Dans le cadre d’un suivi longitudinal des sportifs, ces mesures sont intéressantes pour mettre en place des critères objectifs et spécifiques à l’athlète de retour sur le terrain après une blessure. [more ▼] Introduction: The treatment of choice for tendinopathies is eccentric reeducation. Although the clinical results appear favourable, the biomechanical changes to the tissue are not yet clear. Even if the mechanotransduction theory is commonly accepted, the physiology of tendons is not clearly understood. We aimed to better define the biomechanical and histological changes that affect healthy tendon after eccentric and concentric training. Materiel and Methods: This study compared the effects of 2 methods of training (eccentric (E) training and concentric (C) training) with untrained (U) rats. The animals were trained over a period of 5 weeks. The tricipital, patellar and Achilles tendons were removed, measured and a tensile test until failure was performed. A histological analysis (hematoxylin and eosin and Masson's trichrome stains) was also realized. Results: There was a significant increase in the rupture force of the patellar and tricipital tendons between the U and E groups. The tricipital tendons in the control group presented a significantly smaller cross-sectional area than the E- and C-trained groups, but none was constated between E and C groups. No significant difference was observed for the mechanical stress between the three groups for all three tendons. Histological studies demonstrated the development of a greater number of blood vessels and a larger quantity of collagen in the E group. Discussion and conclusion: The mechanical properties of tendons in rats improve after specific training, especially following eccentric training. Our results partly explained how mechanical loading, especially in eccentric mode, could improve the healing of tendon. [more ▼] This work is part of a project that deals with the three-dimensional (3D) analysis of normal and pathological gaits based on a newly developed system for clinical applications, using low-cost wireless accelerometers and a signal processing algorithm. This system automatically extracts relevant gait events such as the heel strikes (HS) and the toe-offs (TO), which characterize the stance and the swing phases of walking. The performances of the low-cost accelerometer hardware and related algorithm have been compared to those obtained by a kinematic 3D analysis system and a force plate, used as gold standard methods. The HS and TO times obtained from the gait data of 7 healthy volunteers (147 trials) have been found to be (mean ± standard deviation) 0.42±7.92 ms and 3.11±10.08 ms later than those determined by the force plate, respectively. The experimental results demonstrate that the new hardware and associated algorithm constitute an effective low-cost gait analysis system, which could thus be used for the assessment of mobility in routine clinical practice. The aim of this review was to gather the actual knowledge concerning the platelet-rich plasma (PRP) treatment in tendon lesion. The interest of this well-known technique, applied since almost 15 years in facial and dental surgery, is growing in the sports world because of its potential ability to treat refractory tendinopathies and to get back quickly a functional use. However, given the relative recent knowledge about PRP in sports medical, the studies could not yet reproduce the beneficial effects obtained in-vitro. Most studies about the subject indeed show an improvement in symptoms after PRP treatment but, because of the many biases introduced, such as the lack of population, the lack of control group or the many different kind of PRP used, the studies cannot be conferred a high level of evidence. Therefore, there is no evidence of formally advise the use of PRP in the treatment of tendinopathy. So the main research field in the close future will have to determine a standard for the manufacture of PRP and the protocols of the future studies in order to be able to compare them. [more ▼] Les tendinopathies apparaissent fréquentes au sein de la population active et sportive. Actuellement, la rééducation excentrique constitue une thérapeutique de choix dans leur traitement. Stanish en proposa dès les années 80 les principales règles. Bien que les résultats apparaissent favorables en clinique, les adaptations morphologiques et biochimiques des tissus n’ont pas encore été élucidées. Nous avons démontré que l’effort excentrique (réalisé par des rats en course en descente) participait au développement macroscopique du réseau de vaisseaux sanguins au niveau de l’épitendon et à une amélioration de la résistance à la traction longitudinale tendineuse. Ceci confirme donc que les propriétés mécaniques du tendon de rat sont améliorées après un entraînement excentrique. Le tendon, plus résistant, augmenterait sa quantité de fibres de collagène et probablement les interactions entre elles. La mécanotransduction serait le processus responsable du remaniement tissulaire lors de l’application de contraintes mécaniques. Cependant, en pratique clinique, une vingtaine de séances d’entraînement minimum semblent néanmoins nécessaires pour assurer l’efficacité du traitement. Depuis quelques années, en traumatologie sportive, les propriétés réparatrices des plaquettes sanguines sont mises à profit pour stimuler la cicatrisation tendineuse. Nous avons démontré, selon a modèle animal, que celles-ci permettaient d'accélérer le processus cicatriciel tendineux mais en amélioraient également la qualité. Cependant, ce processus ne peut se dérouler correctement que si le tendon est soumis à des contraintes mécaniques afin d'orienter les neo-fibres de collagène selon les lignes de forces. Nous sommes donc convaincus qu'en pratique clinique, une prise en charge rééducative excentrique sous-maximale est indispensable dans les suites d'une injection de concentré plaquettaire pour traiter une tendinopathie chronique.
