The genus Bartonella is composed of a series of species and subspecies. Ten of them are responsible for human infections. The best-identified diseases are cat scratch disease (B henselae and possibly B clarridgeiae), trench fever (B quintana), bacillary angiomatosis (B quintana and B henselae), and the spectrum of verruga peruana, Carrion disease, and Oroya fever (B bacilliformis). Controversies exist about the implication of a few other microorganisms being involved in these diseases. Several other conditions have been associated with the presence of Bartonella spp, but these observations await confirmation. [less ▲]

Primary cutaneous lymphomas are distinguished by singular clinical, histological and prognostic features that are not shared by other lymphomas originating from the lymphoid tissues. This review is an update of the current concepts about the biology, diagnosis and treatment of mycosis fungoides, the most frequent entity among the primary cutaneous T cell lymphomas. It is currently established that mycosis fungoides originates from T lymphocytes normally homing the dermis. Such a situation implies that treatments at least in the initial stages of the disease, should primarily target the skin without searching for an impact on internal lymphoid tissues. [less ▲]

Hair loss, also called hair effluvium is often considered as an ancillary complaint. However, this situation is quite common in both genders. It is part of numerous clinical presentations in internal medicine and dermatology. Obviously, any correlation between a biologic abnormality and hair loss does not prove a relationship of causality. In absence of pathogenic diagnosis and causality criteria, chances are low to control adequately hair effluvium by a treatment given by the whims of fate. In addition, the risk and frequency of therapeutic inertia are increased. When the hair loss is not controlled and/or compensated by growth of new hairs, several types of alopecia inexorably develop. In a manichean ideal, one expects unreservedly a clear-cut distinction between benign melanocytic neoplasms and the malignant ones. Such a distinction is commonly established by a clinico-pathologic confrontation for the majority of neoplasms. However, the boundaries may be blurred and uncertain for some lesions. We present an exploratory clinicopathologic algorithm of the border area between seemingly benign and seemingly malignant melanocytic neoplasms. The concept of a group of neoplasms called skin melanocytomas shed some light on such quandaries where the risk of misdiagnosis is quite high. [less ▲]

This review focused on epidermal stem cells (ESCs). These cells are distinguished from transitory amplifier cells and post-mitotic cells. ESC are found in the interadnexal epidermis and in the bulge region of hair follicles. A series of ESCs markers are available, but increased sensitivity and specificity require further development. IMPORTANCE OF THE FIELD: Alopecia, psoriatic arthritis, the metabolic syndrome, inflammatory bowel diseases and cardiovascular diseases may occur as skin psoriatic comorbidities. TNF-alpha antagonists are used to treat psoriasis. Adalimumab is one of the recognized active agents for this indication. AREAS COVERED IN THIS REVIEW: The current peer-reviewed publications and presentation of original findings. WHAT THE READER WILL GAIN: Adalimumab is active on recalcitrant psoriasis and some of its comorbidities, particularly arthropathies and Crohn's disease. However, the progression of the radiological alterations is limited with regression of the bony erosions. Psoriatic enthesopathy also regresses. Mortality associated with psoriasis arthropathy is on the decline. Crohn's disease, the most frequent inflammatory bowel comorbidity of psoriasis, is responsive to adalimumab. The effect of adalimumab on the metabolic syndrome and cardiovascular involvement is more erratic. The spectacular effects of adalimumab may be associated with some adverse effects. In particular, despite a marked reduction in the psoriasis area-and-severity index (PASI) score some new acute lesions of cutaneous psoriasis may develop corresponding to paradoxical psoriasis. Other potential adverse effects include infections, granulomas, rapid growth of cancers and occurrence of lymphomas. TAKE HOME MESSAGE: Adalimumab frequently controls moderate-to-severe forms of cutaneous psoriasis and some of its comorbidities. [less ▲]

Summary Background Lactobacillus johnsonii (La1) has been reported to protect skin immune system homeostasis following ultraviolet (UV) exposure. Objectives To assess the effects of a dietary supplement (DS) combining La1 and nutritional doses of carotenoids on early UV-induced skin damage. Methods Three clinical trials (CT1, CT2, CT3) were performed using different UV sources: nonextreme UV with a high UVA irradiance (UV-DL, CT1), extreme simulated solar radiation (UV-SSR, CT2) and natural sunlight (CT3). All three clinical trials were carried out in healthy women over 18 years of age with skin type II-IV. In CT1, early markers of UV-induced skin damage were assessed using histology and immunohistochemistry. In CT2, the minimal erythemal dose (MED) was determined by clinical evaluation and by chromametry. Chromametry was also used to evaluate skin colour. Dermatologists' and subjects' assessments were compiled in CT3. Results A 10-week DS intake prevented the UV-DL-induced decrease in Langerhans cell density and the increase in factor XIIIa+ type I dermal dendrocytes while it reduced dermal inflammatory cells. Clinical and instrumental MED rose by 20% and 19%, respectively, and skin colour was intensified, as shown by the increase in the DeltaE* parameter. The efficacy of DS was confirmed by dermatologists and subjects under real conditions of use. Conclusions Nutritional supplementation combining a specific probiotic (La1) and nutritional doses of carotenoids reduced early UV-induced skin damage caused by simulated or natural sun exposure in a large panel of subjects (n = 139). This latter result might suggest that DS intake could have a beneficial influence on the long-term effects of UV exposure and more specifically on skin photoageing. [less ▲]

