The application of multiple imputation (MI) techniques as a preliminary step to handle missing values in data analysis is well established. The MI method can be classified into two broad classes, the joint modeling and the fully conditional specification approaches. Their relative performance for the longitudinal ordinal data setting under the missing at random (MAR) assumption is not well documented. This paper intends to fill this gap by conducting a large simulation study on the estimation of the parameters of a longitudinal proportional odds model. Cervical cancer is a major public health concern in Morocco where it represents the second most common and lethal cancer in women. Human papillomavirus (HPV) vaccines have been licensed in Morocco since 2008 but there are no available data on their acceptability. This study aimed to assess awareness of HPV and the vaccine, and to identify factors associated with acceptability of the vaccine among parents in Morocco. We carried out a questionnaire-based survey using face-to-face interviews in a sample of 852 parents (670 mothers and 182 fathers) with at least one unmarried daughter </=26 years. We collected data within public and private health centres and clinics in four regions in Morocco between July and August 2012. The main outcome measure was parental acceptability of the HPV vaccine for their daughter(s). Responses revealed very low awareness of HPV infection (4.7%) and the HPV vaccine (14.3%). None of the participants had vaccinated their daughter(s) against HPV and vaccine acceptability was low among mothers (32%) and fathers (45%). Higher education and income, previous awareness of the HPV vaccine and endorsement of the belief that a recommendation from the Ministry of Health or a doctor to have the vaccine would be encouraging, were associated with mothers' HPV vaccine acceptability. Non-acceptability among mothers was associated with having more than two daughters, believing the vaccine was expensive, lack of information and believing that whatever happens to an individual's health is God's will. The only factor associated with the fathers' acceptability of the vaccine was the cost of the vaccine. Increasing HPV and HPV vaccine awareness through educational campaigns, along with active recommendation by physicians and a publically funded vaccination programme could increase parental acceptability of the HPV vaccine in Morocco. BACKGROUND: -The relationship between biomechanical properties and biological activities in aortic aneurysms was investigated with finite element simulations (FES) and 18F-fluoro-deoxyglucose (18F-FDG) positron emission tomography (PET). METHODS AND RESULTS: -The study included 53 patients (45 males) with aortic aneurysms, 47 infrarenal (AAA) and 6 thoracic (TAA), who had at least one 18F-FDG PET/computed tomography. Over a 30-month period, more clinical events occurred in patients with increased 18F-FDG uptake on their last examination than in those without (5/18 (28%) vs. 2/35 (6%); P=0.03). Wall stress and stress/strength index computed by FES and 18F-FDG uptake were evaluating a total of 68 examinations. 25 (38%) examinations demonstrated at least one aneurysm wall area of increased 18F-FDG uptake. The mean number of these areas per examination was 1.6 (18/11) in TAAs vs. 0.25 (14/57) in AAAs, while the mean number of increased uptake areas co-localizing with highest wall stress and stress/strength index areas was 0.55 (6/11) and 0.02 (1/57), respectively. Quantitatively, 18F-FDG PET uptake correlated positively with both wall stress and stress/strength index (P<0.05). 18F-FDG uptake was particularly high in subjects with personal history of angina pectoris and familial aneurysm. CONCLUSIONS: -Increased 18F-FDG PET uptake in aortic aneurysms is strongly related to aneurysm location, wall stress as derived by FES and patient's risk factors such as acquired and inherited susceptibilities. OBJECTIVES: Currently, the prevalence of abdominal aortic aneurysm (AAA) in patients with coronary artery disease (CAD) and the correlation between CAD severity and AAA prevalence are not clearly known. We conducted a prospective study to determine the prevalence of AAA in patients undergoing coronary angiography and to determine the risk factors and a coronary profile associated with AAA. METHODS: Over an 18-month period, abdominal aortic ultrasound was performed on 1,000 patients undergoing coronary angiography for suspected or known CAD, or prior to valve surgery. Clinical characteristics and coronary profile were collected from the patients. RESULTS: The overall number of previously repaired, already diagnosed, and new cases of AAA in the study population was 42, yielding a prevalence of 4.2%. Among the patients with newly detected AAAs, only two had an AAA diameter of >54 mm and were therefore treated surgically. In men aged >/=65 years, the prevalence reached 8.6%, while in men with three-vessel CAD it was 14.4%. Multivariate