The aim of this prospective study was to establish a protocol for fast localised abdominal sonography of horses (FLASH) admitted for colic. The FLASH protocol was then presented to clinicians without extensive ultrasound (US) experience to determine whether they could learn to use it in less than 15 min. The clinical subjects comprised 36 horses that had been referred for colic over a 2 month period. Each horse was examined at admission and FLASH findings at seven topographical locations were compared to serial clinical examinations, surgical and non-surgical outcomes, or with post-mortem reports. FLASH was able to show free abdominal fluid and abnormal intestinal loops, with a mean time of 10.7 min required to complete the protocol. The positive and negative predictive values of requirement for surgery of dilated turgid small intestinal loops using FLASH were 88.89% and 81.48%, respectively. The results suggested that FLASH is a technique that can be used in an emergency setting by veterinarians without extensive US experience to detect major intra-abdominal abnormalities in horses with colic. An 11-year-old, entire male coton de tulear was presented on emergency with acute and severe depression, acute abdominal pain and vomiting of 24 hours duration. Historical complaints included right perineal swelling, dyschezia and tenesmus of 18 months duration. Abdominal ultrasonography and radiography suggested a pneumoperitoneum and positive-contrast colonography showed leakage of contrast medium into the caudal abdomen and the presence of a large retroperitoneal pouch. Exploratory laparotomy allowed the visualisation of faecal leakage from the retroperitoneal space into the peritoneal cavity. Using a perineal approach, a large necrotised rectal diverticulum filled with faeces was found over the retroperitoneal structures. A standard herniorrhaphy was then performed. The dog recovered uneventfully and dyschezia did not recur at the nine month follow-up. Rectal diverticulum rupture associated with peritonitis has not been described in the veterinary literature, to the authors' knowledge, and should be considered as a rare differential diagnosis in dogs being presented with gaseous peritonitis. This retrospective study reports diagnostic findings in 74 horses with ultrasonographic diagnosis of femorotibial joint damage; it describes the ultrasonographic features of meniscal tears and determines the prevalence of medial or lateral meniscal involvement and of associated synovial effusions. Horses were classified into four groups: with medial meniscal damage, with lateral meniscal damage, with lesions in both menisci, and with no ultrasonographic evidence of meniscal damage. After ultrasonographic appearance, meniscal lesions were described as central degeneration, horizontal tear, partial oblique tear of the distal angle, combined horizontal and oblique tears, or complex tear. Meniscal protrusion or other associated ultrasonographic or radiographic abnormalities were recorded. Of the 74 horses, 54 (73%) had medial meniscal damage, 5 (6.75%) had lateral meniscal damage, 5 (6.75%) had lesions in both menisci, and 10 (13.5%) had no meniscal lesion. Meniscal protrusion occurred in 20 cases (27%). Horizontal tears were the most frequent type of meniscal lesion (26 horses). Complex lesions were found in 6 lateral menisci and 14 medial menisci. Lesions of the cranial meniscal ligaments were seen in 10 horses. Synovial effusion of one or several joint compartments was found in 51 cases (68.9%). This study demonstrates the high prevalence of meniscal tears and synovitis in horses with ultrasonographic evidence of femorotibial derangement. Based on this series of clinical cases, horizontal tears of the medial meniscus appear to be the most frequent soft tissue injury of the equine stifle. [less ▲]
