis is a common chronic inflammatory dermatitis mediated by cytokines released by dendritic plasmacytoid cells in combination with Th1- and Th17-cells. This complex and multifactorial condition is commonly associated with some comorbidities. The complexity of a coordinated management of such intricated pathologies is obvious. It calls for the so-called medicine "centered around the patient" which tops off the evidence-based medicine. Vitamin D by its immunomodulatory effect on T helper cells exerts a therapeutic benefit on psoriasis and some of its comorbidities including the metabolic syndrome. A vitamin D oral treatment was proposed in order to reduce the cardiovascular risk, and the ensuing morbidity and mortality. [less ▲]