Superficial dermatomycoses are frequent conditions in humans and animals. Specific treatment modalities have been designed using a variety of different antifungal compounds. The need for antifungal susceptibility testing (AST) has been growing steadily over the last two decades due to the extending number of newer antifungal agents. Objective inter- and intraindividual comparisons of their respective efficacies are nearly impossible to perform in vivo. Currently, a series of standardized AST methods and interpretative guidelines have been designed. However, their clinical relevance for dermatomycoses is not consistent. The corneofungimetry bioassay was designed to test comparatively a series of antifungals on pathogenic fungi growing on sheets of human stratum corneum. Computerized morphometric assessments bring numerical values allowing statistical comparisons. Variants of corneofungimetry address more specific aspects related to fungal cell adhesion, fungitoxicity and lipid-dependent fungi. [less ▲]

A pruritic vesicular, pustular or squamous eruption confined to the lateral aspect of fingers or toes corresponds to the overall pattern of dysidrosis. Such a clinical pattern is defined by its peculiar topography. It is encountered in diseases resulting from various etiologies. Thus, conceptually, dysidrosis is a common clinical aspect shared by diverse disorders related to different etiologies and pathomechanisms. The dermatopathological assessment and the patch test procedure shed some light on this nosological diversity. Allergic contact dermatitis, irritation dermatitis, mycids, alterations of acrosyringia, and drug or food reactions are eventually involved in this process. However, most cases of dysidrotic dermatoses do not disclose their origins and they remain idiopathic. [less ▲]

Nephrogenic systemic fibrosis (NSF) is a rare gadolinium-dependent disorder of the skin and viscera. The aim of this study was to revisit some immunopathologic clues of NSF, including the characterization of glycosaminoglycans, cell tensegrity, and cell proliferation in the dermis. Immunohistochemistry was done using antibodies directed to vimentin, CD34, Factor XIIIa, calprotectin, alpha-smooth muscle actin, Ulex europaeus agglutinin-1 (UEA-1), and MIB1/Ki67 and to glycosaminoglycans, including CD44 var3, versican, and perlecan. The vimentin+ cell density was markedly increased. The vast majority of them corresponded to CD34+ or Factor XIIIa+ dermal dendrocytes (DD) showing distinct cell tensegrity. CD34+DD were slender, elongated, and usually scattered in the dermis but focally clustered in nodular collections. By contrast, Factor XIIIa+ was plump with squat dendrites showing no evidence for being under mechanical stress. Cells in the vicinity of the microvasculature were rounded and exhibited calprotectin immunoreactivity typical for monocyte/macrophages. The microvasculature highlighted by UEA-1 and alpha-smooth muscle actin looked unremarkable. The cell proliferation highlighted by the MIB/Ki67 immunoreactivity was unusually high (>20%) in the interstitial stromal cells. Stromal cells enriched in versican were plump, abundant, and seemed interconnected each other by a dense network of dendrites. By contrast, the immunolabeling for perlecan and CD44 var 3 was unremarkable. In conclusion, the cell population involved in NSF seemed phenotypically heterogeneous, and its growth fraction was clearly boosted in the skin. The intracellular load in versican was prominent. The aspect of cell tensegrity did not suggest the influence of mechanical stress putting stromal cells under tension in the dermis. [less ▲]

BACKGROUND: Drug-induced toxic epidermal necrolysis (TEN) probably results from a complex and specific immune cell reaction involving lymphocytes and macrophages. OBJECTIVE: To assess the functional role of macrophages in TEN. METHODS: Immunohistochemistry was performed on biopsies from early blisters developed in 9 TEN patients. The amount of extracellular myeloperoxidase (MPO) was measured by ELISA in TEN blister fluid and serum. Controls were blister fluids taken from 9 second-degree burns. In addition, 3-chlorotyrosine (a specific marker of MPO activity) was searched for using liquid mass chromatography both in TEN and burn blister fluids. RESULTS: Immunohistochemistry revealed numerous CD68+ macrophages in 8/9 TEN patients; 5-20% of these cells and rare CD15+ neutrophils exhibited MPO immunoreactivity, while keratinocytes were negative. The amount of MPO was significantly higher in TEN blister fluid than in TEN serum, suggesting macrophage production of MPO in the skin. In addition, MPO was significantly more abundant in TEN blister fluid than in burn blister fluid. 3-Chlorotyrosine was detected in 7/9 TEN blister fluids, but in only 2/9 burn blister fluids. DISCUSSION: MPO produced by macrophages was functionally active in most TEN patients, leading to the production of hypochlorous acid, a potent oxidative compound that alters keratinocytes. [less ▲]

Skin capacitance is an electrical property measurable at the level of the stratum corneum. Values are related to the moisture content of the tissue. Using the silicon image sensor technology, it has recently become possible to measure capacitance at 50 microm intervals on the skin surface. The resulting nonoptical picture corresponds to skin capacitance imaging. This novel tool for dermatologists highlights with precision the hollow skin surface patterns, including dermatoglyphics, the patterns of the shallow skin lines, and wrinkles. The topographic heterogeneity in skin surface hydration is made visible. In addition, the sweat gland and follicular openings are disclosed. Skin disorders induced by surfactants or presenting as hyperkeratotic areas, including some neoplasms, may show typical aspects. It is concluded that skin capacitance imaging brings added value to the clinical assessment performed by dermatologists. [less ▲]
