Thirty-four subjects with questionable Alzheimer's disease (QAD) were included in a 3-year prospective study and underwent neuropsychological testing and measurement of brain metabolism using FDG-PET at entry. Seventeen patients (50%) did not convert to AD during the follow-up period. Compared to elderly controls of similar age, the cerebral activity of non-converters was reduced in the dorsolateral prefrontal cortex. Moreover, the variability of metabolism in the posterior cingulate cortex was related to their visuospatial memory performance, while disparity in parietal activity was related to their verbal memory performance. These results demonstrate the cerebral metabolic heterogeneity of patients with QAD. Initial functional images of converters showed that activity was already impaired in the posterior cingulate, lateral temporal cortex, anterior cingulate and orbitofrontal cortex. This metabolic pattern is consistent with a pre-dementia stage of AD, and highlights the fact that significant frontal metabolic involvement may be associated with impaired activity in posterior associative cortices in very early AD. Les processus contrôlés et automatiques de récupération mnésique ont été évalués au moyen de la Procédure de Dissociation des Processus appliquée à une tâche de complètement de trigrammes chez 59 patients diagnostiqués comme « questionable Alzheimer’s disease » (QAD ou Mild Cognitive Impairment). Par ailleurs, le métabolisme cérébral du glucose des patients a été mesuré par FDG-PET. Comparativement à des volontaires âgés sains appariés, le profil mnésique des patients QAD était caractérisé par un déficit des processus contrôlés, mais une préservation des processus automatiques. Après un suivi de 30 mois, 27 des patients ont développé une maladie d’Alzheimer, tandis que 23 patients restèrent des QAD stables (9 sujets n’ont pas complété le suivi ou ont reçu un autre diagnostic au terme de celui-ci). Les deux sous-groupes présentaient le même degré de déclin des processus de mémoire contrôlés. Des corrélations cognitivo-métaboliques, ainsi qu’une analyse en composantes principales, ont permis de montrer que les corrélats métaboliques des processus contrôlés (à l’entrée dans l’étude) n’étaient les mêmes chez les patients qui allaient développer la maladie d’Alzheimer et chez les patients qui allaient rester stables. Chez les patients qui développaient ultérieurement une maladie d’Alzheimer, l’utilisation correcte des processus contrôlés était positivement corrélée à l’activité du cortex préfrontal dorsomédian, qui pourrait jouer un rôle dans les processus réflexifs de monitoring agissant sur les produits de la récupération. L’activité du cortex préfrontal dorsomédian était corrélée à l’activité métabolique des régions frontales bilatérales et du cortex cingulaire postérieur. Par contraste, chez les patients QAD stables, nous avons trouvé une corrélation avec la formation hippocampique antérieure, une région qui intervient dans la réactivation de l’épisode d’encodage des événements. Fourteen patients with frontal lobe lesions and 14 normal subjects were tested on a recognition memory task that required discriminating between target words, new words that are synonyms of the targets and unrelated distractors. A deficit was found in 12 of the patients. Moreover, three different patterns of recognition impairment were identified: (I) poor memory for targets, (II) normal hits but increased false recognitions for both types of distractors, (III) normal hit rates, but increased false recognitions for synonyms only. Differences in terms of location of the damage and behavioural characteristics between these subgroups were examined. An encoding deficit was proposed to explain the performance of patients in subgroup I. The behavioral patterns of the patients in subgroups II and III could be interpreted as deficient postretrieval verification processes and an inability to recollect item-specific information, respectively. This study explored recognition memory performance for novel versus familiar words in Alzheimer's disease (AD) patients and normal controls (NCs), using an adaptation of E. Tulving and N. Kroll's (1995) procedure. Results showed that both groups exhibited more hits and more false alarms for familiar than for novel words. The groups did not differ in the recognition of familiar words, reflecting preserved familiarity processes in AD. However, AD patients made more false alarms than NCs in the recognition of novel words, reflecting impairment of recollection processes in AD. A positron emission tomography analysis of clinico-metabolic correlations in AD patients showed a correlation between recognition of novel words and right hippocampal activity, whereas recognition of familiar words was more related to metabolic activity in the left posterior orbitofrontal cortex. The goal of this study was to explore in patients with Alzheimer's disease (AD) the brain correlates of free and cued recall performance using an adaptation of the procedure developed by Grober and Buschke (1987). This procedure, which ensures semantic processing and coordinates encoding and retrieval, has been shown to be very sensitive to an early diagnosis of AD. Statistical parametric mapping (SPM 99) was used to establish clinicometabolic correlations between performance at free and cued verbal recall and resting brain metabolism in 31 patients with AD. Results showed that patient's score on free recall correlated with metabolic activity in right frontal regions (BA 10 and BA 45), suggesting that performance reflected a strategic retrieval attempt. Poor retrieval performance was tentatively attributed to a loss of functional correlation between frontal and medial temporal regions in patients with AD compared with elderly controls. Performance on cued recall was correlated to residual metabolic activity in bilateral parahippocampal regions (BA 36), suggesting that performance reflected retrieval of semantic associations, without recollection in AD. In conclusion, this study demonstrates that the diagnostic sensitivity for Alzheimer's disease of the cued recall performance in the Grober and Buschke procedure (1987) depends on the activity of parahippocampal regions, one of the earliest targets of the disease. Moreover, the results suggest that the poor performance of patients with AD during free and cued recall is related to a decreased connectivity between parahippocampal regions and frontal areas. This study examined the performance of normal controls (NC) and Alzheimer's disease (AD) patients on free recall, semantic cued recall and object cued recall of both subject-performed tasks (SPTs) and verbal descriptions of actions, by controlling familiarity of actions associated to objects. The results showed that both groups performed better after SPT encoding than after verbal encoding. in all three types of recall. In addition, this SPT advantage was greater for AD patients than for NC in the object cued recall test, emphasizing AD patients' sensibility to the congruence of cues between encoding and retrieval conditions. Following verbal encoding. NC showed a better recall for less familiar actions than for highly familiar actions, whereas AD patients exhibited the opposite pattern. These results reflect that AD patients did not benefit from a distinctiveness effect at encoding for improving subsequent retrieval of verbal information, probably due to a reduced level of elaboration during encoding. However, there was no effect of action familiarity on recall performance by both groups following SPT encoding. These results suggest that memory for verbal actions and SPTs is governed by different principles. In addition. they demonstrate the robustness of the SPT effect in AD patients, who were able to improve memory performance in the SPT condition not only with highly familiar actions but also with less familiar actions. The mobile phone may be useful to keep in contact with spatially disoriented and memory impaired patients. In keeping with this idea, this study describes the training program developed to teach two patients with mild Alzheimer's disease (CI and ML) how to use their own mobile phone. Each training session was divided into two parts. In the first part, the spaced-retrieval technique was used to promote the consultation of a card pasted on the back of the phone. The card detailed each stage of phone utilization and which keys had to be pressed to call somebody. In the second part, the patients received repetitive exercises of calling based upon the errorless learning principle. At the end of three-months rehabilitation, the results showed different learning patterns for the patients. ML needed more spaced-retrieval sessions to spontaneously consult the card and to correctly use the phone, compared to CI However, by the repetition of calling exercises, both patients showed a decrease of instruction card consultation, whereas they were still able to correctly call somebody. This learning ability is hypothesized to be a consequence of a relatively preserved procedural memory in both patients. In conclusion, this study highlights the effectiveness of combined specific learning techniques for improving AD patient's autonomy in daily life activities. [less ▲]

