Background: Artificial sources of restricted light wavelength, particularly tanning beds, are progressively gaining importance in photoaging. Objective: To assess the kinetics and the long-term evolution of skin pigmentation and tensile functions in sunbed worshippers over a period of 8 years. Methods: Photoaging was explored in women who were both sunshine and sunbed worshippers. A series of 65 phototype III women aged 31-46 years completed a 100-month survey. Quarterly assessments were performed on the forearms to measure (a) the skin color individual typology angle (ITA degrees ), (b) the extent in mottled subclinical melanoderma (MSM) using the ultraviolet light-enhanced visualization method and (c) the rheological properties of skin. Results: A progressive increase in both skin extensibility and hysteresis was observed, contrasting with a decrease in biologic elasticity. These rheological changes were correlated with the ITA degrees changes, but not with the MSM extent. The kinetics of evolution of each test variable were distinct over time. Discussion: This work is the first attempt at evaluating the kinetics of changes in physical parameters during a long period of frequent exposures to tanning sunbeds and sunshine for lifestyle purposes. The alterations were quite important in the color, MSM and rheological functions of the skin. [less ▲]

The medical history of a leg ulcer is spangled with episodes contributing to its chronicity before leading to possible healing. The complexity of the involved biological factors is undisputable. In order to simplify the management of these lesions, some doctors and nurses summarize the evolution of a leg ulcer in 3 phases according to the colour of the wound bed: black, yellow and red. The latter stage is commonly considered as a favourable condition for skin healing. We criticize such a concept by showing the anatomo-functional heterogeneity of the red wound bed. [less ▲]

The Stewart-Treves Syndrome is defined as an angiosarcoma (very aggressive malignant tumor originating from endothelial cells) appearing in a specific clinical setting. This tumor develops in patients suffering from chronic lymphedema of the upper limb following mastectomy and axillary lymph node dissection for breast cancer. The diagnosis relies on medical history, clinical examination and a histological assesment (biopsy or resection). This syndrome represents a rare clinical entity. Unfortunately, the prognosis is poor. A large surgical resection is the treatment of choice if the patient is a candidate for a surgical resection with a curative intent Radiotherapy is sometimes used as a palliative local treatment. Chemotherapy is only used in more advanced cases, not curable by surgery alone. [less ▲]

Iodinated contrast agents are frequently involved in delayed polymorphic adverse skin reactions. Acute generalized exanthematous pustulosis following administration of iodinated contrast agents is a rare but severe form of such reactions. The disease is characterized by the sudden occurrence of an erosive and pustular erythroderma with fever, leukocytosis and sometimes peripheral adenopathies and liver involvement. This condition is considered as an immunologic reaction, primarily involving T lymphocytes. The overall mortality reaches about 1%. Elucidating the differential diagnosis with other acute paroxysmal drug eruptions (toxic epidermal necrolysis, Steven-Johnson syndrome and drug hypersensitivity syndrome) is of paramount importance for establishing the adequate treatment of PEAG. Diaper dermatitis affects nearly 50% of infants as well as some bed-ridden adults particularly in the elderly people. This condition is induced by irritation and maceration, and may be further exacerbated by Candida sp. colonization and infection. A key factor in the pathogenesis is the alteration in both the barrier function and the water-holding capacity of the stratum corneum. The purpose of this study was to assess the effects of an unmedicated zinc oxide-petrolatum paste and to observe for possible shielding and restoring effects on the water-holding capacity of the stratum corneum altered by diaper dermatitis. The paste appeared to load the hollow skin microrelief. It was inferred that this effect was responsible for a reticulated shielding effect. Compared with healthy skin and untreated diaper dermatitis, the application of the anhydrous paste resulted in a trend to normalize electrometric properties of skin reflecting the combination of transepidermal water loss and the water-holding capacity of the stratum corneum. Les indications thérapeutiques émergentes de l’IRM (« immune response modifier ») imiquimod 5% en crème (Aldara®) sont des carcinomes basocellulaires peu agressifs et les kératoses actiniques. La maladie de Bowen est un autre candidat potentiel pour une telle option thérapeutique innovatrice. Cet IRM peut s’avérer particulièrement précieux, sous supervision dermatologique, tant en milieu hospitalier qu’en dehors, et ce, particulièrement chez des patients à mobilité réduite et chez ceux refusant toute intervention chirurgicale, de même que dans le cadre de lésions cutanées multiples et/ou difficilement accessibles à d’autres thérapeutiques. L’imiquimod s’affirme à la fois comme un traitement non-invasif et comme une alternative à d’autres techniques utilisées en cancérologie cutanée (cryothérapie, curetage, exérèse chirurgicale, radiothérapie, 5-fluorouracile topique, photothérapie dynamique, …).
