Cutaneous malignant melanoma represents one of the most dramatic skin cancers because its incidence is steadily growing in White populations. Of note, its metastatic risk and mortality dramatically increase when the primary neoplasm reaches about one millimeter thick. It is believed that angiogenesis and lymphangiogenesis associated with cutaneous melanoma potentially influence the neoplastic progression of the primary tumor and its metastases. In some instances, both the intratumoral and peritumoral microvasculature are correlated to booming of the tumoral growth fraction. In addition, the vascular network serves as a migration path for the intravascular and perivascular neoplastic spread. Hence, the quantification of the microvasculature might help establishing a prognostic factor of evolution. Among the available methods, spectral analysis of immunohistochemical sections highlighting vessels helps defining the microvasculature distribution. The benefit of using spectral analysis is discussed and the modalities of application of this analytical method are scrutinized. [less ▲]

Nephrogenic systemic fibrosis (NSF) is a rare gadolinium-dependent disorder of the skin and viscera. The aim of this study was to revisit some immunopathologic clues of NSF, including the characterization of glycosaminoglycans, cell tensegrity, and cell proliferation in the dermis. Immunohistochemistry was done using antibodies directed to vimentin, CD34, Factor XIIIa, calprotectin, alpha-smooth muscle actin, Ulex europaeus agglutinin-1 (UEA-1), and MIB1/Ki67 and to glycosaminoglycans, including CD44 var3, versican, and perlecan. The vimentin+ cell density was markedly increased. The vast majority of them corresponded to CD34+ or Factor XIIIa+ dermal dendrocytes (DD) showing distinct cell tensegrity. CD34+DD were slender, elongated, and usually scattered in the dermis but focally clustered in nodular collections. By contrast, Factor XIIIa+ was plump with squat dendrites showing no evidence for being under mechanical stress. Cells in the vicinity of the microvasculature were rounded and exhibited calprotectin immunoreactivity typical for monocyte/macrophages. The microvasculature highlighted by UEA-1 and alpha-smooth muscle actin looked unremarkable. The cell proliferation highlighted by the MIB/Ki67 immunoreactivity was unusually high (>20%) in the interstitial stromal cells. Stromal cells enriched in versican were plump, abundant, and seemed interconnected each other by a dense network of dendrites. By contrast, the immunolabeling for perlecan and CD44 var 3 was unremarkable. In conclusion, the cell population involved in NSF seemed phenotypically heterogeneous, and its growth fraction was clearly boosted in the skin. The intracellular load in versican was prominent. The aspect of cell tensegrity did not suggest the influence of mechanical stress putting stromal cells under tension in the dermis. [less ▲]

Son parcours résistif et capacitif module les résistances périphériques au débit sanguin. La microvascularisation est sous l’influence de divers facteurs, mais globalement, elle semble presque immuable dans la peau saine. En revanche, certaines circonstances pathologiques engendrent des hyperplasies ou des hypoplasies vasculaires. Parfois aussi, le réseau semble garder son extension normale, mais sa structure intime est remaniée. Actinic keratosis should no more be considered as a single neoplasm calling for an individual treatment. The concept of actinic photocarcinogenesis presently turns upside down the perception of skin cancers induced by nonionizing electromagnetic radiations. Currently, the clinician should cope with the whole skin area that has been damaged by chronic exposures to the visible and the invisible spectrum of the light. The therapeutic archetypes are 5-fluorouracile 5% and imiquimod 5% carefully applied to the skin area. [less ▲]

Under specific light illumination, skin produces peculiar fluorescent aspects. Such property can be used to provide diagnostic arguments or to disclose some peculiar functions of the skin. Some superficial infections (erythrasma, tinea versicolor, dermatophytic tinea,..) reveal fluorescence that may be intense. The pilosebaceous follicles enriched in Propionibacterium spp show fluorescence. Such property is lost during some anti-acne treatments. A quenching effect is observed after application of sunscreens. The (pre)neoplastic areas prepared for dynamic phototherapy become fluorescent. Pyranine labeling of the stratum corneum exhibits fluorescence allowing the assessment of the renewal activity of the epidermis. [less ▲]

The medical history of a leg ulcer is spangled with episodes contributing to its chronicity before leading to possible healing. The complexity of the involved biological factors is undisputable. In order to simplify the management of these lesions, some doctors and nurses summarize the evolution of a leg ulcer in 3 phases according to the colour of the wound bed: black, yellow and red. The latter stage is commonly considered as a favourable condition for skin healing. We criticize such a concept by showing the anatomo-functional heterogeneity of the red wound bed. [less ▲]

The genus Bartonella is composed of a series of species and subspecies. Ten of them are responsible for human infections. The best-identified diseases are cat scratch disease (B henselae and possibly B clarridgeiae), trench fever (B quintana), bacillary angiomatosis (B quintana and B henselae), and the spectrum of verruga peruana, Carrion disease, and Oroya fever (B bacilliformis). Controversies exist about the implication of a few other microorganisms being involved in these diseases. Several other conditions have been associated with the presence of Bartonella spp, but these observations await confirmation. [less ▲]
