The critical bacterial colonizaion of leg ulcers can impair their healing rate, aggravate the patient discomfort and increase the medical and nursing costs. In recent times, the dressings designed for leg ulcers have followed a pace of conceptual revolution. Some of them are now offered containing an antiseptic of the silver salt family. The silver concentraton delivered into the wound bed is important to consider when assessing treatment efficacy. The diversity of the silver-based dressings currently on the European market is as large as their differences in activity. Only a minority of these dressings adequately control the wound biocenosis. Their cost which is high for the patient, must be compared to that of nursing care that may become less important. The expected beneficit is a reduction in healing time. [less ▲]

L'insuffisance veineuse des membres inférieurs est un état physiopathologique insidieusement progressif qui se manifeste fréquemment chez la femme. Par la cascade de ses conséquences, elle entraîne une microangiopathie cutanée. L'ulcère chronique de jambe en est l'issue tardive avec son cortège d'aspects médicaux et socio-économiques. La pathogénie des lésions cutanées est en relation directe avec l'hémorrhéologie et l’adhésion des leucocytes intracapillaires. La migration de leucocytes dans les tissus périveinulaires, avec subséquemment une activation inappropriée de ces cellules, est en relation avec l’importance des troubles trophiques, de la lipodermatosclérose et de la chronicité des ulcères veineux. La contention élastique et quelques médications veinotropes exercent une action thérapeutique sur les symptômes de jambe lourde. Elles ont aussi des effets préventifs sur le développement du tableau progressif de cette pathologie microvasculaire. [less ▲]

Chronic leg ulcer is the late issue of the process accompanied by varied medical and socio-economic aspects. The pathogeny of the cutaneous lesions is directly related to alterations in hemorrheology and leukocyte adhesion inside capillaries. Elastic contention and some venotropic drugs exert preventive effects on the development of leg ulcers. They also bring some therapeutic improvement of the symptoms of leg heaviness. [less ▲]

During the past decades, the incidence of skin cancers has been on the rise in Wallonia as in other regions of similar latitude in the rest of the world. Awareness of the population to the dangers of sun exposure and to the benefit of early cancer diagnosis is of the upmost importance and undisputed. The impact of the information delivered by the media and of the actions initiated by physicians is heterogeneous. The positive and unquestionable aspect is hampered if the population at risk is not properly targeted. It is also reduced if the problem understanding creates a surge in telly-belly syndrome. Erysipelas is a serious infection of the skin. In case of delay in initiating adequate antibiotic treatment, complications, sometimes dismal, can supervene. In addition, erysipelas shows a tendancy to recurrences. The prevention of an episode of erysipelas calls for correct personal hygiene and adequate use of topical antiseptics in case of skin effraction, even when minimal. When erysipelas is established, a rapidly initiated antibiotic treatment for a prolonged period prevents streptococcal gangrene complications. Elastic contention of any leg edema from venous or lymphatic origin and prophylactic antisepsis of discrete wounds help in preventing erysipelas recurrences. [less ▲]

Chronic leg ulcers are typically wounds that do not heal at a normal rate. Impaired healing appears to be due to primary microvascular changes and it is aggravated by ongoing bacteria-driven vasculitis. The various cytokines identified in experimental wounds are also present in leg ulcers. VEGF is strongly implicated as a promoter of blood vessel growth in patients with venous disease. In addition, there is good evidence of increased expression of bFGF, TGF-beta1, and PDGF in lipodermatosclerosis. All of these growth factors are involved in wound healing. Upregulated TGF-beta1 is probably one of the main causes of the fibrosis observed in lipodermatosclerosis. In leg ulcers, cytokines appear to be trapped in the perivascular fibrinoid deposits. It is not the nature and amount of cytokines that are inadequate in leg ulcers, but rather their spatial distribution. Dermal dendrocytes (DD) are resident factor XIIIa-enriched macrophages. They likely play a role in tissue repair when boosted adequately. New therapies aiming at helping the release of cytokines by DD apparently promote and improve the healing phase. [less ▲]
