In the trend of the endovascular revolution for occlusive arterial disease, management of aneurysmal disease has known an analogous change in the nineties. Elective endo-aneurysmal stentgrafting (EVAR or endovascular aneurysm repair) is nowadays of widespread application in contemporary practice all over the world, sometimes by over-enthusiast proponents. There is a burden of recent publications on that topic. Critical analysis of relevant data reveal compelling evidence that elective EVAR is a safe procedure with a threefold reduced 30-day morbidity-mortality, when compared to conventional open aneurysm repair. EVAR also deemed efficace in terms of AAA rupture prevention. At the other hand, mid-term follow-up revealed that EVAR does not endure at the long-term in any late survival benefit, compared to open AAA repair. The major concern and drawback of EVAR is its higher cost and the need for indefinite, life-long surveillance, with a 20% reintervention rate (almost catheter-based endovascularly) at 5-year follow-up. In this review-paper, short-term gains of EVAR are balanced against the inherent disadvantages and long-term losses. According best available evidence, EVAR should not longer be the first choice for high-risk patients, judged unfit for open aneurysm repair. There also exist doubt on the use of EVAR in young fit patients with an anticipated long life expectancy. In approximately half of the AAA patients, EVAR can be considered as the appropriate treatment option. [less ▲]

Since 2000, the Faculty of Medicine at the University of Liege has introduced Problem-Based Learning (PBL) in the curriculum of medical studies (7 years). This contextualized learning raises the issue oc capacity of knowledge transfer and generalization, and maturation of clinical reasoning, troughout the curriculum. Two tests were administered to 108 students (year three to six) and to reference panel. The selected topic was endocrinology. We adapted the script concordance test (i.e., the SCT, see http://cme.umontreal.ca/tcs/) with 60 questions asking for evaluation of the impact of a new information, in a context (short clinical scenario) that was written similar versus different of the original learning context. A True/False test was used to assess factual knowledge. Progress throughout the curriculum was associated with increase in clinical reasoning performance together with a decrease in factual knowledge scores. Early capacity of knowledge transfer was observed, with no difference between the 4 years studied. Overall, the SCT score obtained out of the original context were unexpectedly higher than in such a context. We hypothesize that a new information may result in greater activation of reasoning in a new and realistic context than in a known and possibly factitious context. [less ▲]

Contexte et objectif: L'acquisition de connaissances et le développement du raisonnement clinique constituent des objectifs majeurs de la formation médicale. Depuis 2000, la Faculté de médecine de l'Université de Liège a introduit des séminaires d'Apprentissage Par Problèmes tout au long de son curriculum de formation (7 ans). L'objectif de cette recherche est d'évaluer la maturation du raisonnement clinique et la capacité de transfert à de nouvelles situations par comparaison à l'évolution des connaissances factuelles, dans un curriculum privilégiant la contextualisation des apprentissages (à partir de vignette cliniques). Méthodologie: Un test de connaissances factuelles (Vrai/Faux) et un test de raisonnement clinique (Test de Concordance de Scripts) ont été administrés à 104 étudiants (de la 3ième à la 6ième année) et à un panel de référents. Le Test de Concordance de Scripts (TCS) a été adapté en créant 2 séries de questions : une série portant sur des situations similaires à celles de l'apprentissage et une série portant sur des situations inédites. Le domaine de recherche choisi était l'endocrinologie. Résultats et Conclusion: La maturation au cours du curriculum est associée à un accroissement des performances au TCS et à une diminution des performances au test de connaissances factuelles. Une corrélation positive entre les performances aux deux tests est observée uniquement pour les étudiants plus jeunes (3ème et 4ème années). La différence entre les performances au TCS en contexte d'apprentissage versus en contexte inédit ne change pas en fonction du niveau d'étude, ce qui suggère une capacité de transfert présente dès le début de la formation APP. Contrairement à nos attentes, le score aux questions TCS portant sur des situations inédites était supérieur à celui obtenu pour des situations similaires à celles de l'apprentissage. Ces résultats suggèrent qu'un contexte nouveau et/ou plus plausible pourraient activer davantage le raisonnement. [less ▲]

Interest in the consumption of fresh fruits and vegetables is, to a large extent, due to its content of bioactive nutrients and their importance as dietary antioxidants. Among all of the selected fruits and vegetables, strawberries and black grapes have relatively high antioxidant capacities associated with high contents of total phenolic compounds, ascorbic acid, and flavonols. More interesting, the results of this study indicated that in most fruits and vegetables storage did not affect negatively the antioxidant capacity. Better, in some cases, an increase of the antioxidant capacity was observed in the days following their purchase, accompanied by an increase in phenolic compounds. In general, fruits and vegetables visually spoil before any significant antioxidant capacity loss occurs except in banana and broccoli. When ascorbic acid or flavonoids (aglycons of flavonols and anthocyanins) were concerned, the conclusions were similar. Their content was generally stable during storage. [less ▲]

