Il en existe deux types cliniques qui correspondent, d’une part, à une évolution particulière d’une cicatrice et, d’autre part, à une apparition présumée spontanée. Cette lésion est caractérisée par des dépôts massifs de faisceaux de collagène. Cet aspect la distingue de la cicatrice hypertrophique qui représente, de manière primordiale, une accumulation de fibroblastes et de petits vaisseaux. [less ▲]

Breast surgery currently remains very subjective and each intervention depends on the ability and experience of the operator. To date, no objective measurement of this anatomical region can codify surgery. In this light, we wanted to compare and validate a new technique for 3D scanning (LifeViz 3D) and its clinical application. Materials and methods. - We tested the use of the 3D LifeViz system (Quantificare) to perform volumetric calculations in various settings ("in situ" in cadaveric dissection, of control prostheses and in clinical patients) and we compared this system to other techniques (CT scanning and Archimedes' principle) under the same conditions. Results. - We were able to identify the benefits (feasibility, safety, portability, low patient stress) and limitations (underestimation of the “in situ” volume, subjectivity of contouring, and patient selection) of the LifeViz 3D system, concluding that the results are comparable with other measurement techniques. Conclusion. - The prospects of this technology seem promising in numerous applications in clinical practice to limit the subjectivity of breast surgery. [less ▲]

Distal lower leg soft tissue defect is frequently a challenge to repair, particularly on the tibial crest. The coverage of this kind of lesion has some limitations because of regional minimal blood supply and paucity of local soft-tissue flaps. The perforator pedicled propeller (PPP) method tries to find a new place in lower leg reconstruction in bringing similar tissues at the recipient site and avoiding long and difficult free flap transfer or muscular sacrifice. The authors report on the use of PPP method for a tibial crest exposure after trauma and for a soft tissue defect with osteomyelitis on the tibial crest. [less ▲]

La reconstruction mammaire est actuellement partie intégrante de la prise en charge du cancer du sein. Lorsqu’elle est proposée en immédiat, plusieurs contraintes se présentent au chirurgien plasticien. La prise en charge des patientes devant bénéficier d’une mastectomie se complique par les traitements complémentaires de radiothérapie. Nous proposons, dans ce travail, une prise en charge multidisciplinaire des patientes en les adressant à des sous-groupes spécifiques. Une reconstruction immédiate sera proposée à certaines patientes compte tenu des traitements complémentaires par radiothérapie. Les choix d’orientation sont complexes et une littérature pauvre à ce sujet ne nous permet pas d’orienter de façon systématique toutes les patientes. De nouvelles études, notamment prospectives, sont nécessaires pour orienter les patientes désirant une reconstruction immédiate associée à une radiothérapie post-mastectomie. [less ▲]

Oncoplastic surgery combines large lumpectomy and defect remodeling by different plastic surgery methods. These procedures improve the cosmetic result after partial mastectomy and widens the possibilities for conservative treatment. Different techniques are used from simple glandular remodeling to more difficult techniques for breast plasties with or without simultaneous controlateral symetrisation procedure. The surgical option depends especially on the ratio between the volume of the tumor and the volume of the breast and the position of the tumor. This more effective conservative treatment facilitates also postoperative radiotherapy, reduces the sequellae and the psychological impact of breast cancer treatment. [less ▲]

Breast reconstruction is now an integral part of breast cancer treatment. There are several different procedures that make use of flaps or prostheses, or a combination of the two. Which of these procedures is indicated depends on the sequelae of the initial treatment and the patient's wishes and morphology. However, the constraints caused by the prosthetic material and the now regular use of microsurgical techniques have greatly favoured the purely autologous options. The different surgical techniques and their respective advantages and disadvantages will be described, before considering the timing of the various stages of the procedure. We will also give a summary of our experience with free flaps. [less ▲]