Using Positron Emission Tomography (PET), we investigated cerebral regions associated with the episodic recognition of words alone and words bound to contextual colours. Two modes of colour encoding were tested: incidental and intentional word-to-colour binding. Word-only recognition was associated with brain activation in a lexico-semantic left middle temporal region and in the cerebellum following an incidental colour encoding, and with brain activation in the left posterior middle frontal gyrus, right anterior cingulate and right inferior frontal gyrus following an intentional encoding. Recognition of bound features was associated with activation in left prefrontal and superior parietal regions following an incidental colour encoding, and with preferential right prefrontal cortex activation following an intentional colour encoding. Our results are in line with the hypothesis of a parietal involvement in context processing, and prefrontal areas in monitoring retrieval processes. Our results also support the hypothesis of a 'cortical asymmetry for reflective activity' (CARA). [less ▲]

Alzheimer's disease represents an important problem for our societies, both in social and economical dimensions. Built in July 1997, the day care center of Liege aims at practicing strategic cognitive interventions in early Alzheimer patients or when pathological memory impairments occur. By means of an interdisciplinary work (by neuropsychologists and occupational therapists), the ultimate goal of the interventions is to facilitate the patient's autonomy in activities of daily living. Because of the variability of clinical symptoms of the disease, the goal of these cognitive interventions is to optimize the performance of each patient by making the most of its preserved abilities. Moreover, another goal of the day care center is also to support the caregivers, by giving them advices in order to adequately face with difficulties presented by the patient. [less ▲]

We report a cognitive investigation of a case of deep dysphasia appearing in the context of primary progressive aphasia. Over a period of 5 years, patient CO presented progressive difficulties in word finding and in oral comprehension, while nonverbal cognitive functions remained preserved. As in other deep dysphasic patients, CO's repetition performance showed marked imageability and lexicality effects, and semantic paraphasias. The same effects were observed in writing-to-dictation. Regularisation errors occurred in word reading. CO's short-term memory span was less than two words. A cognitive analysis of language processing revealed difficulties in phoneme identification and rhyme judgement, in detecting grammatical class for orally presented words, and in oral and written naming. The interpretation of CO's deep dysphasic symptoms within interactive models of language processing confirmed the importance of a phonological short-term storage impairment as an explanatory factor of deep dysphasia.
