Androgenetic alopecia (AGA) is a common yet poorly understood condition. In particular, the significance of inflammatory cells close to the infrainfundibulum of transitional hairs remains obscure. This study was conducted in 20 men who used a lotion containing the antimicrobials, piroctone olamine and triclosan, regularly for 18 months. At entry, mild pruritus and abnormal hair loss were reported by the volunteers. During treatments, pruritus resolved rapidly. Comparison of clinical photographs and trichograms taken at 3-month intervals suggested signs of hair regrowth with moderate increase in density of transitional hairs. Biopsies taken at 6-month intervals were processed for immunohistochemistry. There was a decrease in the density of activated T cells in the region of the follicular infrainfundibulum and isthmus over time. UgG deposits found within the epithelial sheaths at entry to the study were absent in the subsequent biopsies. The volume of the sebaceous glands, as assessed by computerized morphometry, was not modified by the treatment. It was concluded that inflammation associated with AGA transitional hairs may be driven by the presence of microorganisms harboured in the infrainfundibulum. They may modulate the severity of AGA and may potentially be controllable. This offers a new possibility of prevention and partial reversal of hair loss in AGA. [less ▲]

En tous cas, elle permet d'aligner des chiffres dont les variations peuvent être étudiées dans le temps. Le présent travail a permis de tester, de "calibrer" la méthodologie colorimétrique dans le domaine de l'érythème cutané. D'une part, l'effet vasodilatateur (érythémateux) de l'histamine, de l'autre, le blanchiment induit par l'application de topiques corticoïdes et phytopharmaceutiques ont été mesurés et discutés. Immunohistochemical and electron microscopic studies support a Schwann cell origin for most of the granular-cell tumors of the skin. We studied 9 granular-cell tumors by immunohistochemistry, using antibodies to CD68, S-100 proteins, factor XIIIa, L1 antigen, desmin and vimentin. The CD68 monocyte-macrophage-associated lysosomal antigen was expressed in granular cells which however had no Mac 387 reactivity. The 'satellite fibroblasts' were factor XIIIa-positive similarly to dendrocytes and perineurial cells. Our study reveals the lack of specificity of CD68 antibody to identify macrophages. It also presents granular cell tumor of the skin as an exception in the group of schwannomas by the presence of factor XIIIa-positive cells inside the neoplasm. [less ▲]

Pleomorphic T cell lymphoma of the medium to large cell type expressing CD30 antigen is a most aggressive peripheral T cell lymphoma. Currently, there is no satisfactory treatment available for this neoplasm. We report the case of a woman with prominent skin and bone marrow involvement without detectable lymph node localization. This unusual presentation of disease responded well to polychemotherapy combined with autologous bone marrow transplantation. [less ▲]

Vulva is an uncommon localization for basal cell carcinomas. We report three cases and review the literature. There is often a long delay before patients seek medical attention for these lesions. Moreover the clinical aspect is often indistinctive and misleading. The gross polymorphism of vulvar neoplasms therefore calls for biopsy of any suspicious areas. Surgical excision or Moh's chemotherapy are the treatments of choice for vulvar basal cell carcinomas. The recurrence rate remains quite high even when the lesion looks completely resected. [less ▲]

Computerized image analysis was used to compare lentigines (brown freckle-like cutaneous spots) induced by treatment with psoralens and ultraviolet light A (PUVA-induced lentigines) with those induced by solar exposure (actinic lentigines). For these conditions, which appear to be distinctive clinically and histologically, the number, size and shape of the macules were analyzed, revealing significant differences in the two latter parameters. This indicates that UV irradiations of different wave lengths act differently on melanocytes and/or on the interrelation between melanocytes and keratinocytes. The overall effect is much more self-limited in PUVA lentigines than in actinic lentigines, suggesting that the pathophysiology of these lesions is probably different. [less ▲]

We studied the fate of dermal dendrocytes in patients treated with psoralens and ultraviolet light by combining immunohistochemistry and computerized image analysis. Factor-XIIIa-positive dermal dendrocytes were found to be altered in these patients. When compared with controls, dermal dendrocytes were often increased in number and had an uneven size and tissue distribution. Their cytoplasm was occasionally fragmented. These changes were more pronounced when early photosclerosis was present. The alterations described probably reflect vascular changes, and may be responsible for immunomodulating actions and disorder of the connective tissue structure induced by ultraviolet light.