analysis showed that age >/=65 years (p = .003), male gender (p = .003), family history of AAA (p = .01), current smoking (p = .002), and three-vessel CAD (p < .001) were significantly associated with a higher prevalence of AAA. CONCLUSION: The prevalence of AAA was high in men aged >/=65 years and in those with three-vessel CAD regardless of age. While our findings do not prove the cost-effectiveness of screening for AAA in these high risk patients, they do support the usefulness of a quick ultrasound examination of the abdominal aorta during routine transthoracic echocardiography in such patients. Blackcurrants constitute an important source of potential health promoting phytochemicals (e.g. phenolic compounds, ascorbic acid) due to their antioxidant properties. Only small portions of berries are consumed fresh, most of their intake coming from processed foods, such as juices. Seven techniques were used to measure antioxidant capacity and antioxidant compounds of 10 juices. This yielded a large disparity of results. No single test, even the most popular ORAC assay, is able to compare the antioxidant capacity of similar food matrix such as blackcurrant juices. By combining tests from a large battery of « antioxidant » assays, it is possible to improve the discrimination of a food matrix by establishing a Global Antioxidant Score (GAS) which correlates well with graphical representations like Chernoff faces or stars. The latter approaches may help food industry managers and authorities to compare their « antioxidant » products with those existing on the market. [less ▲]

Abstract Background: The prevalence of chronic pathologies such as cardio-vascular diseases is increasing, in part due to dietary habits. Some dietary patterns may influence health more than individual foods, nutrients or food groups. The objective of this research was to identify dietary patterns associated with common cardiovascular risk factors (CVRF) in a sample of adult individuals living in the Greater Region. Methods: Data were taken from the cross-sectional NESCAV (Nutrition, Environment and Cardiovascular Health) study, aiming to describe the cardiovascular health of the Greater Region’s population (1). Dietary intake was assessed using a 132-item food frequency questionnaire (FFQ), from which 44 food groups were formed. Food group consumptions were adjusted for energy intake using the residuals method of Willet and Stampfer (2). Participants under a special diet to manage their CVRF were excluded. Likewise, those who had a history of cardiovascular disease or with confirmed diabetes, hypertension, and/or dyslipidemia were also discarded as they may have changed their diet. Thus, a total of 1691 individuals were included in the study (865 from Luxembourg, 527 from Wallonia and 299 from Lorraine). We applied the reduced rank regression method to identify specific dietary patterns strongly associated with common CVRF (3). The CVRF-specific dietary patterns were constructed by choosing intake data as predictors and CVRF as outcomes. Nine CVRF (BMI, waist to hip ratio, total cholesterol, triglycerides, HDL cholesterol, LDL cholesterol, blood glucose, diastolic and systolic blood pressure) were used in the statistical analysis. Associations between dietary patterns and CVRF were adjusted according to gender, age, smoking status and level of physical activity. Results: Two main dietary patterns were identified. A pattern characterized by high intakes of alcohol, potatoes and high-fat food such as pastries, fried foods, offal, processed and smoked meat, margarine, but low intakes of cereals, soups, soft drink and olive oil, was significantly associated with an increase of all CVRF. On the other hand, a healthy pattern characterized by high consumption of brown bread, nuts, soups, vegetables, smoked and canned fish, olive oil and oil rich in omega 6 and 3, high-fat dairy products, butter and margarine, but a low consumption of fried foods, rice/pasta, meat, ready meal, soft drink and beer, was associated with a decrease of all CVRF except LDL. Conclusions: Our results suggest that dietary patterns of the Greater Region population are related to CVRF. Diet rich in alcohol and high-fat food is associated with a higher cardiovascular risk whereas a healthy diet is associated with a lower cardiovascular risk. These results emphasize the need for cross-border preventive nutritional strategies. [less ▲]

