Delayed onset muscle soreness (DOMS) follows unaccustomed muscular exercise, most notably in the eccentric mode. That concept refers to a dull ache combined with tenderness, stiffness and weakness of the previously active muscles. lsokinetic device constitutes a specific model in generating and investigating DOMS. Respective effects of concentric and eccentric actions have been compared, emphasizing on the variability in the response (serum activity of CK for instance). The particular sensitivity of the hamstrings was underlined although causes remained unexplained. Some treatment have been proposed in the management of DOMS. Several studies reported that anti-inflammatory agents fail to alleviate pain and discomfort even if other authors indicated a relative effectiveness. Based on the repeated-bout effect, submaximal eccentric exercise currently represent the most useful preventive strategy. [less ▲]

Objectifs. Évaluer la fonction musculaire d’un groupe de patientes fibromyalgiques et la comparer à celle d’une population féminine de référence. Méthodes. Seize patientes fibromyalgiques et 87 sujets sains féminins sédentaires ou sportifs de loisir ont exécuté un protocole expérimental composé de quatre épreuves. La première évaluait le moment de force isocinétique concentrique maximum des muscles extenseurs et fléchisseurs du genou dominant. Un dynamomètre de Colin autorisait une mesure de la force manuelle isométrique. La résistance musculaire à la fatigue s’appréciait lors d’une épreuve isocinétique de flexion–extension des genoux : le sujet enchaînait 30 contractions concentriques d’intensité maximale à la vitesse angulaire de 180 °/s. La dernière épreuve évaluait l’endurance musculaire statique à l’aide de tests exigeant le maintien d’une position donnée. Résultats. Toutes les variables musculaires apparaissaient réduites chez les sujets fibromyalgiques. Ce déficit, déjà manifeste pour les efforts anaérobiques, s’accentuait lors des efforts en aérobie. Le déficit moyen correspondait à 39 % (p < 0,001) pour la force musculaire, 40 % (p < 0,0001) pour la résistance musculaire à la fatigue et 81 % (p < 0,0001) pour l’endurance musculaire statique. Les perceptions nociceptives au cours de l’effort, évaluées à l’aide d’une échelle visuelle analogique, étaient supérieures chez les patientes fibromyalgiques. Conclusion. L’exploration combinée des trois filières énergétiques confirme l’altération générale de la fonction musculaire du sujet fibromyalgique. Cette étude suggère cependant une perturbation préférentielle des composantes oxydatives. [less ▲]

Objectives. To compare muscle performance in women with fibromyalgia and in healthy women. Patients and methods. Sixteen women with fibromyalgia syndrome (FMS) and 85 healthy women who were physically inactive or engaged in recreational sports underwent measurements of four parameters: maximal concentric isokinetic muscle strength of the knee extensors and flexors in the dominant limb, isometric grip strength on a Colin dynamometer, muscle fatigue resistance during 30 maximal concentric isokinetic contractions of the dominant knee flexors and extensors at 180° angular velocity, and static endurance during posture maintenance. Results. All muscle variables were decreased in the FMS patients as compared to the controls.The decreases were more marked during aerobic than during anaerobic exercise.Mean decreases were 39% (P<0.001) for muscle strength, 40% (P<0.0001) for fatigue resistance, and 81% (P<0.0001) for static endurance. Pain during exercise as evaluated using a visual analog scale was more marked in the FMS patients. Conclusion. This study of the three pathways supplying energy to muscle confirms that muscle function is globally impaired in FMS patients.The results suggest that the impairment predominates on aerobic processes. OBJECTIVE: To realize a clarification about fibromyalgia, attempting to consider diagnostic criteria, prevalence, pathophysiology and therapeutic approach. METHOD: A systematic literature search was conducted to select articles about fibromyalgia and connected diseases. The database are Premedline, Medline and Medlineplus. RESULTS: Fifty-eight articles about fibromyalgia and twelve articles about connected diseases were selected to realize this review of literature. DISCUSSION: Fibromyalgia constitutes a syndrome characterized by widespread musculo-skeletal pain, present above the waist and below the waist and in the axial skeleton. Widespread pain must have been present for at least three months. "Spasmophilie", chronic fatigue syndrome and myofascial syndrome represent diseases connected with fibromyalgia: differential diagnosis must be established. Researches related to fibromyalgia suggest a reduction of muscular performances associated with histological and biochemical anomalies. Patients are characterized by shorter and nonrestorative sleep. Psychological, neuroendocrine and central alterations appear often associated with fibromyalgia. The reduction of pressure tolerance and pain thresholds may be linked to the alterations of neuroendocrine substances. Literature recommend a multidisciplinary therapeutic approach in management of fibromyalgia. CONCLUSION: The pathophysiologic mechanisms in fibromyalgia appear multiple and interdependent. With the aim to optimizing treatment, investigations are necessary to determine biochemical repercussions of various therapeutic approaches. [less ▲]

The objectives of the study were to measure the isokinetic strength of the hip flexor-extensor and adductor-abductor muscle groups in unilateral lower limb amputees and to evaluate abnormalities in order to improve the usual rehabilitation programs. Thirty-three patients with unilateral lower limb amputation were included in this study; they served as their own controls, the amputated side being compared to the intact side. Using an isokinetic device, the relative peak torques (N.m by body weight), bilateral asymmetries and agonist/antagonist torque ratios were calculated for the hip flexor, extensor, adductor and abductor muscle groups. A significant reduction of mean concentric relative peak torque measured on the amputated limbs was generally observed when compared to the intact contralateral limb (p < 0.05). The mean percentage of relative peak torque decrease was greater for the extensors than for the flexors (33 +/- 20% and 22 +/- 23%, respectively, at 30degrees/s angular velocity), and for the abductors than for the adductors (23 +/- 22% and 11 +/- 19%, respectively, at 30degrees/s angular velocity). Furthermore, the flexor/extensor torque ratio was significantly increased for the amputated limb (for instance, 1.25 +/- 0.33 versus 1.09 +/- 0.25 for the intact limb at 30degrees/s). The reduction of the abductor/adductor ratio of the operated limb reached the level of statistical significance at 60degrees/s only (1.25 +/- 0.46 versus 1.07 +/- 0.36 for the intact limb). In conclusion, isokinetic testing revealed a significant reduction of the hip stabilizing muscle strength as a consequence of lower limb amputation. The more marked decrease of the strength of the abductor and extensor muscle groups, resulting in imbalances of the agonist/antagonist strength ratios should be taken into account to adapt the conventional rehabilitation programs. The etiology of FM remains elusive, but the diagnosis is well established. The criteria for the classification are widespread pain combined with tenderness at 11 or more of the 18 specific tender points sites. The prevalence is 2% in the general population.