To test the efficacy of BM-573 in atherogenesis, the effect of 10 weeks of treatment with BM573 (10 mg/l) on early aortic atherosclerotic lesions of apo E deficient mice was assessed. These mice were fed with chow diet, with spontaneous increase of total plasma cholesterol and triglycerides. In this experiment, while BM-573 did not affect body weight, it significantly decreases early atherogenesis lesions confirmed by macroscopic, microscopic and biochemical analysis. These results confirm that selective antagonism of TP receptor is effective in reducing atherosclerotic lesion in apo E deficient mice. Consequently, BM-573 could be a potential drug for prevention of atherosclerosis [less ▲]

Oxidative stress is defined as an imbalance between the production of reactive oxygen species (ROS) and the antioxidant network, in favour of the former. Our lifestyle (smoking, alcoholism, obesity, intense physical exercise), but also our inadequate diet, contributes to significantly increase the production of ROS in our organism. This is potentially associated with an increased risk of developing ageing-related pathologies such as cardiovascular diseases and cancer. As a matter of prevention, it is necessary to have in hands a high technology allowing to correctly evidence the oxidative stress status of an individual in order to render optimal our antioxidant defences and to decrease the oxidative damages in DNA, proteins and lipids. Oxidative stress is associated with the development of several disorders including cardiovascular disease and cancer. Among conditions known to influence oxidative stress, the use of oral contraception (OC) in women has been a matter of ongoing discussion. METHODS: A total of 897 eligible and healthy volunteers were recruited from among the patients of 50 general practitioners participating in the ELAN study (Etude Liegeoise sur les ANtioxydants). A subsample consisting of 209 women aged 40-48 years was studied for a comprehensive oxidative stress status (OSS), including the analysis of antioxidants, trace elements and three markers of oxidative damage to lipids. Among 209 subsample, 49 (23%) were OC users (OCU), 119 (57%) non-contraception users (NCU) and 41 (20%) were intrauterine (hormonal and copper) devices users (IUD). RESULTS: After adjustment for smoking, systolic and diastolic blood pressure and BMI (or waist circumference), a marked and significant increase in lipid peroxides was observed among OCU women when compared with NCU and IUD users. A cut-off value of 660 mu M in lipid peroxides allowed the discrimination of OCU from the two other groups. In contrast, no difference was observed in the plasma concentration of both oxidized low-density lipoprotein (LDL) and their related antibodies. The increased level in lipid peroxides was strongly related to higher concentrations of copper (r < 0.84; P < 0.0001, cut-off value 1.2 mg/1). When compared with NCU and IUD users, plasma antioxidant defences were significantly altered in OCU women as shown by lower levels of beta-carotene (decrease of 39%; P < 0.01) andytocopherol (decrease by 22%; P < 0.01). In contrast, higher concentrations of selenium (increased by 11.8%; P < 0.01) were observed in OCU women. Blood concentrations of vitamin C, alpha-tocopherol and zinc were unaffected by OC use. CONCLUSIONS: The intake of OC significantly increases the lipid peroxidation in women aged 40-48 years. This may represent a potential cardiovascular risk factor for these women. [less ▲]

Atherosclerotic cardiovascular disease, according to World Health Organization, is the primary cause of heart disease and stroke. Atherosclerosis is a chronic vascular disease whose development is influenced by several mediators. Among them, the action of eicosanoïds such as thromboxane A2 and 8-iso-PGF2a have recently received a lot of attention. The aim of our study was the evaluation of benefits of original molecules, synthesised in our lab, targeting the thromboxane receptor (TP) in an apo E deficient mouse. We previously demonstrated in several in vitro and in vivo pharmacological experiments that our original sulfonylurea derivate, BM-573 was a potent combined inhibitor of the thromboxane synthase and antagonist of TP. Since TP is implied in atherosclerosis development, such antagonist could have a great therapeutic impact in atherogenesis.To test the efficacy of BM-573 in atherogenesis, the effect of 10 weeks of treatment with BM573 (10 mg/kg) on early aortic atherosclerotic lesions of apo E deficient mice was assessed. These mice were fed with chow diet, with spontaneous increase of total plasma cholesterol and triglycerides. Afin d’examiner l'efficacité du BM-573 dans l'athérogenèse, des souris apo E-/- ont été traitées durant 10 semaines avec le BM573 (10mg/kg). Au cours de cette expérience, le traitement des animaux par le BM-573, a eu pour effet de diminuer les lésions athéromateuses précoces de manière significative. Ces données ont été confirmées par des analyses histopathologiques et biochimiques. Ces résultats confirment que l'antagonisme sélectif des récepteurs TP associé à une inhibition de la thromboxane synthase réduit significativement les lésions athéromateuses chez les souris apoE-/-.. Le BM-573 est, par conséquent, un agent thérapeutique potentiel pour la prévention de l'athérosclérose [less ▲]

