Les progrès technologiques réalisés par l’image- rie médicale l’ont placée au centre de la prise en charge des patients oncologiques, tant au niveau du diagnostic, du pro - nostic et du suivi que dans la prise en charge thérapeutique. En effet, l’imagerie représente, à l’heure actuelle, la pierre angulaire des traitements de radiothérapie. Les objectifs du radiothérapeute sont d’irradier le plus précisément possible la tumeur à dose curative, tout en évitant les organes sains. Pour y arriver, le radiothérapeute utilise de façon routinière l’imagerie anatomique (Scanner et IRM). Depuis quelques années, le développement des différentes imageries métabo - liques et fonctionnelles, comme l’imagerie par émission de positons (PET-CT) et la résonnance magnétique fonctionnelle, ouvrent de nouvelles possibilités thérapeutiques grâce aux informations qu’elles apportent sur la biologie des tumeurs. Cet article décrit, de manière non exhaustive, les différentes imageries anatomiques et métaboliques à la disposition du radiothérapeute. [less ▲]

La cystectomie est actuelle- ment le traitement «standard» des cancers infiltrants de la vessie. Cependant, elle est liée à un taux élevé de complications et à une altération de la qualité de vie, en particulier, chez les patients âgés présentant de nombreuses comorbidités. Purpose: To our knowledge, no study has specifically assessed the time course of anxiety during radiotherapy (RT). The objective of this study was to assess anxiety time courses in patients with nonmetastatic breast cancer. Material and methods: This multicenter, descriptive longitudinal study included 213 consecutive patients with breast cancer who completed visual analog scales (VASs) assessing state anxiety before and after the RT simulation and the first and last five RT sessions. Results: Pre- and post-session anxiety mean levels were highest at the RT simulation (respectively, 2.9 ± 2.9 and 1.6 ± 2.5) and first RT session (respectively, 3.4 ± 2.9 and 2.0 ± 2.4), then declined rapidly. Clinically relevant mean differences (P1 cm on the VAS) between pre- and post-simulation/session VAS scores were found only for the RT simulation ( 1.3 ± 2.7; p < 0.001) and first RT session ( 1.4 ± 2.4; p < 0.001). Five percent to 16% of patients presented clinically relevant anxiety (pre- and post-simulation/session VAS scoresP4 cm) throughout treatment. Conclusions: To optimize care, RT team members should offer all patients appropriate information about treatment at the simulation, check patients’ understanding, and identify patients with clinically relevant anxiety requiring appropriate support throughout RT. High-quality radiotherapy (RT) treatment planning requires the combination of information arising from multiple medical imaging modalities. For this reason, RT implies the setup and the management of complex flows of images between the various modalities and software of the hospital. Even though biomedical images are most commonly stored and transferred using the DICOM standard, it remains hard to automatize and optimize these clinical flows that are very specific to each hospital. This stems from the fact that programming the DICOM network protocol requires both a high level of familiarity with the DICOM standard as well as substantial experience in computer programming. This motivates the introduction of the Orthanc software in the medical practice to improve the RT imaging workflow. [less ▲]

Cardiac toxicity is one of the most concerning side effects of anti-cancer therapy. The gain in life expectancy obtained with anti-cancer therapy can be compromised by increased morbidity and mortality associated with its cardiac complications. While radiosensitivity of the heart was initially recognized only in the early 1970s, the heart is regarded in the current era as one of the most critical dose-limiting organs in radiotherapy. Several clinical studies have identified adverse clinical consequences of radiation-induced heart disease (RIHD) on the outcome of long-term cancer survivors. A comprehensive review of potential cardiac complications related to radiotherapy is warranted. An evidence-based review of several imaging approaches used to detect, evaluate, and monitor RIHD is discussed. Recommendations for the early identification and monitoring of cardiovascular complications of radiotherapy by cardiac imaging are also proposed. [less ▲]

Background and purpose: Optimizing communication between radiotherapy team members and patients and between colleagues requires training. This study applies a randomized controlled design to assess the efficacy of a 38-h communication skills training program. Material and methods: Four radiotherapy teams were randomly assigned either to a training program or to a waiting list. Team members’ communication skills and their self-efficacy to communicate in the context of an encounter with a simulated patient were the primary endpoints. These encounters were scheduled at the baseline and after training for the training group, and at the baseline and four months later for the waiting list group. Encounters were audiotaped and transcribed. Transcripts were analyzed with content analysis software (LaComm) and by an independent rater. Results: Eighty team members were included in the study. Compared to untrained team members, trained team members used more turns of speech with content oriented toward available resources in the team (relative rate [RR] = 1.38; p = 0.023), more assessment utterances (RR = 1.69; p < 0.001), more empathy (RR = 4.05; p = 0.037), more negotiation (RR = 2.34; p = 0.021) and more emotional words (RR = 1.32; p = 0.030), and their self-efficacy to communicate increased (p = 0.024 and p = 0.008, respectively). Conclusions: The training program was effective in improving team members’ communication skills and their self-efficacy to communicate in the context of an encounter with a simulated patient. Future study should assess the effect of this training program on communication with actual patients and their satisfaction. Moreover a cost-benefit analysis is needed, before implementing such an intensive training program on a broader scale. [less ▲]

Purpose <br /> <br />Studies showed beneficial effect of light therapy on well-being at work. Our randomized cross-over study aimed at analyzing the effects of light exposition with the use of Luminette® in a hospital department without access to natural light. <br />Materials and methods <br /> <br />The study design proposed an alternation between two periods of four weeks of use of Luminette® and two periods of four weeks without Luminette®. After every period, participants completed questionnaires (sociodemographic data, seasonal and general depression, anxiety, quality of sleep, slumber and general health). <br />Results <br /> <br />Twenty-five persons participated in the study (average age = 36.5, SD = 7.7). The sample showed several benefits after one month of Luminette®: diurnal slumber (P = 0.046), general health perception (P = 0.026), physical functioning (P = 0.042), pains (P = 0.023) and role limitations due to emotional problems (P = 0.013). One month later, certain benefits remained without light therapy: diurnal slumber (P = 0.028), pains (P = 0.044) and emotional problems (P = 0.042). <br />Conclusion <br /> <br />This study has showed that the use of Luminette® could lead to similar positive results to those obtained with light therapy in other studies. This study has confirmed that Luminette® could have a positive effect on well-being at work. [less ▲]

Purpose The distance between the patient's home and a radiotherapy department may represent a hurdle for the patient and influence treatment choice. Therefore, it is necessary to check whether the geographical distribution of radiotherapy centers is in accordance with cancer incidence, taking also into account the cost of travelling to the radiotherapy department. The objective of this study is double; first, to map the current locations of radiotherapy centers across the country and second, to evaluate the observed spatial disparities with appropriate tools. Materials and methods A model of operational research (P-median) is used to suggest the optimal locations and allocations and to compare them with the current situation. This is an exploratory study with simple inputs. It helps to better understand the current geographical distribution of radiotherapy centers in Belgium as well as its possible limitations. Results–conclusion It appears that the current situation is on the average acceptable in terms of accessibility to the service and that the method presents huge potentialities for decision making so as to yield a spatial system that is both efficient and equitable.