Abstract Background: In epidemiological studies, the validation of dietary assessment instruments is important to avoid biased associations with outcome measures. Objective: Our objective was to assess the validity of the 134-item food frequency questionnaire (FFQ) used in the Nutrition Environment and Cardiovascular Health (NESCAV) study. Methods: The FFQ was validated against a 3-day dietary record (DR) on a sample of 29 women. The intra- class correlation coefficient (ICC) and Bland and Altman plots were used to assess absolute agreement, whereas relative agreement was appraised by Spearman’s correlation coefficient and Cohen kappa coefficient based on cross classification of 3-category nutrient intake. Results: The two methods differed significantly for the majority of micronutrients with FFQ yielding higher intakes than the DR. The bias between the two methods was nonetheless acceptable with an average overestimation by the FFQ of 11% for macronutrients and 29% for micronutrients. Regarding precision, results differed by 48% for micronutrients and 50% for macronutrients. Correlations on energy-adjusted data by the two methods were satisfactory with an average correlation of 0.47 and 16/25 coefficients above 0.40. Only vitamin A and cholesterol showed poor correlations of 0.02 and 0.05, respectively. On average, the correct classification rate in 3 categories was 50.3% and 19/25 kappa coefficients were above 0.20. Poor agreement was found for protein, cholesterol, starch, vitamins A, B12 and E with weighted kappa coefficient less than 0.20. Conclusion: Although absolute values of dietary intakes were not always accurate, the relationship and agreement between FFQ and DR may be considered as satisfactory. In particular, the FFQ was able to categorize subjects into 3 broad categories of intakes for most nutrients. Results for proteins, cholesterol, starch, vitamins A, E and B12 however ought to be interpreted with caution [less ▲]

Background: HIV/AIDS was first reported in Ecuador in 1984 and its prevalence has been increasing ever since. In 2009, the National AIDS Program reported 21,810 HIV/AIDS cases and confirmed that the worker population was amongst the most affected groups. The objective of this study was to assess knowledge about HIV transmission and prevention measures in company workers in Ecuador. Methods: A cross-sectional survey based on a random sample of 115 companies (1,732 workers), stratified by three large provinces and working sectors (commerce, manufacturing and real estate) was conducted. A validated instrument developed by Family Health International was used to evaluate HIV prevention knowledge and common local misconceptions about HIV transmission. Descriptive statistics, chi square test and logistic regression analysis were performed using SAS. Results: Incorrect knowledge about HIV/AIDS transmission were found in 49.1% (95% CI: 46.6-51.6) of subjects. Incorrect knowledge was higher among males (OR=1.73 [1.39 –2.15]), older subjects (OR=1.35 [1.02 – 1.77]), subjects with lower education (OR=3.72 [2.44 – 5.65]), manual labor workers (OR=2.93 [1.82 – 4.73]) and subjects without previous exposure to HIV intervention programs (OR=2.26 [1.79 – 2.86]). Incorrect knowledge about preventive measures was found among 32.9% (95%CI: 30.6-35.2) of respondents. This proportion was higher among subjects with lower education (OR=2.28 [1.52 – 3.43]), married subjects (OR=1.34 [1.07 – 1.68]), manual labor workers (OR=1.80 [1.34 – 2.42]), and subjects not previously exposed to HIV intervention programs (OR=1.44 [1.14 – 1.83]). Conclusions: HIV intervention programs targeting company workers are urgently needed to improve knowledge and reduce HIV transmission in Ecuador. [less ▲]

Abstract Background: Validation of Food frequency questionnaire (FFQ) is particularly important element, as incorrect information may lead to false associations between dietary factors and diseases. The aim of the study was to evaluate the validity of the FFQ used in NESCAV (Nutrition, Environment and Cardiovascular Health) study, by comparing the estimated intakes of fruits and vegetables and of several micro-nutrients with corresponding nutritional biomarkers. Methods: Relative validity was assessed in a sample of 922 subjects (452 men and 470 women). Comparisons between FFQ-estimates and their corresponding biomarkers were performed through correlation and cross classification into quintiles by using both crude and energy-adjusted FFQ-estimates. Correlations adjusted for confounders were also computed. All analyses were performed separately for men and women. Results: Concerning micro-nutrients, significant correlations were found for vitamin B9, D, E, B12 β -carotene and iodine in both men and women. Energy-adjustment led to an increase of all correlations cited previously. However, after excluding supplement users, correlations for vitamin D were not significant anymore. Concerning fruits and vegetables, all correlations were significant. Vegetables alone and fruits and vegetables correlated better in men (r around 0.2) than in women (r around 0.1). In men, correlation was also better for vegetables alone and fruits and vegetables than fruits alone. Conclusion: These data demonstrate that this FFQ is a reasonable tool to assess intakes of fruits and vegetables and of several micro-nutrients. We conclude that our FFQ is suitable to be used in NESCAV study, although protein and vitamin D estimates should be interpreted with caution [less ▲]