Although previously studied in patients with chronic kidney disease, there is less data for the use of cystatin C and cystatin C-based formulas in heart transplant recipients. The ability of creatinine and cystatin C to detect renal failure (glomerular filtration rate [GFR] below 60 mL/min/1.73 m(2)) in heart transplant patients has been compared. The accuracy and precision of a creatinine-based formula (Modification of Diet in Renal Disease [MDRD]) versus a cystatin C-based formula (Rule's formula) to estimate GFR have also been studied. GFR was measured using the (51)Crethylenediamine tetraacetic acid tracer in 27 patients. There was no significant difference between GFR and the reciprocal of creatinine or cystatin C. Receiver operating characteristic curves for cystatin C and creatinine were similar. Both formulas were well correlated with the GFR. The bias of the cystatin C-based was significantly better than one of the MDRD formula, but the standard deviation appeared better for the MDRD formula (bias of +3.9 mL/min/1.73 m(2) versus +12 mL/min/1.73 m(2) and SD of 8.5 versus 11.6, respectively). Plasma cystatin C has no clear advantage over serum creatinine to detect renal failure in heart transplanted patients. [less ▲]

Contexte et objectif: En 2000, la Faculté de Médecine de l'ULg a introduit le PBL tout au long du cursus des études. Les enseignements en PBL étant contextualisés (basés sur des vignettes cliniques) l'apprenant devra décontextualiser les acquis afin de pouvoir les utiliser dans des situations inédites. L'objectif de cette recherche est de développer une méthodologie permettant d'adapter le TCS à l'évaluation de la maturation des capacités de décontextualisation/recontextualisation (entre la troisième et la sixième année du cursus). Méthodologie: Le TCS, créé pour l'évaluation du raisonnement diagnostique et thérapeutique en fin de formation, sera utilisé ici autour d'un même raisonnement physiopathologique appliqué à des problèmes qui diffèrent sur le contexte. Concrètement, nous élaborons un TCS à partir de deux séries de questions. La série "contexte original" comporte des informations extraites des vignettes utilisées lors de la formation, sans éléments distracteurs. L'objectif d'apprentissage est mis au premier plan dans le scénario et la fonction physiologique étudiée est perturbée dans le même sens que dans la vignette originale de formation. La série "contexte inédit" ne comporte que des informations nouvelles, avec des éléments distracteurs neutres. L'objectif d'apprentissage apparait au second plan et la fonction étudiée varie dans un sens différent de la vignette originale d'apprentissage. L'information dont l'impact est à évaluer est hors du contexte original pour les deux séries. Résultats et Conclusions: L'endocrinologie est le module d'enseignement choisi pour constituer les questions Celles-ci sont rédigées à propos du diabète (type 1 et 2), de l'hyper- et hypo-thyroïdie, du syndrome de Cushing et de l'acromégalie. Il apparait possible de concevoir un TCS centré sur le raisonnement physiopathologique en vue d'étudier l'impact de la contextualisation des apprentissages. [less ▲]

More extensive use of non-heart-beating donors (NHBD) could reduce mortality on liver transplantation waiting lists, but this is associated with more primary nonfunction (PNF). We assessed which parameters are involved in the development of PNF in livers from NHBD in a previously validated pig liver transplantation model, in which livers were transplanted after exposure to incremental periods of warm ischemia. The risk of PNF was unacceptably high (>50%) when livers were exposed to >30 minutes' warm ischemia before a short cold ischemic period. This study examined how PNF is affected by Kupffer cell activation (beta-galactosidase), the generation of cytokines tumor necrosis factor alpha and interleukin 6, antioxidant mechanisms (ascorbic acid, alpha-tocopherol, reduced glutathione), circulating redox-active iron, and sinusoidal endothelial cell function (hyaluronic acid clearance). Kupffer cells were more activated in PNF recipients, as suggested by higher beta-galactosidase levels (15 minutes after reperfusion), and secondarily, by higher production of tumor necrosis factor alpha and interleukin 6 (180 minutes after reperfusion). In addition, a-tocopherol and reduced glutathione were lower, and ascorbic acid and redox-active iron higher in PNF recipients. Finally, PNF grafts displayed progressively decreasing hyaluronic acid clearance (suggesting sinusoidal endothelial cell dysfunction) and parenchymal edema. Consequently, a reduced-flow phenomenon was documented. In grafts from NHBD that are destined to fail, beta-galactosidase activity (a surrogate of Kupffer cell activation) is higher, proinflammatory cytokines are overproduced, some antioxidant mechanisms fail, and circulating redox-active iron is more rapidly released. A no-flow phenomenon is eventually observed in these failing grafts. [less ▲]
