Platelet-Rich Plasma (PRP) contains growth factors involved in the tissular healing process. The aim of the study was to determine if an injection of PRP could improve the healing of sectioned Achilles tendons of rats. After surgery, rats received an injection of PRP (n=60) or a physiological solution (n=60) in situ. After 5, 15 and 30 days, 20 rats of both groups were euthanized and 15 collected tendons were submitted to a biomechanical test using cryo-jaws before performing transcriptomic analyses. Histological and biochemical analyses were performed on the 5 remaining tendons in each group. Tendons in the PRP group were more resistant to rupture at 15 and 30 days. The mechanical stress was significantly increased in tendons of the PRP group at day 30. Histological analysis showed a precocious deposition of fibrillar collagen at day 5 confirmed by a biochemical measurement. The expression of tenomodulin was significantly higher at day 5. The mRNA level of type III collage, matrix metalloproteinase 2, 3 and 9 was similar in the 2 groups at all time points whereas type I collagen was significantly increased at day 30 in the PRP group. In conclusion, an injection of PRP in sectioned rat Achilles tendon influences the early phase of tendons healing and results in an ultimate stronger mechanical resistance. [less ▲]

Abstract Background: Adhesion formation is common after abdominal surgery. The incidence and severity of adhesion formation following open or laparoscopic surgery remain controversial. The role of CO(2) pneumoperitoneum is also widely discussed. This study aimed to compare adhesion formation following peritoneal injury by electrocoagulation performed through open or laparoscopic procedures in a rat model. Materials and Methods: Sixty male rats were randomized to undergo a 1.5-cm peritoneal injury with unipolar cautery under general anesthesia: open surgery (Group A, n=20), laparoscopic surgery with CO(2) pneumoperitoneum (Group B, n=20), and laparoscopic surgery with air pneumoperitoneum (Group C, n=20). Duration of the procedures was fixed at 90 minutes in all groups, and pneumoperitoneum pressure was kept at 10 mm Hg. Ten days later, the animals underwent a secondary laparotomy to score peritoneal adhesions using qualitative and quantitative parameters. Results: Forty-five rats developed at least one adhesion: 95% in Group A, 83% in Group B, and 55% in Group C (P<.01; Group C versus Group A, P<.01). According to number, thickness, tenacity, vascularization, extent, type, and grading according to the Zuhkle classification, no significant difference was observed between Groups A and B. The distribution of adhesions after open surgery was significantly different than after laparoscopic surgery (P<.001). It is interesting that Group C rats developed significantly fewer adhesions at the traumatized site, and their adhesions had less severe qualitative scores compared with those after open surgery (P<.01). Conclusions: In this animal model, CO(2) laparoscopic surgery did not decrease the formation of postoperative adhesion, compared with open surgery. The difference with the animals operated on with air pneumoperitoneum emphasizes the role of CO(2) in peritoneal injury leading to adhesion formation. [less ▲]

Introduction: The treatment of choice for tendinopathies is eccentric reeducation. Although the clinical results appear favourable, the biomechanical changes to the tissue are not yet clear. Materiel and methods: This study compared the effects of two methods of training (eccentric (E) training and concentric (C) training) with untrained (U) rats. The animals underwent training over a period of five weeks. The tricipital, patellar and Achilles tendons were subsequently removed to perform a traction test to the point of tendon rupture, and a histological analysis was performed. Results: There was a significant improvement in the rupture force of the patellar and tricipital tendons between the U and E groups. The tricipital tendons in the control group presented a significantly smaller cross-section than the E- and C-trained groups. No significant difference was observed for the constraints between the three groups for all three tendons. However, a tendency towards improvement was observed between the trained and the U groups for the patellar tendon. Histological studies demonstrated the development of a greater number of blood vessels and a larger quantity of collagen in the eccentric group. Discussion and conclusion: The mechanical properties of tendons in rats improve after specific training, especially following eccentric training. [less ▲]