BACKGROUND: Pregnancy is possibly associated with altered wound healing including the development of hypertrophic or keloidal scarring. The management of these lesions may prove to be difficult, and there are no universally accepted treatment protocols. Objective To assess erythema fading of hypertrophic scars following cold applications. METHODS: Skin color was assessed using the Visi-Chroma VC100 technology following validation of the method. We compared the effects of freezing or cooling hypertrophic scars developed in 45 Caucasian pregnant women. The effects of liquid nitrogen spray cryotherapy, hydrogel cooling pads, and thermal contact cushions were compared. RESULT: This pilot study showed that the three treatment modalities substantially decreased erythema. The thermal contact cushion appeared to be the most effective biothermal means for achieving this effect. CONCLUSION: Freezing or cooling hypertrophic scars helps reducing erythema of hypertrophic scars. This effect probably results from an inhibitory action exerted on the microvasculature. Data suggest that repeat applications of mild cooling might be more effective than a few short freezing sessions. [less ▲]

Mammary reconstruction following mastectomy involves breast augmentation, which probably sets the skin under tension. This aspect of mechanobiology has rarely been explored in an objective way. The aim of the present study was to mesure the biomechanical properties of skin using a suction method. Eight sites of the thorax were assessed in 26 cases of mastectomy performed in 20 women aged 49+/-7 years. In order to distinguish the effects of breast augmentation from forces exerted by earth gravitation, measurements were performed in standing and lying down positions, both before and after breast augmentation of about 450ml. Mechanobiological stimulation of the skin influences melanocyte activity. The clinical impact on melanocytes can be perceived by dermoscopy. Our aim was to assess the orientation of Langer's lines using the combination of ultrasound shear wave propagation and dermoscopy in 70 adults of darker skin complexion. On the back, 44/70 patients showed a honeycomb melanotic pattern without any main orientation. By contrast, a streaky parallel pattern of melanotic lines oriented in the direction of Langer's lines was found in 26/70 patients. Indeed, the maximum speed of ultrasound propagation was found parallel to the main orientation of the laddering melanotic pattern. The parallel melanotic pattern probably reflects the main orientation of the epidermal rete ridges aligned in the direction of Langer's lines. This aspect could be ascribed to the deepening of these structures and/or to mechanobiology affecting melanocytes. The aspect is reminiscent of that previously described in striae distensae and atrophic scars. [less ▲]

As the incidence of breast cancer increases, breast reconstruction is more and more often proposed and its indications are viewed in terms of quality of life. In the past, delayed reconstruction was the rule. Progress in reconstructive techniques currently allows performing immediate reconstruction in selected cases, even when the operative field has to be irradiated. These new techniques generally improve the result of breast reconstruction. Moreover, expansion of screening techniques allows detection of small tumors for which a conservative excision is sufficient. Nevertheless, this kind of less aggressive surgery may require a reconstructive procedure in order to maximise the aesthetic result. [less ▲]

There are glaring discrepancies in the microanatomical descriptions of cellulite in the literature. We revisited this common skin condition in women with a microscopic examination of 39 autopsy specimens. A control group consisted of 4 women and 11 men showing no evidence of cellulite. The lumpy aspect of the dermohypodermal interface appeared to represent a gender-linked characteristic of the thighs and buttocks without being a specific sign of cellulite. Incipient cellulite identified by the mattress phenomenon was related to the presence of focally enlarged fibrosclerotic strands partitioning the subcutis. Such strands possibly serve as a physiologic buttress against fat herniation limiting the outpouching of fat lobules on pinching the skin. These structures might represent a reactive process to sustained hypodermal pressure caused by fat accumulation. Full-blown cellulite likely represents subjugation of the hypertrophic response when connective tissue is overcome by progressive fat accumulation. Histologic aspects reminiscent of stretch marks are identified within the hypodermal strands, resulting in clinical skin dimpling. [less ▲]

It now seems generally agreed however that the operation has no deleterious influence on the follow up of breast cancer patients and that it has favorable psychologic effects. Breast reconstruction can be performed either by using prostheses or analogous tissues. These two different modalities have each their own advantages and limitations. Deep knowledge of the techniques and their indications allows to select the most appropriate approach in the individual patient and, thus, to improve long term results. [less ▲]
