Its origin can be multiple (i.e. subluxations, sprains, pathologies of tendons, ligaments, peripheral nerves, multiple operations). The goal of this article was intended to discuss appropriate physiotherapy in hyperlax patients. Patients and Method: The recovery process was analyzed in three cases (Marfan Syndrome, Ehlers-Danlos Syndrome and Osteogenesis Imperfecta). Hypermobility was assessed using the Brighton scale, pain using the Visual Analogue Scale and quality of life using the Medical Outcome Study Short Form-36. Bone density was evaluated by QDR X-ray absorptiometry. We emphasized that it was important to avoid stretching and to train the patient within a controlled range of motion. Submaximal eccentric exercises within a safe range of motion were incorporated to increase the active control of the joint positioning. Thus, in one patient, isokinetic rehabilitation was successfully undertaken. Each treatment had to be adapted to the individual patient and had to include specific home exercises. Conclusion: In each case, physiotherapy gave good results in relation to pain, quality of life and stability of rehabilitated joints. Safety must be assured and specific evaluations such osteodensitometry, cardiac explorations and interventions of bracing, proprioceptive and functional strengthening can be very helpful. [more ▼] Introduction: Joint hypermobility involves an increased range of motion compared to normal amplitudes for the same age, sex and ethnic group. Benign Joint Hypermobility Syndrome (BJHS) affects between 5 and 10% of the Caucasian population but it also concern rare hereditary dystrophies with abnormal collagen structure or metabolism (i.e.: Ehlers-Danlos Syndrome (EDS)). Patients with hypermobility suffer from joints problems and chronic pain is the most frequently reported symptom. Case Report: An EDS patient presented pain in the right elbow and the right wrist after a season of tennis. Her physiotherapy consisted of wrist prono-supination and flexion-extension muscle group reinforcement and proprioceptive training. To protect the wrist against excessive load, strengthening exercises of prono-supinator and flexor-extensor muscles of elbow and wrist were undertaken on an isokinetic device after an evaluation. She was also given an orthosis restricting the joint range of motion of the wrist. The patient rapidly noted a decrease in pain and an increase in the stability of her right arm even when playing tennis. Isokinetic evaluation objectified symmetric values and an improvement in maximal torque of 20 to 25% in all trained muscles of the right elbow. She was also given individualized home exercises. Conclusion: The goal of rehabilitation is to avoid hypermobility by using the muscles as a protective brake in the control of joint positioning. Thus, muscles were reinforced in eccentric mode with starting position at the maximum length of these muscles when unstreched. The exercises could be carried out safely on an isokinetic device. Indeed eccentric exercises at slow speed and limited range of joint motion avoided risk of luxation. Patients must be informed that it’s a long-term treatment and that in addition to exercises with the physiotherapist personal home exercises are essential. A diagnosis of myasthenia gravis was suggested by single-fibre EMG of the extensor digitorum communis muscle. In view of the continued absence of dysphagia after 6 months of pyridostigmine treatment, this diagnosis was considered as definitive. Discussion: We review the various characteristics of myasthenia gravis and ways of investigating dysphagia. Conclusion: It is important to be aware of and investigate all the direct and indirect signs of dysphagia and establish the exact aetiology, in order to provide the best possible treatment. Objective: To compare the outcome of patients performing an isokinetic eccentric training with that of age-, gender-, activity-matched patients receiving a non-strengthening classical rehabilitation. Methods: Ninety-two patients with unilateral chronic lateral epicondylar tendinopathy (mean duration of symptoms 8 +/- 3 months) were assigned either to a control group (n = 46) or to an eccentrically trained group (n = 46). The control group underwent a passive standardised rehabilitation programme that excluded strengthening exercises. In addition to this programme, the trained group also performed eccentric exercises based on the repetitive lengthening of the active musculo-tendinous unit. The latter exercises started with submaximal contraction intensity and slow speed movement. Modalities were progressively intensified (increase in intensity contraction and speed movement) over a long priod of treatment. Programme effectiveness was assessed through pain score evaluation, a disability questionnaire, muscle strength measurement and ultrasonographic examination. Results: Compared to the non-strengthening control group, the following observations were made in the eccentrically trained group: (1) a significantly more marked reduction of pain intensity, mainly after one month of treatment; (2) an absence of strength deficit on the involved side through bilateral comparison for the forearm supinator and wrist extensor muscles; (3) an improvement of the tendon image as demonstrated by decreasing thickness and a recovered homogenous tendon structure; and (4) a more marked improvement in disability status during occupational, spare time and sports activities. Conclusion: These results highlight the relevance of implementing isokinetic adapted eccentric training in the management of chronic lateral epicondylar tendinopathy. [more ▼] Relatively little is known about isokinetic profiles of the forearm and wrist muscles. The aims of the study were therefore to prove the reproducibility of the measurements, to establish reference concentric and eccentric strength values and to determine the effectiveness of an isokinetic eccentric training model in treating chronic epicondylitis. The concentric and eccentric assessment of healthy male and female subjects allowed the analysis of different parameters: absolute and bodyweight normalized peak torque; bilateral asymmetries and conventional pronator / supinator and flexor / extensor ratios. We have studied the coefficients of variation in a test – retest session. At the same time, 32 patients diagnosed with chronic lateral epicondylitis benefited from an adapted isokinetic eccentric training model (20 to 30 sessions). The absolute reliability in repeated tests procedure of peak torque measurement appeared satisfactory with CV inferior to 12 %. The bilateral asymmetries did not exceed 24 %. PT ratio study reflected the superiority of the PRO and FLE muscles in comparison to the opposite groups. The eccentric rehabilitative model presented a very satisfactory effect on treating chronic epicondylitis with a significant reduction of pain (Visual Analogic Scale), improvement in the ultrasonographic examination (normalized echoic aspect in 53 % of cases) and normalization of maximal isokinetic strength. The isokinetic testing of the forearm pronosupinator muscles and wrist flexor-extensor muscles appeared reproducible. Referential values were proposed for different parameters allowing easier interpretation of pathological cases. The isokinetic eccentric rehabilitative model promoted the epicondylitis treatment.