Introduction: Platelet-Rich Plasma (PRP) contains lot of growth factors which could enhance the healing process of different tissues. We aimed to determine if a single injection of PRP could improve the cicatrisation of ruptured Achilles tendons of rats. Material and Methods: A 5mm defect was surgically made in the Achilles tendon of 120 rats. A few hours after surgery, 45 rats received a PRP or PBS injection in situ. After 5, 15 and 30 days, 20 rats of both groups were euthanized and 15 collected tendons were immediately submitted to a biomechanical tensile strength test until rupture using a “cryo-jaw” device. After, theses samples were used for transcriptomic analyses. Histological and biochemical analyses were performed on the five remained tendons in each group. Results: Tendons in the PRP group were more resistant to rupture at 15 and 30 days than those in the control group. The transverse area of tendons in the PRP group was significantly higher at day 5 and 15. The constraint was significantly increased in tendons of the PRP group in the late phase of the healing (day 30). Histological and immunohistological analysis showed an increased staining for fibrillar collagen at day 5 confirmed by a biochemical analysis showing an increased collagen concentration in the callus. The expression of tenomodulin, a tenocyte differentiation marker, was significantly higher in the PRP-treated tendons at day 5. No significant difference in terms of mRNA for type III collagen and matrix metalloproteinase 9 was observed at any time between the 2 groups. Conclusion: A single injection of PRP in sectioned Achilles tendon of rats few hours after surgery influences the early phase of tendons healing, resulting in an ultimate stronger mechanical resistance. The estimated antioxidant capacity of different matrices can vary considerably between research reports. Besides intrinsic factors (not studied here), our work showed that may have various causes. Firstly, different methods are used to measure antioxidant capacity. Secondly, the results obtained for a single matrix by one method (such as ORAC) can vary with the extraction conditions. Parameters having a great impact on the amount and composition of antioxidants in extracts, and thus on the measured antioxidant capacity, notably include the extraction solvent composition, temperature, extraction time (duration), solvent-to-solid ratio, and storage conditions. Standardization of the extraction procedure is thus necessary for accurate and reproducible determination of the antioxidant capacity and phenolics in different food matrices by different laboratories. In this study we optimised such a procedure for four fresh plant matrices (orange, apple, leek, and broccoli). The optimised procedure requires extraction in a mixture of acetone/water/acetic acid (70/28/2, v/v/v) for 1 h at 4°C, with a solvent-to-solid ratio of 20 mL per 1 g. Fresh material should be used, but if this is not possible, one may lyophilise the plant matrices or store the extracts for a few days at -20°C before analysis. [less ▲]

Aortic dissection is one of the most serious aortic diseases by its potential for rupture, but also for other complications, such as cerebral or splanchnic ischemia, which may be fatal. If open surgery is the rule for lesions of the ascending aorta (type A), type B (not concerning the ascending aorta) is first a matter of medical treatment except when complications are present. In this case the placement of a stentgraft is a valuable alternative to open surgery. We report a patient presenting with a type B aortic dissection, characterized by rapid expansion and complicated by peri-aortic leakage, who was successfully treated by thoracic aortic stentgraft placement. This was done in a hybrid operating room associating the characteristics of a classical operating room for cardio-vascular surgery with those of an interventional radiology suite. [less ▲]

The authors recently observed two patients who suffered acute lower limb ischemia secondary to thrombosis of a small (diameter of less than 2 cm) popliteal artery aneurysm. A literature search lead to the conclusion that small popliteal aneurysms, covered with mural thrombus and presenting a distorted neck due to elongation, are prone to thrombosis. Another risk factor is a restrained outflow bed as a result of previous micro-embolisations into the tibial arteries. The authors conclude that small popliteal artery aneurysms are not so innocent as is often presumed. [less ▲]

