La névralgie post-zostérienne (NPZ) est une catégorie particulière du groupe des douleurs associées au zona (DAZ). Il s’agit d’une neuropathie particulièrement douloureuse survenant ou persistant après la guérison des lésions cutanées d’un zona. Elle est plus fréquente chez les sujets âgés et peut s’avérer invalidante. Les traitements antiviraux, topiques et/ou oraux, les antalgiques et les anti-dépresseurs tricycliques sont régulièrement source de déception et/ou d’effets secondaires pour le patient atteint de NPZ. L’efficacité et la sécurité d’emploi des analogues de l’acide gamma-amino-butyrique ouvrent de nouvelles perspectives dans la prise en charge de la NPZ. Nous décrivons dans cet article les caractéristiques et modalités de prescription de ces molécules peu connues des dermatologues. [less ▲]

Intertrigo is a common inflammatory disorder of various origins. It can represent a single skin manifestation or be part of a disorder possibly exhibiting specific manifestations on other parts of the body. Hence, intertrigo may represent a sign of a peculiar disease. It may also result from physicochemical aggressions of the skin following occlusion of the large skinfolds particularly in case of stoutness. In this intance, a treatment similar to that of diaper dermatitis can be offered. A paste enriched or not with miconazole nitrate is indicated to prevent or treat most presentations of intertrigo. BACKGROUND: Seborrheic dermatitis is considered to be a Malassezia-driven disease. Little objective information is available so far from biometrological quantitative assessments of this skin condition. Pramiconazole is a novel triazole with potent in vitro antifungal activity, especially against Malassezia spp. OBJECTIVE: To study the sequential effects of pramiconazole on Malassezia, inflammation and epidermal changes. METHOD:This study was performed in 2 groups of subjects suffering from seborrheic dermatitis. The first group (n = 17) remained untreated and was used as control. Clinical, mycological and biometrological assessments were performed at inclusion and during the following 2 weeks. The second group of subjects (n = 10) received a single 200-mg oral dose of pramiconazole at inclusion. Clinical, mycological and biometrological evaluations were performed before and during 1 month following the single antifungal intake. For both parts of the study, several parameters were assessed including yeast density, desquamation, erythema, itching and sebum excretion. RESULTS: In the control group, no significant changes were observed in any of the parameters during the observation period. The findings were markedly different in the pramiconazole-treated subjects. The yeast density was significantly improved on days 3, 7 and 28. Desquamation, erythema, itching, and the global clinical evaluation as assessed by the patients and investigators became significantly improved on days 7 and 28. A trend in decrease of scaliness was noted. No effect on sebum excretion was evidenced. In conclusion, a single 200-mg dose of pramiconazole exhibitsin vivo efficacy in controlling some important clinical aspects of seborrheic dermatitis. Following a reduction in the number of yeasts on day 3, a decrease in the severity of clinical signs and symptoms occurred from day 7 onwards. Sebum excretion appeared uninvolved in the clearing process of seborrheic dermatitis. CONCLUSION: A single 200-mg dose of pramiconazole appears to abate seborrheic dermatitis. The density in Malassezia present on lesional skin is first decreased, followed by clearing of the clinical signs. [less ▲]

The profound endocrine upheaval of pregnancy is frequently associated with changes in the function and structure of the blood and lymph microvasculature of the skin and mucosae. Palmar erythema is frequent but rarely severe and is associated with burning sensations. Spider telangiectasias develop in the majority of pregnant White women but are less often identified in women with darker skin. Hemangiomas and glomus tumors occur in one-third of women. A firm edema may develop on the face and extremities in the final months of pregnancy. Venous hypertension and varicosities of the lower limbs are common. Gingivitis, gum hypertrophy, and pyogenic granuloma are common in the oral cavity. The vaginal mucosa is also affected, showing a violaceous aspect, at times accompanied by varicosities of the vulva. Many gestational vascular changes regress spontaneously in the postpartum phase. Some vascular tumors may need to be treated with a vascular laser or intense pulsed light. [less ▲]

Impetigo is a superficial bacterial infection of the skin. It is a contagious condition particularly affecting children. In many instances, a topical antibacterial treatment is recommended to clear the disease. Topical antibiotics and antiseptics can be used, but the risk for antibiotic-induced bacterial resistance of the community-acquired type has been pointed out. The present study was undertaken as a randomized intraindividual comparative trial between fusidic acid (FA) cream and povidone iodine (PVP-I) gel in 40 children suffering from impetigo. Cultures revealed Staphylococcus aureus in the vast majority of the cases. The lesions present on each upper limb were treated twice daily with one or the other test formulation. Clinical assessments were performed twice weekly until cure. After one week, cure was obtained in 57.5% of the PVP-I-treated limbs compared to 32.5% of the FA-treated limbs. At the end of treatment cure was obtained first on the PVP-I treated sites in 67.5% of cases compared to 15% on the FA-treated sites. Clinical cure occurred simultaneously on both test sites in 17.5% of cases. In conclusion, PVP-I provided faster clinical cure than FA. Considering the treatment cost, the raising community-acquired staphylococci resistance to FA, and the results of the present study, PVP-I gel appears superior to FA cream in childhood impetigo. Un défaut local des mécanismes de contrôle de la flore commensale est vraisemblablement à l’origine de la prolifération pathologique de ces micro-organismes. Diaper dermatitis is the most frequent skin disorder of the newborn. Several clinical types are distinguished. The most frequent type results from increased fragility of the newborn buttock skin when covered by diapers. According to the mechanisms involved and the severity of the dermatitis, one can distinguish the intertrigo of the chubby baby, and the so-called "W", "Y" and "red panties" types of diaper dermatitis. When the effects of occlusion are not controlled by adequate absorption by the diapers maceration of the stratum corneum occurs. As a result, degradation of the skin barrier function takes place. In addition, the value of the coefficient of friction of the skin increases with epidermal weakening to rubbing. In addition, fecal enzymes alter urines and skin. Judicious hygiene measures and a correct choice of care and diapers are mandatory. Cutaneous colonisation by microorganisms, in particular the yeasts Candida spp, is the main complication. Adequate preventive and curative measures can combat diaper dermatitis with confidence. A miconazole paste allows to improve the tribological properties of the interface between diapers and the skin. It also corrects the degradation of the skin barrier function, reduces inflammation and abates the impact of Candida spp. in the pathogenesis of the skin disorder. [less ▲]

The topical treatment of psoriasis benefits from the alternate use of dermocorticosteroids and vitamin D3 analogues. A new galenic formulation allows to combine them in a single application. Dovobet (LEO Pharma) ointment is the association of calcipotriol 50 microg/g with betamethasone dipoprionate 0.5 mg/g. This formulation boosts the therapeutic activity of calcipotriol. It also decreases the irritative inflammatory reaction due to calcipotriol without increasing the atrophogenic risk of the dermocorticoid. [less ▲]
