Xerosis represents a physiological response of the stratum corneum (SC) to environmental threats. The influence of the environmental dew point (DP) is not fully understood. This parameter is the air temperature at which the relative humidity is maximum. This study aimed to assess the relationship between the environmental DP and the water-holding capacity of the skin and lower lip vermilion. For comparison, SC property was evaluated after occlusive application of cooled and uncooled hydrogel pads. Electrometric measurements using a dermal phase meter (DPM) device were performed on the back of the hands, the cheeks and the lower lip of 40 healthy menopausal women. Assessments were performed in the outdoor conditions during winter and spring. The same measurements were recorded after hydrogel pads, at room temperature or cooled to 4 degrees C, were placed for 15 min on the test sites. The environmental DP was recorded at each evaluation time. The SC water-holding capacity was discretely influenced by the DP. In the open-air environmental conditions, a positive linear relationship was found on the cheeks between the DP and DPM values. The relationship was weaker on the lips. Conversely, a consistent increase in DPM values was recorded immediately after removal of the cooled and uncooled hydrogel pads. The observations made in the open-air testing conditions are consistent with the predicted events following the Arrhenius law. By contrast, the combination of cooling and occlusion by the hydrogel pads is responsible for the reverse effect on the SC. [less ▲]

Malignant melanoma (MM), particularly its fast-growing type, is prone to interstitial, intravascular and extravascular migratory metastases. There is no information linking their growth fraction, the configuration of the MM advancing edge, the extent in vascularity and the propensity to metastatic progression. The objective of this study was to determine the growth fraction, the size of vascularity and the contours of the progression border of 32 fast-growing MM with regard to the presence or absence of a micrometastatic spread inside the skin and overt metastases in the sentinel lymph nodes. In vivo high resolution colorimetry was performed as a clinical estimate of MM vascularity. Euclidean geometry and fractal analysis were used on immunohistochemical sections. The relative microvasculature profile area beneath MM, and the fractal dimension D of the MM frontline were assessed. The MIB/Ki-67 index was determined in MM cells. Value a* of colorimetry was similarly increased in the presence or absence of micro-metastases. No difference in growth fraction was revealed between these neoplasms. Correlations were found between vascularity and angiotropism, and between the micrometastatic process and the sentinel lymph node involvement. By contrast, no correlation was shown between vascularity and the fractal D dimension of the MM advancing edge. In sum, this study establishes a link between the extent of MM growth fraction, vascularity and the presence of dermal and nodal micrometastases in fast-growing MM. [less ▲]

BACKGROUND: Pregnancy is possibly associated with altered wound healing including the development of hypertrophic or keloidal scarring. The management of these lesions may prove to be difficult, and there are no universally accepted treatment protocols. Objective To assess erythema fading of hypertrophic scars following cold applications. METHODS: Skin color was assessed using the Visi-Chroma VC100 technology following validation of the method. We compared the effects of freezing or cooling hypertrophic scars developed in 45 Caucasian pregnant women. The effects of liquid nitrogen spray cryotherapy, hydrogel cooling pads, and thermal contact cushions were compared. RESULT: This pilot study showed that the three treatment modalities substantially decreased erythema. The thermal contact cushion appeared to be the most effective biothermal means for achieving this effect. CONCLUSION: Freezing or cooling hypertrophic scars helps reducing erythema of hypertrophic scars. This effect probably results from an inhibitory action exerted on the microvasculature. Data suggest that repeat applications of mild cooling might be more effective than a few short freezing sessions. [less ▲]

The incidence of syphilis is increasing in most parts of the world including some major European cities. The lesions developing during secondary syphilis may be difficult to diagnose clinically. Similarly, the histopathologic changes do not always fulfill the typical diagnostic criteria. The objective of this study was to assess the contribution of immunohistochemistry for the identification and localization of Treponema pallidum on the skin. We retrieved from our files 12 paraffin-embedded biopsies of skin lesions, which had posed diagnostic problems in the past. Only a serologic test had proven that the patients had syphilis. Controls consisted of lichenoid dermatoses unrelated to syphilis and borreliosis. Immunohistochemistry using an antispirochete (T. pallidum and Borrelia) antibody was performed retrospectively. In all samples from primary and secondary syphilis, T. pallidum was highlighted, but none of the control lesions unrelated to syphilis showed positivity. Interestingly enough, T. pallidum present in the lower mid-part of the epidermis often outnumbered that in the dermis. This difference was more striking in secondary syphilis compared with primary syphilis. Immunohistochemistry for T. pallidum considerably increased the sensitivity and the specificity of the histologic diagnosis. The strong epidermal homing of T. pallidum is highlighted in early syphilis. [less ▲]