Cardiovascular disease is the main cause of morbidity and mortality in patients with Marfan syndrome. The most life threatening complication is aortic root aneurysms leading to aortic dissection or rupture. It can be prevented by regular aortic follow-up and prophylactic aortic surgery. Modern aortic surgery has led to a substantial increase in the life expectancy of these patients. We report two cases of Marfan syndrome with cardiovascular complications. BACKGROUND: Abdominal aortic aneurysm (AAA) is a major cause of preventable deaths in older patients. Oxidative stress has been suggested to play a key role in the pathogenesis of AAA. However, only few studies have been conducted to evaluate the blood oxidative stress status of AAA patients. METHODS AND RESULTS: Twenty seven AAA patients (mean age of 70 years) divided into two groups according to AAA size (</= 50 or > 50 mm) were compared with an age-matched group of 18 healthy subjects. Antioxidants (vitamins C and E, beta-carotene, glutathione, thiols, and ubiquinone), trace elements (selenium, copper, zinc, and copper/zinc ratio) and markers of oxidative damage to lipids (lipid peroxides, antibodies against oxidized patients, and isoprostanes) were measured in each subject. The comparison of the three groups by ordinal logistic regression showed a significant decrease of the plasma levels of vitamin C (P = 0.011), alpha-tocopherol (P = 0.016) but not when corrected for cholesterol values, beta-carotene (P = 0.0096), ubiquinone (P = 0.014), zinc (P = 0.0035), and of selenium (P = 0.0038), as AAA size increased. By contrast, specific markers of lipid peroxidation such as the Cu/Zn ratio (P = 0.046) and to a lesser extent isoprostanes (P = 0.052) increased. CONCLUSION: The present study emphasizes the potential role of the oxidative stress in AAA disease and suggests that an antioxidant therapy could be of interest to delay AAA progression. [less ▲]

We report the case of a patient admitted at the emergency unit after a severe car accident. As ECG showed a ST segment elevation in all leads, the working diagnosis was coronary dissection. Coronary angiography revealed a large interventricular septal rupture, confirmed by echocardiography. After discussion and as haemodynamics permitted, 6 weeks of medical observation were decided. A surgical repair was then performed, and provided a perfect repair of the shunt. We discuss about the prevalence and management of this rare traumatic complication. [less ▲]

Blackcurrant berries contain high amounts of flavonoids with various health benefits as anti-inflammatory properties attributed to their antioxidant potential. Leaves and buds actually used to produce food supplement could also exhibit such interesting properties. <br />In the literature, various methods are often used and valid indicators of the antioxidant potential of dietary substances. However these assays do not provide evidence that antioxidants have in vivo or ex vivo activity when consumed. To obtain biologically relevant information, the antioxidant activities of the extracts were evaluated on cellular models implicating the measurement of blood haemolysis, the Cellular Antioxidant Activity on endothelial cells and the anti-inflammatory activities on isolated equine stimulated neutrophils and purified myeloperoxidase. <br />These tests generally showed that the blackcurrant leaf extract have the highest antioxidant and <br />anti-inflammatory (inhibition of MPO activity and ROS production on activated neutrophils) capacities correlated to the highest total phenolics content. Bronchogenic cysts arise from an abnormal budding of the ventral diverticulum of the foregut or the tracheobronchial tree during embryogenesis. Pericardial location of these cysts is very rare. We describe a case of a young asymptomatic woman with an intrapericardial cystic mass compressing the right heart. Because of severe adhesions of the mass to the ascending aorta and to the right coronary artery, these structures were injured during surgical resection requiring the replacement of the ascending aorta and a coronary artery by-pass graft. Only the histopathologic findings provided the final diagnosis. [less ▲]

Strawberries are a good source of micronutrients, especially antioxidant phenolics. More information is needed to better exploit the health-promoting effect of such fruits. Several studies focused on the effects of genotype, cultural practices and seasonal variation on the antioxidant potential of strawberries, but often yielding contradictory results and/or focusing on only one source of variability. In the present study, we showed that total phenolic compounds, ascorbic acid and antioxidant capacity strongly differed between genotypes of strawberry. The precise results revealed the importance of genetic background for the antioxidant capacity and for the content of total phenolics (with up to 3.3-fold variations). Other parameters may also influence the antioxidant capacity in strawberry such as harvest time, culture conditions and environmental factors. Moreover, in this study, the harvesting time (at the same ripening stage) appeared to be very important, more important than genotype. Variations of the antioxidant capacity of up to 4.1-folds were observed following the harvesting time of “Elsanta” cultivar. [less ▲]
