The clinical examination and the ancillary tests at the disposal from the veterinarian may be poor diagnostics in horses suffering from subclinical inflammatory airway disease or in clinical remission of recurrent airway obstruction (RAO). The objective of this study was to determine whether the methacholine bronchoprovocation test -which is daily used for diagnosis of asymptomatic human asthma- may be used as an applicable and repeatable method for the diagnosis of an increased susceptibility to bronchospasm in horses. The test, realised on 8 horses (6 with RAO history and 2 controls) presents a good feasibility and significant repeatability. Dust exposure increased the bronchoreactivity, while it did not affect other clinical and functional parameters or inflammatory markers. In the future, the bronchoprovocation test could therefore find its place in clinicat practice for the detection of asymptomatic horses which are susceptible to develop bronchospasm in poor environmental conditions. The clinical examination and the ancillary tests used in clinical routine may be poorly characteristics in horses suffering from subclinical inflammatory airway disease or in clinical remission of recurrent airway obstruction (RAO). Methacholine bronchoprovocation test, which is used for diagnosis of human asthma, has been used in some studies in horses, but none have examined his potential diagnostic interest. The objective of this study was to determine whether this test may represent an applicable and repeatable method for the diagnosis of an increased susceptibility to bronchospasm in horses, and whether there is a relationship between this test and the markers of inflammation in the bronchoalveolar lavage. Eight horses (6 with RAO history and 2 controls) were used. They underwent bronchoprovocation tests 2 times at 24h interval. They were then tested before and after 7 days of an environmental challenge. Simultaneously, bronchoalveolar lavages were done for the measurement of several inflammatory markers. The test has a good feasibility and significant repeatability. Dust exposure increased the bronchoreactivity, while it did not significantly affect other clinical and functional parameters or inflammatory makers. In the future, the bronchoprovocation test could therefore find its place in clinical practice for the detection of asymptomatic horses which are susceptible to develop bronchospasm in poorer environmental conditions. La rhabdomyolyse récurrente induite par l’exercice est un syndrome dont les manifestations cliniques sont variables, elles peuvent être différentes d’un cheval à l’autre et d’un épisode à l’autre pour un même individu. Une connaissance approfondie des examens complémentaires à réaliser est donc nécessaire pour le diagnostic de cette affection musculaire. En outre, la rhabdomyolyse induite par l'exercice peut entraîner une intolérance à l’effort d’intensité potentiellement importante. Des conseils thérapeutiques et prophylactiques sont également donnés afin de traiter les crises de rhabdomyolyse et de prévenir les épisodes de myopathies chez des animaux sensibles ou prédisposés à présenter l’atteinte. Upper respiratory tract obstructions represent the second cause of equine poor performance, after musculo-skeletal troubles. Two types of endoscopy allow observation of these airways: endoscopy at rest –allowing mainly the diagnosis of structural abnormalities– and dynamic endoscopy –allowing detection of functional abnormalities of the region. Up to now, highspeed treadmill endoscopy has been considered the gold-standard for diagnosis of a dynamic obstruction of the upper respiratory tract. However, this technology does not reproduce conditions of field exercise nor the effect of the rider. Recently, overground dynamic endoscopy has been developed to examine the horse in field conditions –while ridden, lunged or driven– but also to compensate the problems of high-speed treadmill availability. The present study compared observations made on 11 saddle horses both with dynamic treadmill and dynamic field endoscopy. The results collected with both methods during strenuous exercise led to similar conclusions in terms of diagnosis and prognosis in 8 out of 11 cases. For the 3 others horses, the overground endoscopy revealed abnormalities that was not observable during treadmill endoscopy. In the future, overground endoscopy could become helpful for field practitioners, in cases of poor performance or for pre-purchase examinations. On the other hand, field studies could be conducted to assess the genuine prevalence of upper respiratory tract abnormalities and their implication in poor performance of the sport horse. [less ▲]