BACKGROUND: Skin aging is a multifaceted process influenced by various factors affecting at variable degree at different body sites. This study focused on the aging process affecting dermal dendrocytes and the microvasculature. We also explored the effect of substance P released by capsaicin because this neuropeptide is known to affect the microvasculature environment. METHODS: The first part of the present study was undertaken in two groups of 35 men who were younger than 30 years and older than 65 years, respectively. A skin biopsy was taken from the intact skin of a thigh during orthopedic surgical intervention after road accident. The second part of the study was performed on 30 men aged 65-68 years. They applied once daily a 0.05% capsaicin gel and the vehicle in a randomized design on the volar forearms. A skin biopsy was taken at inclusion and after a 5-month treatment phase. Immunohistochemistry was performed using a panel of antibodies. Dermal cells were distinguished according to the presence of factor XIIIa, CD34 or thrombomodulin. Blood and lymphatic vessels were studied by Ulex europaeus agglutinin-1, human podoplanin, human vascular endothelial growth factor-C (H-VEGF-C) and alpha-actin immunoreactivities. RESULTS: In the study phase 1, a significant decrease in thrombomodulin-positive cells and vascularity were evidenced in the aged group. In the study phase 2, capsaicin appeared to boost the factor XIIIa-positive dendrocytes, the thrombomodulin-positive cells and the blood vessel network as well. CONCLUSIONS: Specific subsets of the dermal dendrocyte populations and the blood microvasculature appear affected by aging. Capsaicin may limit these aging effects. [less ▲]

BACKGROUND: The incidence of syphilis is on the rise in many cities worldwide. Both the clinical and histopathological changes do not always fulfil the typical diagnostic criteria. Immunohistochemistry is available to identify and localize Treponema pallidum. Two main patterns of T. pallidum distribution were described in the skin, namely heavy intraepidermal infiltration and scattered pattern inside the lichenoid cell infiltrate. OBJECTIVE: To search for a third pattern consistent with dissemination of the disease. METHODS: Immunohistochemistry using an antispirochete (T. pallidum and Borrelia) antibody was used in all cases of syphilis submitted to our laboratory. In addition, deposits of immunoglobulins and complement indicative of immune complexes were searched for. RESULTS: According to the cases, T. pallidum was disclosed in a combination of three patterns. The previously described epidermal and lichenoid patterns were confirmed, and a third pattern predominating in a perivascular location throughout the dermis was clearly identified in primary syphilis. The microorganisms were abundant in extracellular and intracellular locations, particularly inside endothelial cells. No deposits of immunoglobulins and complement were disclosed. CONCLUSION: The perivascular cuff of T. pallidum throughout the dermis might be representative of the dissemination phase of T. pallidum. [less ▲]

Skin weathering and photoaging of the balding scalp have not attracted much investigative attention so far. However, the concept of field cancerization, in particular actinic field carcinogenesis, is likely applicable to this body region. The aim of the study was to finetune the epidermal actinic changes present on hairlessness scalp. The ultraviolet light-enhanced visualization (ULEV) method was used to assess a series of 50 healthy men older than 50 years with documented baldness for over 15 years. They were scrutenized looking for altered epidermal structures corresponding to photoaging-associated changes. Attention was focused on the faint mosaic melanoderma (FMM) and atypical scaliness. Observations were made on the scalp and forehead. FMM was recognized in each case. It presented either as a single manifestation of photoaging or it was associated with an unusual pattern of discrete rimmed scaliness. In this latter case histology and morphometry disclosed keratinocyte dysplasia. A similar scaly pattern was not seen on the forehead. Subtle skin surface changes were disclosed on long-standing sun-exposed balding scalps. The scaly aspect was distinct from dandruff, seborrheic dermatitis or any other common inflammatory scalp dermatosis. The presently described changes had not previously been described during early balding. It is suggested that the presently described condition may be associated with or indicative for actinic field carcinogenesis and incipient keratinocyte dysplasia. [less ▲]

BACKGROUND/OBJECTIVE: Sensitive/reactive skin is regarded as a manifestation of sensory irritation. This susceptibility condition to various exogenous factors suggests the intervention of some neuropeptides and other neurobiological mediators. Mast cells are among the putative implicated cells. METHOD: The present immunohistochemical and morphometric study was performed on two groups of 36 gender- and age-matched subjects complaining or not from reactive skin as determined by electric current perception. In the mid upper part of the dermis, the numerical density in mast cells and the size of the microvasculature were assessed distinguishing the blood and lymphatic vessels. RESULTS: Globally, the distributions of data were large in reactive skin. This condition was characterized by a prominent increase in both the numerical density in mast cells and the overall size of the lymphatics. By contrast, no difference was found in the size of cutaneous blood vessels. More precisely, it appeared that a subgroup of people with reactive skin exhibited these changes contrasting with some other individuals whose data remained close to the normal range. CONCLUSION: Mast cells and lymphatics are probably involved in the process of sensory irritation affecting a subgroup of the population. [less ▲]

In recent years, the screening accuracy of clinical dermoscopy has increased in the early detection of evolving atypical melanocytic neoplasms. However, the most dramatic cases of human malignant melanomas (MM), i.e. the fast-growing neoplasms, usually escape the classical clinical criteria of MM. As a result, a number of puzzling cases exhibit uncertain clues for MM. Thus, the risk of microscopic misdiagnosis is likely on the rise if the histological criteria are not fine-tuned. This review summarizes a conceptual classification of atypical melanocytic neoplasms grouped under the heading of melanocytomas. Some immunohistochemical markers are tentatively used as discriminators between fast-growing MM and melanocytomas. However, some differences seem to be more statistically significant than clinically useful due to extensive overlap in immunoreactivity from any case to case. A multipronged immunohistological screening is therefore welcome. [less ▲]
